Employee was removing cast liners within rawmill for replacement. Using a sledge hammer to loosen the first liner, employee struck corner of liner and a piece fractured and came back and struck employee in the nose causing laceration and broken nose.

Employee continued to work unrestricted until EE went to an orthopedic specialist on 12/30/2025. EE was diagnosed with a tear of the right rotator cuff. The extent of the tear is unspecified. The employee is now on restricted duty.
Employee was trying to beat a slide gate on a silo closed using a sledgehammer. Cement sprayed into their eyes and knocked off their safety glasses. Employee was treated at the Hospital Emergency Department.
Employee was cutting a hose fitting off an airline and the knife slipped and lacerated EE's left pinky finger. EE received 3 sutures to close the laceration.

Employee received a broken finger while using a pry bar to dislodge a piece of rock. The bar slipped off the rock and smashed his finger.
Employee was raising jack stand on the fuel tank. The hand crank slipped off the jack and struck the employee in the nose. Employee will have surgery on 9/16/10 to repair his nose.
EE was cleaning sticky coal from the buckets in the coal elevator. While cleaning a bucket, a piece of coal fell and struck him on the arm.
The original injury date was 3/28/09 when the employee was tightening an overhead bolt on the crusher and he felt a pain in his right shoulder. The employee was not placed on restricted duty or incur any lost time as a result of this injury until 5/20/09 when he had surgery.
A Bridgeburg employee from the Cowansville Shop was removing the parking brake from a CAT 990 with Come-A-Longs. One of the Come-A-Longs and a nylon strap slipped, and the unit pinched the workers forearm. The worker went to a physician the following day and was put on lite duty.
EE was cleaning spilled clinker from under #1 cooler. As he was pulling the broom, he hit the clloler framework with his left forearm.
While pounding steel with a hammer, the man had a steel splinter in his right thumb which was removed at the emergency room.
The injured man had a steel splinter in his left eye (no witnesses) late on shift. Went to doctor on Sat & had splinter removed. Said he did not know it happened until next day.
The injured was sledging a steel pin. A steel splinter hit and cut is right arm causing a small wound. The Emergency room Dr. gave antibiotics to treat for infection. No lost time or other treatment needed.

The injured man was helping a mechanic remove a water pump from his rock truck. The mechanic bumped the man on the lips with a wrench. A month later the injured man's dentist noticed that his crown post was loose. We were notified of the incident on 5/8/06.
A SMALL METAL SLIVER FROM HAND CHISEL EMBEDDED IN RIGHT THIGH. THE SLIVER WILL BE LEFT IN THE THIGH UNLESS COMPLICATIONS ARISE IN THE FUTURE. EE WAS GIVEN AN ANTIBIOTIC & IS BACK TO WORK WITH NO RESTRICTIONS.
The injured man received one stitch in finger when a sliver of steel was removed.
