Fall to the walkway or working surface

EE was adjusting the press when EE stepped back and slipped, falling to the ground. EE injured EE's ankle, knee and back and was put on light duty. Diagnosis pending.

Over-exertion in pulling or pushing objects

Miner pulled himself up by grabbing onto a pipe and felt a pop in their hand.

Roller shaft slipped during maintenance activity and smashed miner's thumb

Miner was working to free a motor that appeared seized. The pipe wrench being used to spin the sprocket broke loose, smashing the Miners right ring finger. Laceration was vertical from the front of the finger, over the top and down the back side. The miners fingernail was removed and 7 stitches were used.

Climbing down vertical ladder off of the filter press. Employee got a sharp pain in wrist and thumb area of left hand.

Miner was walking on wet concrete surface when he slipped and landed flat on his back. Resulted in Buttock Contusion/ sprain. X-ray taken and miner was put on light duty until X-ray could be reviewed. X-ray was negative and worker was returned to job of injury.

Fall from machine

Miner was doing maintenance work and decided to assist another miner to move a Lower concave ring. The injured miner placed the end of a 6' pry bar onto a hole in the concave ring. As he attempted to move the ring, the bar slipped and the miner fell to the ground spraining his right ankle.

Two miners were installing a control center cover. One miner accidentally allowed the cover to slip and that pinched the other miners left index finger between the cover and the machine. This caused a laceration which required stitches.

Operator complained of pain in left shoulder. Operator stated that condition was ongoing for some time. The operator asked that he run a different piece of equipment to relieve the symtoms and we complied with his wishes.

Employee slipped and fell onto posterior and left shoulder. Level concrete paved surface. Slippery due to inclimate weather.

Worker was using a razor knife to cut a piece of conveyor belt. Worker lost control of tool and inflicted a 5cm cut to his left forarm.
