EE was was working on the plant. EE was applying pressure to a prybar when EE's hands slipped off and EE's right little finger was pinched by the bar, resulting in a fracture to the tip of EE's right pinky finger.

Employee was changing razor blade in utility. While removing blade from package, employee cut right thumb. Laceration required four (4) sutures.

Injured employee was holding a drift pin while a second employee struck pin with a hammer. Second employee missed the pin striking injured employee on the left ring finger. Injured employee suffered fracture and laceration.

While adjusting conveyor belt tracking, EE was using hammer to strike adjustable return roller bracket. EE missed hitting bracket causing his right arm to be drawn into belt & he became wedged between belt & roller.

Employee was assisting in replacing a bolt on primary jaw crusher. While striking a slugging wrench with a sledge hammer, employee's hand struct ajoining metal rail causing a fracture of the tip of right thumb. Employee didn't go to clinic until two days after accident.

STANDING ON THE FEED BOX OF A CRUSHER, 15' OFF THE GROUND, EE WAS REACHING ABOVE HIS HEAD TIGHTENING 2 BOLTS WITH A WRENCH. THE WRENCH SLIPPED OFF THE BOLT CAUSING EE TO FALL BACKWARD TO THE G ROUND.
