EE was installing a thrown track on a D10R when a job mechanic struck metal on metal with a sledge hammer resulting in a metal shard puncturing EE's chest.
Employee was closing open doors on train cars, prying them shut with a bar, when bar slipped out of door, releasing pressure on bar causing employee to stumble, causing pain to shoulder, back, & neck area - On 1-6-14 EE was treated in the ER with a return to work with no limitations. On 2-7-14 EE was seen by a specialist. On 2-10-14 EE would be off work for 3 weeks.
Employee was breaking a track on a D-10 T Dozer, striking the Cat with a hammer with one hand & had his other hand resting on track, Track moved, catching his finger and mashing it. Received 4 stitches and tore fingernail off.
The employee stated that he was removing a breaker box cover and he had his knife in his right hand and it was open, the cover started to slip so he let go of the knife and it fell striking him in the left thumb cutting the bottom side of his thumb.
Employee was extracting a bearing out of the hardbar on a 475 Komatsu Dozer using a hammer. The bearing released, twisted, and caught the employees right hand causing a laceration requiring six stitches.
Employee was using a sledge hammer to remove a tooth off a loader bucket of a 994. A piece of metal flew off and penatrated the left knee. This caused a lot of bleeding and surgery to remove the imgesef piece of metal.
EE was putting chains on a motor grader using a hammer. While striking the chain roll pin, a piece of the hammer broke off and struck the EE in the bottom of the right eyelid. A sliver of metal was removed by a doctor; glue was used to seal hole in right eye-ball.
EE was cutting a piece of water hose with a pocket knife and cut the top of his left thumb nail. Did not miss work, cut required stitches.
EE was helping others replace springs pads on 800 Mack truck (ST140), when one of the jacks kicked out and caught ee right index finger between spring and crib block.
Employee was lifting a heavy component using his boom, when the chain he was using slipped off mashing his right ring finger. Required stitches.
Employee was putting drive line bolts back in a 980G loader. The rachet slipped off the bolt head, striking the employee on the left side of his nose and corner of his left eye. (stitches)
Employee was putting a wear plate on bottom of 994 loader bucket using a pry bar to lift the plate. The bar slipped, mashed right hand.
Individual was pulling on a wrench in order to loosen a bolt when he felt a "pop" in the neck / shoulder area.
Employee was using 2 come-a-longs to align the top plate of the carousel so it could be welded. One of the come-a-longs slipped while employee's hand was under the plate and mashed his right ring finger requiring seven stitches.
Employee was blocking up loader to fix flat tire, used a metal strip, this strip came out when the loader set down striking the employee in his chest and bottom of his chin.
EE was using a pry bar to align the push-arm of a dozer, he felt pain across his left shoulder down to his thumb.
Employee was raising major bogie using a come-along to install rollers. The come-along slipped, allowed the bogie to fall mashing his left hand between the bogie and the dozer frame.
Employee using come-along, both hooks were pulled open. The force released caused him to smash the end of his little finger on his right hand. Required surgery and two stitches.
EMPLOYEE WAS PRYING ON A RETAINING RING. THE BAR HE WAS PRYING WITH SLIPPED OFF HITTING THE EE OVER HIS RT. EYE, CAUSING A CUT THAT REQUIRED (5) SUTURES.
EE working on a pump wrench slipped and hit him above the right eye making a small cut. Required sutures. EE did not miss work.
EMPLOYEE WAS USING A 12 LB SLEDGE HAMMER WHILE WORKING ON AN UNDERCARRIAGE. HE EXPERIENCED A SHARP PAIN IN HIS LOWER ABDOMEN.
Employee was standing on two hydraulic hoses tightening a bolt when the extention to his wrench broke causing him to fall. He caught himself with his left arm, causing him to injure his left shoulder
REMOVING RADIATOR OUT OF MACHINE STANDING ON LADDER WITH PRY BAR WEDGING RADIATOR OUT OF SOCKET, PRY BAR SLIPPED OFF RADIATOR. STARTED FALLING OFF LADDER, TURNED & JUMPED TO PREVENT FURTHER IN JURIES, LANDED ON LEFT LEG, BRUISING FOOT & KNEE.
