The injured employees wrench slipped resulting in the ring finger of EE's left hand being struck against some angle iron or conveyor frame.

The employee was using a utility knife to cut a piece of conveyor. The knife slipped and lacerated the employee's right leg.

Employee states he was prying on a wear plate that was to be removed from a front end loader bucket. The pry bar broke resulting in it striking him above the lip.

Employee hit his left ring finger with a hammer trying to knock a pin out of a roller.

CLEANING ROCK OUT OF TAILPULLEY, PRY BAR SLIPPED AND HIS HAND HIT THE FINS IN THE PULLEY, CUTTING HIS LEFT POINTING FINGER.

WHILE USING A PRYBAR TO ALIGN A ENGINE GOING INTO A BACKHOE, THE PRYBAR SLIPPED AND CAUGHT HIS LEFT LITTLE FINGER BETWEEN THE PRYBAR AND THE ENGINE.CONTUSION AND LACERATION TO THE LEFT LITTLE FINGEER AND BROKE THE END OF FINGER.

USING A 6'PRY BAR TO FIX PROBLEM IN FEEDER, BAR SLIPPED & CAUGHT HIS LT. THIRD FINGER BETWEEN BAR & FRAME.
