Injured person was setting tamp-in steel anchors in holes drilled into cement. Hammer deflected off of setting tool and struck the individual's thumb. The blunt force trauma caused a laceration requiring stitches.

EE was ensuring bucket pin did not contain pressure. Employee was using a pry bar to test for movement when the pin moved and the pry bar in a glancing blow struck the employee on the side of the forehead causing a laceration.
Employee struck finger with a hammer
While hammering a new tooth on a new shank on a 980H a piece of the tooth broke off striking him in the forearm. The metal chip penetrated the skin and lodged about 3 inches up his arm. The employee required 2 stitches and will need to return to the doctor to have the chip removed.
Employee was repairing welding gun. The welding gun was in left hand. The left hand was behind screw being removed. Force was applied to screwdriver to remove screw, when the tool slipped from the head of screw. The force being applied caused the tool to puncture left hand.
Employye was using a utility knife to cut a piece of grease line off a bulk roll, the blade of the knife came out of the holder a cut the employees right thumb(employee is left handed) . The employee recieved three stitches in his right thumb and returned to duty.
WHILE CUTTING A PIECE OF CONVEYOR BELT WITH RIGHT HAND, THE RAZOR KNIFE SLIPPED AND CUT EE'S LEFT INDEX FINGER.
Employee was working on a tail pulley when the comealong that was being releases failed and pinched his thumb inbetween the bearing and take-up frame.
WHILE STRIKING A BAR WITH A HAMMER MISSED STRIKING RIGHT INDEX FINGER.
EE WAS STANDING ON THE REAR PLATFORM OF THE WATER TRUCK TRYING TO LOOSE THE PUMP CAP WITH A WRENCH. WHILE PULLING ON THE WRENCH, THE WRENCH SLIPPED OFF THE CAP CAUSING THE EE TO LOOSE HIS BALA NCE AND FELL APPROX. 36" TO THE GROUND. THE EE FELL ON HIS LEFT SHOULDER AND WRIST, SUFFERING A FRACTURED LEFT WRIST.
DRIVING IN WEDGE BOLT, EE MISSED WEDGE BOLT & HIT A WRENCH ON THE FLOOR. THE WRENCH POPPED UP & STRUCK EE IN THE MOUTH. EE NEEDED 3 STITCHES.
EE WAS CLEANING OUT PRIMARY CRUSHING PLANT. STONES SLID & STRUCK HIS RIGHT KNEE.
EE WAS TRYING TO KNOCK THE BOWL LINER FROM THE CONE WHEN HE STRUCK HIS SMALL LEFT FINGER WITH A HAMMER FRACTURING THE FINGER TIP.
EE WAS WORKING TO UNHOOK CHAIN AND OVER-EXTENDED HIMSELF STRAINING HIS LEFT SIDE.
EMPLOYEE ALLEGES WHILE WORKING ON A CHUTE A HAMMER SLIPPED OUT OF HIS HAND AND STRUCK HIM IN THE LEFT LEG IN THE SHIN AREA.
