Employee was tightening a bolt on a bar disintegrator when the wrench slipped off hitting the employee in the mouth causing a small cut on the upper part of the chin and a chipped tooth.
Injured EE reached for falling screwdriver. Index finger caught between screwdriver & equipment housing. Resulting in fractured index finger.
Two maintenance technician were replacing a rotary lock motor. As the motor was stuck a sledge hammer was used to tap loose the motor. However when one of the technicians struck the motor with the sledge hammer not realizing the other technician apparently had placed left hand under the motor to support it when left thumb was struck.
As the employee was finishing barring out a crude hopper, they went to put the bar away and felt a pop in right shoulder, and pain afterwards.
Was performing end of shift housekeeping duties. Pinched thumb in between cleaning rake and metal frame of conveyor system.
Employee was repairing a broken airline on top of tank #6. Employee was cutting new airline to fit in the location. Knife employee was using went through the airline and contacted the employee's left thumb with the open blade.
The employee snipped the bands off a pallet located in the bed of a pickup with a wire snips and felt a twinge in their back. At first appointment on 12/5, the provider restricted the employee to desk work until an MRI was completed, but did not indicate it was work related. The neurologist provided a report on 2/5 that said it is work related.
Employee was cleaning rail cars. Employee came to a door that was difficult to open and pushed on the bar employee was using harder. The door freed up and the employee hit the ground with left shoulder.
Maintenance Electrician pinched right index finger between pipe wrench and bench vice while tightening a pipe fitting causing a laceration requiring 3 sutures.
Employee cut left middle fingertip when attempting to cut an airline hose. The injury required 5 sutures. There were no restrictions.
The employee was removing the tin end of a condenser containing a calcium crown when employee's tooling slipped and employee hit employee's left forearm against the adjacent crown causing a laceration. The employee was not wearing forearm cut protection as required by standard work.
The employee was pushing bags of lime briquettes into furnace retorts using an ~ 20' pushing rod. The rod caught on the inside of the retort and stopped suddenly causing a sprain/strain injury to the employee's right shoulder.
Employee had an accident yesterday while ee was assembling a electrical cord. EE was using a screwdriver to tighten a screw when the screwdriver slipped and punctured hand. EE immediately cleaned it and applied a dressing, but today hand is quite swollen.
While employee was attempting to open a fuller valve with a cheater pipe the pipe hit a beam. This caused kiln dust to fall from the beam into the left eye of the employee. The employee was treated by a local eye doctor and was given polysporin Ophthalmic Ointment. EE returned to work for EE's next scheduled shift.
Employee pinched finger between tool and railroad tracks
Employee was using a pry bar as a hammer and felt their shoulder pop, but said they were fine. Their shoulder started hurting worse on 8-16-16 so they were taken to the dr for evaluation. Dr put employee on restrictions until seen by orthopedic dr.
Worker was scaling high wall using rope access methods. EE stacked excess rope (tail) on the ground above EE. EE needed more rope & pulled the tail and the tail dislodged a small rock. Rock hit EE in the nose. Ground area where rope was stacked was not cleaned properly.
Employee was using an air lance and pry bar to break material free in the impactor chute. The material broke free and employee hit elbow on the top of the chute door causing bursitis on the elbow.
An Employee was attempting to remove a stone clog in the SCD screen number 131. EE used three tools to remove the clog; shovel, hoe and steel pipe. During the process ee reached for the shovel, which had fallen to the floor, and stepped on the blade of the hoe kicking the handle up off the floor into face causing left side eye tooth to puncture upper lip.
Employee bends over to loosen some bolts on a mill and felt a strain in his mid to lower back.
The employee was beating the cone at the bottom of tank #6 to loosen mud that had accumulated at the bottom of the tank. The head of the hammer glanced off the side of the tank. Because of this, he hit his left hand on the guard rail of the catwalk. This caused severe pain, numbness, and swelling in his left hand.
A DMS builders LLC employee cut his leg and required stitches resulting from cutting plastic pipe.
Operator was working to unplug 273 feed bin. He was working on the bin for over an hour. When material started to flow, he used a sledge hammer to close the hammer gate. He tapped the gate side to side to close the gate. He lost his footing and caught his fingers between the hammer gate and sledge hammer handle. He sustained a broken left index finger and stitches.
Using a ratchet wrench with an extension bar was being used to loosen hold down bolts on the chipper blades. The employee was pushing the wrench and lost his grip. His hand struck against the metal frame around the chipper, causing a laceration and fracture to the 4th finger of his left hand.
Employee pulling on a chain-fall, when weight shifted on chain-fall the pull chain was pulled from employees hand. The employee tried to hold the chain, but it was pulled from his grasp causing injury to his right bicep muscles.
Temporary staffing employee was using a razor knife and cutting excess tape from a vacuum hose when he accidentally cut his left thumb. He sustained a superficial laceration to the left thumb.
Operator was pushing a charging rod into a retort when he felt a strain in his neck. Operator uses a 15 foot pole to push six 50 pound bags of material into a 12 foot long retort. When the operator was pushing the 4th or 5th bag into the retort he felt a strain in his neck.
