Employee was holding a B/O hammer and another employee was hitting it with a sledge hammer driving the wedges out of Drag rope sockets on the bucket. Employee made a bad strike causing a piece of metal to fly off the B/O hammer and lodge in EE left forearm. Employee received 2 stitches in left forearm.

Employee was traveling and examining return air course off old #1 unit and EE's ride was stuck in mud. Attempting to pull ride out with come-along it broke striking EE in mouth. Employee went to dentist and a crown on EE's tooth was broken. 4-15-2016 it was brought to my attention that a crown was broken and replaced on EE's teeth after seeing a dentist.

Employee was pushing on leveling bar with channel locks, with mechanic. When my back got tight, and started to hurt. When he went to stand up straight he had a sharp pain in my lower middle back.

Employee was cutting the plastic top off of a belt chain bucket with a belt knife. The knife slipped resulting in a cut on lower right leg. The cut did not require stitches, the cut became infected and went to the Dr. and received antibiotic to treat the infection.

The employee was assisting a contract mechanic install a track on a dozer, the contract mechanic hit the alligator link on the track with a sledge hammer and a piece of the hammer splintered off and hit the employee in the arm.

Another employee was driving a bucket tooth wedge in with a sledge hammer when a piece of the wedge being struck became a projectile and struck employee in his right eye. employees were removing bolts on the end of a cutter drum using a torque multiplier and a long ratchet. There injured employee had set the multiplier on a bolt and put his hand on the equipment to stand. At this point the other employee pulled the ratchet down pinching the injured employees finger between the piece of equipment and the ratchet.

Employee was operating a roof bolter and as he was drilling a hole, in the mine roof, the pinner steel stopped up. To unstop the drill steel he struck it with a pinner wrench. The pinner wrench slipped from his hand causing it to bouce backwards striking him in the right eyebrow. He received 7 sutures.
