Coworker was using a bar to loosen a piece of stuck material on the floating conveyor. When they struck the material with the bar a small piece ejected and embedded into employee's left forearm. Will need surgery to be removed - pending!
Miner was hosing material from floor of wash pant tunnel when EE stepped backwards and rolled EE's right ankle.
Strained back while changing screens on crusher.
Employee fell from ladder while performing maintenance activities. The fall resulted in a ruptured disc in EE's back which required surgery.
Miner was climbing into the cab of a front end loader and missed a step. EE stated this caused injury to the back. After the shift EE was seen by the Chiropractic Doctor, who excused EE from work, and released EE to full duty without limitations on 9/19/2016.
Employee states he had been lifting wooden crane pads and back was strained
Employee states he stepped in a hole on the surface he was working on. Employee states he fell and reached out with right hand and put strain on his shoulder. Employee DID NOT seek medical attention until on/about 8/19/13
Employee was under the pontoon, on his back, in a ditch, tightening dredge bolts to 200 ft. lbs. torque. Employee has right shoulder issues. THE EMPLOYEE DID NOT SEEK MEDICAL ATTENTION UNTIL 1/10/12 WHEN HE SAW A CHIROPRACTOR.
Worker was pulling on a bar when the bar broke. Sudden weight shift caused worker to run back of hand across steel guard.
