Struck by flying object

Employee was removing cast liners within rawmill for replacement. Using a sledge hammer to loosen the first liner, employee struck corner of liner and a piece fractured and came back and struck employee in the nose causing laceration and broken nose.

Caught in, under or between a moving and a stationary object

Employee was cleaning the coal elevator and EE dropped EE's hammer into the elevator. EE removed the lower access panel to reach in and retrieve EE's hammer. When EE reached in, the control room operator started the elevator. The employee received abrasions on EE's left forearm.

Struck by flying object

Employee was walking through the plant to take inventory when dust blew into EE's right eye. The employee was wearing safety glasses. EE was prescribed an antibiotic for a corneal abrasion from the ACMH Hospital Emergency Department.

Absorption of radiations, caustics, toxic and noxious substances

Employee was trying to beat a slide gate on a silo closed using a sledgehammer. Cement sprayed into their eyes and knocked off their safety glasses. Employee was treated at the Hospital Emergency Department.

Employee was carrying grizzly bar during a remove and replace job task and steel bar fell from EE's grasp and landed on top of EE's left foot.

Employee was walking through the facility and hit right hand on a platform which caused a contusion/bruise.

The employee was cutting a metal plate using a grinder equipped with a metal cutting wheel. A piece of debris went under EE's face shield & safety glasses & landed in EE's left eye.

Employee was blowing down bin and when they turned to take a step, they tripped over their own foot and fell down. Employee landed on their right shoulder. Employee was diagnosed with a right shoulder strain and possible rotator cuff tear.

Employee was sitting on a bucket in the pack house building watching a pump amperage meter. EE had a heart attack which caused them to fall off the bucket. MSHA was notified via the 800 number and an inspector was sent to investigate. This does not appear to be a work related injury.

Employee was climbing into the front end loader when they fell backwards and hit the back of their head on the concrete. The fall caused a concussion and a laceration which required 7 staples. The employee can't remember what caused the fall.

Employee was using a come-a-long to move an approximate 2,000 lb 5'X8' railroad street cart back onto tracks. Employee felt a strain in lower back with pain.

Cleaning chute on Tyroc screens at crusher. Lost balance and fell on shoulder. Did not miss any work until weeks later injury bothered him and scheduled surgery for 6-16-2015.

EE was pulling in an upward motion trying to loosen a flange on top of the cement silo. ***EE missed no work due to this incident until he had rotator cuff surgery. First day work missed was 4/18/13.

The Pressure Relief vent was relieved. We are not positive of the cause.

An unplanned ignition occurred in the # 2 Coal System. It was found that the Dust Collector Fan from the clinker cooler was not running. This is believed to have caused a pressure fluctuation. The flame could have traveled back into the coal system causing the ignition. The Dust Collector fan was repaired. The relief vent worked properly.

A physical altercation occurred between 2 employees.

The relief vent on the #1 coal system was not rated properly for temperature. We believe that the teflon film in the vent failed. This caused the cyclone to lose its vacuum. Somehow the erratic operation allowed the coal to ignite.

EE was opening #2 Bin Gate by pulling a mechanical lever.

Using compressed air to get bulk cement flowing and when it did, it got into his eyes.
