Miner was welding with a welding hood/shield on. When EE was done, EE removed EE's hood and began to knock extra slag off with a hammer. When EE struck the weld a shard projected back and struck EE's eye. EE did not feel this until several days went by and did not go to the doctor until 7/1/2025.

While tightening a toggle bolt on jaw crusher, the bolt sheared off causing the wrench to spin and struck finger. Laceration to finger needing stitches.

Coworker was using a bar to loosen a piece of stuck material on the floating conveyor. When they struck the material with the bar a small piece ejected and embedded into employee's left forearm. Will need surgery to be removed - pending!

After lifting an oversized rock from a jaw crusher, the rock was placed on the ground. Upon placement it rolled a quarter turn causing the lifting strap to lay across the employees hand which was resting on the handrail.

Miner was cutting a 1 1/2" rubber water hose for the mud classifier when the knife EE was using slipped and cut the tip of EE's finger. No bone was amputated, however the piece of skin from the tip of EE's finger was removed.

Miner was working to free a motor that appeared seized. The pipe wrench being used to spin the sprocket broke loose, smashing the Miners right ring finger. Laceration was vertical from the front of the finger, over the top and down the back side. The miners fingernail was removed and 7 stitches were used.

Miner was shoveling under locked out conveyor in a tunnel with a spade shovel and had employee's right hand on the but of the shovel. When bringing arm back, Miner struck side of tunnel and pinched right pinky finger, removing skin from end of finger, requiring medical attention.

Miner was replacing a teeth on a excavator bucket. One tooth was stuck so miner used a hammer to loosen the tooth from the shank. When struck a small piece of the tooth broke off and struck the miners right forearm causing a puncture wound.

Worker was taking turns with other workers swinging 20lb. sledge hammer to tighten nut on HP-4 crusher cone. Each worker was swinging about 10 times and switching off. This was EE's second time swinging and he made about 8 out of the 10 swings they were making. He stopped and said he felt his back strain. There were 5 miners taking turns.

Employee was under the pontoon, on his back, in a ditch, tightening dredge bolts to 200 ft. lbs. torque. Employee has right shoulder issues. THE EMPLOYEE DID NOT SEEK MEDICAL ATTENTION UNTIL 1/10/12 WHEN HE SAW A CHIROPRACTOR.

Worker was installing a tooth onto the shank of a loader bucket. In order to align the pin the worker tapped on the tip of the tooth with a steel hammer. While doing that a piece of the tooth chipped off and struck the worker in the abdomen. He went to the doctor and they found that the metal chip had went through his coveralls and shirt and was embedded in his abdominal wall.

Miner was doing maintenance work and decided to assist another miner to move a Lower concave ring. The injured miner placed the end of a 6' pry bar onto a hole in the concave ring. As he attempted to move the ring, the bar slipped and the miner fell to the ground spraining his right ankle.

Worker was changing a tail pulley on a conveyor. He was using a 2-lb. hammer to align the equipment. The worker raised the hammer slightly and felt something in his lower back.

Worker was using a razor knife to cut a piece of conveyor belt. Worker lost control of tool and inflicted a 5cm cut to his left forarm.

The miner was using a 6' pry bar to apply pressure to a part. The bar slipped & the miner fell to a sitting position. There was a 2" vent cap sticking up. The cap struck the miner on the tail bone. On 06/19/2006 the miner told a supervisor he felt he should see a doctor. We imediately conveyed him to see a doctor.

While perfoming maintenance and tightening a bolt the wrench slipped and employee's wrist struck an angle iron. The metal contacted his right wrist between his long sleeve shirt and his glove causing a small 1/2 inch laceration.
