Struck against a moving object

Dozer traveled up berm to go around wrong side of hoe and tipped to other side of berm, slid approx 10' landing on side/top. Wearing seatbelt, self exit. Dislocated shoulder, EMS called but no transport. Released to Modified Duty. Cylinder damaged on machine and will replace seatbelt.

Re-installing hydraulic motor on power screen with the assistance of another employee. Miner felt some pain in shoulder and sought medical attention. Miner was given 2 days off before returning to work.
