Employee states they were hand scaling and went to swing the hand bar at a rock when the bar slipped out of their hand and caught the right pinky finger between the hand bar and the top rail of the manlift basket. The pinky finger suffered a fracture.

Injured was using a wrench to tighten a bolt on a crusher. The wrench slipped off and injured's right palm struck the bolt head. The resulting laceration required 6 stitches.
Injured was using a sledge hammer to loosen wear parts on a VSI rotor. While striking a cast bar, a piece of metal separated from the bar and entered into the injured's abdomen. Injured went to urgent care and received 2 stitches.

Employee stated was using a pry bar to clear material from a chute when the bar slipped and employee caught a finger between the chute and the pry bar.

Employee stated was scaling the highwall, pulled down a piece of rock, the rock struck the bottom of the man-lift basket, but the bar was still stuck in behind the rock which caused EE to strike ribs on the man-lift railing.

Employee stated was lifting steel plate into place with chain hoist, tied the plate off to re-hook, released the chain hoist, and the chain slipped causing the chain hook to strike the employee on the cheek.
The employee was trying to unplug the screening hopper with the use of a 15 foot pipe and sledge hammer. The employee struck the pipe which came back and hit his finger. No medical treatment requested until 11-25-14 at which time he was diagnosed with a fracture.

The employee was shoveling sand and struck his elbow on a steel beam. Injury was on 10-3-14 and physical therapy was prescribed on 10-13-14 making it reportable.

Employee was using a hand held hammer to drive a 3/4" bolt, missed the mark and struck his left thumb with the hammer. He refused medical treatment until 3/4/11 at which time x-rays revealed a fracture of the IA joint.

Employee was working in bed of haul truck, knocking off anti-wear plates with a sledge hammer after the retainers were removed, while striking a plate, a piece of metal came off and struck him in the right thigh causing a laceration and imbedded into the soft tissue of his thigh.
Reports was releasing a stuck pin on the bucket of the bob cat with a scaling bar, pin released and the bucket swung forward, the bucket detached and fell, scrapping the front of his legs. Deep abrasion to his right leg approx 18" in length, and superficial abrasions to the left leg.

EE was using a 2lb sledge hammer attempting to remove a come-along safety latch that was outside the hook. He swung hammer and elbow hit guard rail causing hammer to miss latch and hit left hand causing laceration of left little finger.

A ROCK BECAME LODGED IN THE CONVEYOR BELT OF THE HEAD PULLEY OF THE 4"CONVEYOR. WHILE TRYING TO DISLODGE THE ROCK THE PRYBAR SLIPPED HITTING THE EMPLOYEE IN THE CHEEK, BELOW THE RT. EYE CAUSING A 1" LACERATION.
EMPLOYEE WAS PEELING THE PLASTIC WRAP OFF A CARDBOARD BOX WHEN THE TIP OF HIS FINGER CAUGHT THE OPEN BLADE OF THE UTILITY KNIFE. THE EMERGENCY ROOM DOCTOR GLUED THE LACERATION. THE NEXT MORNING WE SENT HIM TO THE DOCTOR TO GET THE WOUND CLEANED. THE DR PUT HIM ON RESTRICTED DUTY EVENTHO HE PERFORMED HIS NORMAL DUTIES.

EMPLOYEE WAS USING A CLAW HAMMER TO DISMANTLE CONCRETE FORMS WHEN A PIECE OF DEBRIS ENTERED HIS LEFT EYE. HE WAS SENT TO A DOCTOR AND THE DEBRIS WAS REMOVED. A PRESCRIPTION OF ZYMAR WAS PRESCR IBED TO BE USED FOR 3 DAYS.