The employee was trying to unplug the log chipper that is used to chip calcium metal with a rod. After trying to repeatedly unplug it the employee stopped and went downstairs to shut the chipper down, that is when he felt pain in his lower back.
While removing waste rock from shovel bucket, rock cut lower right leg
Employee was knelling down cutting belting. The knife he was using went through the belt into his right thigh.
Screen stone building - while cutting rubber belt skirting to size, box opener type razor blade knife slipped and cut left pinky finger. Reportable due to stitches and restricted work duty.
While cutting banding material on a bundle of pipe, banding popped up, hit leather glove first, then up left arm, and cut left forearm.
EE was shoveling out (removing spent briquettes) furnace #9, when the shovel stuck going into retort, stopping quickly. The handle of the shovel struck EE in the chin. Reportable due to stitches.
Employee was shoveling/removing spent calcium alumivate briquettes from the furnace and when he went to push cart with shovels to next furnace he got foreign material in his eye. He flushed but still irritated so he went to eye doctor (reportable due to antibiotic prescribed).
Employee was operating a chain jack which was attached to the underside of the floor above where the employee was working. He was sitting on a gate approximately 3' below the point of attachment, not giving him much room to work.
Crew was bull rigging a 10' piece of pipe up two flights of stairs when the jack stand rolled from under the pipe. Employee was holding the pipe with his hand when jack stand started to roll. Employee let go and put his hand around the front of the pipe where his (L) hand was pinched requiring stitches to his 3rd finger.
Employee felt pain in his lower back after pushing on a wrench to loosen an electrical termination.
While trying to open a jammed screen gate with a come-along, employee applied force and the gate sprang open and smashed finger between come-along handle and building brae. (Reportable due to draining fluid from under fingernail.)
Employee was holding a pin while another employee was using a sledge hammer and the hammer glanced off and crushed his finger.
Employee was placing an empty pallet on a roll table that presents pallets to a palletizer when his finger became pinched between pallet and pallet alignment bar.
EMPLOYEE WAS REPAIRING #2 RAYMOND MILL. HE WAS POUNDING ON A PINION GEAR WITH A HAMMER TO REMOVE THE SHAFT WHEN A SMALL FRAGMENT OF METAL CAME OFF THE PINION GEAR AND HIT HIM IN THE CHEST CAUSING A SMALL PUNCTURE WOUND.
While working on pipe valve wrench hit employee in face.
EMPLOYEE WAS SHOVELING 1/4" PRODUCT ON FLOOR OF DRIER BUILDING. EMPLOYEE STRUCK SHOVEL BLADE INTO CONCRETE CURB WHILE SCOOPING PRODUCT. EMPLOYEE FELT PAIN IN HIS LEFT FOREARM AND WAS RELEASED TO MODIFIED DUTY WITH NO USE OF LEFT ARM AND UPPER LEFT EXTREMITIES. EMPLOYEE RETURNED TO MD ON 10/17/02 AND WAS RELEASED TO FULL DUTY.
EE WAS REMOVING A BEARING FROM AN EXTRUDER WITH A PRY BAR. THE PRY BAR SLIPPED OFF OF THE BEARING AND HE HIT HIS RT. ELBOW ON THE DISCHARGE CHUTE. HE WAS TAKEN TO MEDICAL CLINIC. X-RAYS WERE NEGATIVE. HE WAS PLACED ON RESTRICTED WORK AND HAS A FOLLOW-UP APPOINTMENT ON 4/8/02.
EE WAS USING A LARGE WRENCH TO TIGHTEN A NUT. EE FELT A PAIN IN HIS LEFT SHOULDER. EE WAS SENT TO MD FOR TREATMENT & WAS RELEASED TO RESTRICTED DUTY. LIGHT USE OF LEFT ARM & HAND. EE WAS GIVEN PRESCRIPTION DRUG FOR PAIN.
MECHANIC INSTALLING NEW ROTARY LOCK FOR BAGHOUSE, WRENCH SLIPPED CAUSING LEFT INDEX FINGER TO STRIKE ROTARY LOCK.
EE WAS USING SLEDGE HAMMER TO UNPLUG A HOPPER & FELT PAIN IN HIS BACK. EE RECD MED ATTENTION A FEW DAYS LATER HE IS NOW WORKING WITH A LIFTING RESTRICTION RECEIVING PHY THERAPY & FOLLOW UP MED ATTENTION. RETURN TO FULL CAPACITY IS UNK. BUT CONTINUES TO WORK IN HIS REGULAR JOB WITH SOME LIMITATIONS
EMPLOYEE TRYING TO LOOSEN NUT WITH A WRENCH ON #7 CLADDER TAKE UP SPOOL AND FELT PAIN IN HIS BACK.
EE WAS CLEANING COMPACT WHEEL. HE HIT HIS FINGER WITH HAMMER WHEN HIS HAND SLIPPED FROM CHISEL.