EE STATES HE WAS PUTTING FUEL INE ON TRUCK ENGINE WHEN THE WRENCH SLIPPED STIKING RIGHT HAND.
EMPLOYEE WAS PULLING ON A WRENCH TO LOOSEN A BOLT WHILE WORKING ON A DOZER. HE FELT A PULL IN HIS LOWER BACK ON THE RIGHT SIDE.
EE WAS WORKING ON A MINOR BOGIE & ROLLER ON D-30DOZER THE BOGIE SHIFTED FROM ITS POSITION ON THEHYDRAULIC JACK. THIS PINNED THE EE HAND BETWEEN THE GROUND AND THE AIRGUN. THE EE CUT HIS FINGER AND REQUIRED STITCHES.
EE WAS REMOVING TRANSMISSION FROM A D-11 CAT DOZER, KNOCKING A PIN OUT WITH A SLATE BAR. WHEN THE PIN CAME OUT, COUNTERWEIGHT MOVED CATCHING END OF BAR. BAR CAME AROUND AND HIT HIM IN THE JAW. HE BROKE HIS RIGHT JAW AND REQUIRED SURGERY.
EE WAS HELPING MECHANIC AND USING A HAMMER AND CHISEL... GOT A PIECE OF METAL IN HIS RIGHT EYE.
WHILE WORKING ON A 992G TOP BOOM PIN, LINING THE PIN UP TO INSTALL IN THE BEARING, EMPLOYEE FELT A "POP" IN HIS RIGHT WRIST.
EE WAS TAKING DRAIN PLUG OUT OF A 988 WHEEL UNTI USING BREAKER BAR AND SOCKET. THE PLUG WOULD NOT COME OUT SO EE WAS HITTING THE BAR WITH A HAMMER TO HELP LOOSEN IT, STRUCK HIS FINGER WITH THE HAMMER.
EMPLOYEE WAS CUTTING A 5 GALLON BUCKET WITH A RAZOR KNIFE AND THE RAZOR SLIPPED AND JABBED HIM IN THE RIGHT THIGH. THIS REQUIRED 4 STITCHES.
EE WAS HELPING ANOTHER DISASSEMBLE A JACK ON A DRILL. THEY WERE BOTH PULLING ON A PIPE WRENCH WHEN THE WRENCH SLIPPED AND EE PULLED HIS BACK.
EE WAS USING A SLAP HAMMER ON THE DOZER BLADE HE WAS WELDING ON TO KNOCK OFF THE METAL THAT WAS LOSE A PIECE OF SLAGG METAL CAME OVER THE TOP OF HIS HELMET INTO HIS EYE.
EE WAS CLEANING WELD SLAG WITH A WIRE BRUSH AND SOMETHING HIT HIM IN THE LEFT EYE.
EMPLOYEE PICKED UP A TONG WRENCH THEN TURNED AND WRENCHED HIS BACK. HE FELT SEVERE PAIN IN LOWER LUMBAR SECTION.
EMPLOYEE WAS SETTING A D11R ENGINE USING A PRY BAR TO MOVE THE ENGINE OVER WHEN THE BAR SLIPPED AND STRUCK HIS HEAD ABOVE THE RIGHT EYEBROW.
CUTTING CABLE WITH HAMMER TYPE CABLE CUTTER, A PIECE OF METAL CHIPPED OFF CUTTER AND HIT EMPLOYEE IN LEFT INNER THIGH. CHIP WAS EMBEDDED ABOUT 4".
WHILE DRIVING A BEARING IN A BLADE PIN BOSS, EMPLOYEE HAD A PLATE ON TOP OF THE BEARING AND WHEN HE STRUCK THE STEEL PLATE WITH HAMMER A PIECE OF METAL CAME OFF AND PENETRATED HIS ARM.
EMPLOYEE WAS CHANGING TEETH ON A LOADER BUCKET. ANOTHR EMPLOYEE WAS DRIVING THE RETAINING PIN WHILE EE HELD THE TOOTH IN PLACE. THE PIN SHOT ALL THE WAY THRU STRIRING THE EMPLOYEE ABOVE THE LEFT EYE. THE EMPLOYEE RECEIVED SIX STITCHES TO CLOSE THE CUT.
