Employee was holding a wrench when another employee accidentally hit his arm with a crib/wooden block and cut his arm. Sutures required.

Using the hand wrench to remove a tight bolt. The bolt was located in a confined area of the machine and when technician pulled the wrench to remove the bolt his left shoulder made a snapping noise and started hurting. The pain increased that evening in to the next day.

Employee was using a screwdriver to help peel the jacket off of a cable and slipped and struck the palm of his left hand.

Employee was hitting a metal pin with a hammer and a piece of the hammer broke off and struck him on his left leg and cut his leg. (2 sutures required).

Employee was attempting to square up a piece of bolt with a utility knife and knife slipped cutting his left hand between the thumb and index finger. EE received 7 sutures.

CHAIN RATCHET SLIPPED AND STRUCK EE ON LEFT LENER AREA. HE SUSFRAISED A LACERATION THE REQUIRED STITCHES.

EMPLOYEE WAS CUTTING OFF BELT PIN AND A SLIVER OF METAL LODGED IN HIS RIGHT HAND. THIS METAL WAS LATER REMOVED AND STITCHES WERE USED.

EE WAS USING A UTILITY KNIFE TO CUT A PIECE OF TAPE WHEN KNIFE SLIPPED AND CUT LITTLE FINGER A LEFT HAND.

MAKING A SPLICE IN BELT WHEN A SMALL PIECE OF HAMMER FLEW OFF THE HAMMER FACE AND STRUCK HIM IN THE UPPER LIP. METAL WAS REMOVED AT HOSPITAL AND STITCHED.

EE WAS STRIKING A KNOCKER WHILE MAKING A BELT SPLICE WHEN A SMALL PIECE OF METAL STRUCK HIS RIGHT THUMB AND BECAME LODGED. STITCHES WERE REQUIRED AFTER METAL WAS REMOVED.

EMPLOYEE WAS MAKING A BELT SPLICE WHEN THE UTLITY KNIFE HE WAS USING SLIPPED CAUSING HIM TO CUT HIS THIGH. EMPLOYEE SUSTAINED A LACERATION THAT NEEDED 5 STITCHES.

WHILE ROOF BOLTING WRENCH FELL OFF AND STRUCK EE ON LEFT HAND LITTLE FINGER.

EE WAS PRYING ON A SLATE BAR AND FELT PAIN IN LOWER BACK & NECK. EE ON MARCH 10TH HURT HIS BACK AT HOM & BEGAN MISSING WORK. DR. REPORT CANNOT ISOLATE THE INJURIES. REC'D DR.'S REPORT ON 3-19- 02.

EE WAS RERAILING A LOCOMOTIVE WITH A JACK WHEN THE BAR SLIPPED AND HE CUT HIS RIGHT HAND.

EE WAS USING A RATCHET TO TIGHTEN A BOLT AND RATCHET SLIPPED CAUSING HIM TO STRIKE HIS WRIST.

EE CUT HIS RIGHT LEG WITH A UTILITY KNIFE & DOCTOR PUT HIM OFF WORK BEGINNING 11/15/01.

EE LOST GRIP ON BAR USED TO LOAD BELTING AND BARFELL STRIKING HIS RIGHT FOOT.

EE WAS SPLICING A CONVEYOR BELT WHEN A SMALL PIECE OF METAL CHIPPED OFF KNOCKER AND STRUCK RIGHTINDEX FINGER. WOUND REQUIRED TWO STITCHES AFTER GETTING METAL OUT OF FINGER.

EE WAS TAKING A PART OUT OF A CONTINUOUS MINER &A SLATE BAR THAT WAS UNDER THE PART STRUCK HIM IN THE FACE WHEN THE PART SLIPPED OUT.

EE CUT HIS LEG WITH A KNIFE. RECIEVED MED REPORT ON 7-10-01 AND FOUND OUT HE HAD STITCHES.

EMPLOYEE WAS USING A JACK TO RESET A WRECKED TRACK FLAT CAR. THE HANDLE SLIPPED AND STRUCK HIM IN THE JAW. RECEIVED A MED REPORT INDICATING A FRACTURED JAW ON JUNE 4, 01.

EE WAS CUTTING A PIECE OF BELT AND CUT FINGER - 4 SUTURES.

EMPLOYEE CUT HIS RIGHT LEG WITH A UTILITY KNIFE. REQUIRED SUTURES.

EMPLOYEE WAS CUTTING PIECE OF BELT; KNIFE SLIPPED AND CUT HIS LEG.
