While an employee was manually turning the HPGR drive with a prybar the inertia of motion from the HPGR rolls continued to shift the drive with the prybar wedged in between while the employee's hand was still grasped onto the prybar. The force of prybar pinched the employee's right pinky finger against the bottom of the metal cage used to guard the drive when it is energized.

At approximately 10:50 PM, two employees were cleaning beneath the screw conveyor when one employee came into contact with the other employee's tool, resulting in an injury to the affected employee's right eye.

Employee was repairing a Raymond Roller Mill Journal assembly. EE was lifting the part with a sling and forklift to position it on a work table. EE attempted to reposition the part before placing on the table when the assembly came loose and a portion fell crushing EE's right thumb.

Miner was cutting a conveyor belt with a utility knife when the blade slipped and lacerated EE's right knee.

Fabick CAT employee was working in the Cleveland Cliffs Mining Shop. The employee was working on a motor grader cylinder head, installing a fitting. The wrench slipped off and the employee's hand hit a flange, on the cylinder head, causing a small laceration on the top of EE's left hand.

Container of parts was being lowered to a mechanic in a confined space. The rope broke and the container fell on the employee striking EE on EE's hardhat

Employee was removing valve from Tilden 1 #2 South deaerator line when their left hand was pinched between the valve and a handrail. The employee was wearing gloves at the time of incident and sustained a laceration to their left hand. Employee was transferred to local ED for evaluation and received medical treatment (7 sutures) and returned to work.

Employee was using a pick to cut a coal sample from the rib. While swinging the pick into the rib, a small piece of coal flew back and hit EE in the mouth, causing a small cut to EE's upper and lower lip. Employee received one stitch in EE's lip. Returned to work EE's next shift.

EE was was working on the plant. EE was applying pressure to a prybar when EE's hands slipped off and EE's right little finger was pinched by the bar, resulting in a fracture to the tip of EE's right pinky finger.

Employee was using a pipe wrench to disassemble a water pump. EE was pulling down on the wrench from overhead when the jaws slipped causing the handle to hit EE in the forehead. Employee sustained a laceration to the forehead requiring stitches.

IE was repairing a suction line on a roof bolter when the knife EE was using cut the palm of EE's hand. Only reportable due to receiving sutures.

Employee was removing a fire water valve when the employee's left finger was pinched between the valve and the hand railing while lowering the valve with a chain hoist. Employee was transferred to local ED for imaging which revealed nondisplaced fracture to employee's left pinky finger. Employee was provided a splint for immobilization and protection and returned to work.

Laboratory technician was working with a box cutter knife when they went to preform a task in the lab. The knife slipped from their hand resulting in them instinctually catching the knife and causing the knife to cut their thumb.

Employee was assisting with 518 hoist rope change out. While pushing on prybar to push the rope employee strained EE's L arm.

Employee was using a utility knife to cut nylon straps holding roof bolts. Knife jumped while cutting a strap and cut EE's left leg above the shin bone. Employee received sutures to close the wound.

During the dragline outage, an employee was using a prybar to assist in the task of removing a fairlead sheave. During the task, the pry bar shifted upward and struck the employee in the face and neck area. The employee sustained two cracked teeth and placed on no work for two days.

Employee was using a pry bar to unplug a clogged bit on a blast hole drill. Once the clogged bit was free of material, the employee dropped the pry bar onto EE's foot which contacted EE's foot in between the safety toe and the met guard of the boot causing a superficial fracture to EE's big toe.

EE and crew were turning a floor brace using a sleever bar in one of the bolt holes. Brace was on flat side. EE standing on E side of the N/S aligned brace pulling on the bar toward EE. In the line of fire when piece reached tipping point where crew needed it for rigging to put in place. Piece reached tipping point, EE lost footing. The sleever bar struck EE's leg causing cut.

Employee assisting laying track rails on the 7 North Haulage. EE was helping co-workers roll rail over using a long pry bar. As they were rolling a rail, EE stopped but employees on the other end continued to roll their end, causing the bar EE was using to swing back and struck EE on the right side of EE's neck and jaw. Failed to maintain control of the hand tool EE was using.

EE was adjusting the headlamp on EE's hard. When doing this EE was cutting a zip tie that held the headlamp strap on EE's hat. EE punctured EE's left index finger.

Employee was cutting a plastic band off of concrete block with a utility knife when it struck EE's left thigh causing a laceration. EE was treated at the ER where EE received two sutures and a full release with no restrictions to return EE's next shift.

EE was driving out a pin with a hammer for the finish mill hydraulic arm when the pin broke loose causing the punch EE was holding to shift pinching EE's left middle finger against the pin eye edge causing a laceration on EE's left ring finger.

Employee as working on replacing the #1 drive motor for the screen. While torquing the bolt, the motor pivoted, causing EE's left hand to be caught between motor mount and pivot point; resulting in a fracture to EE's left thumb. EE went to clinic and EE's thumb was splinted.

Miner was helping to change a gearbox on a screw conveyor that was difficult to remove. During this process a wrench they were using slipped and their hand hit a bolt on the equipment causing swelling and bruising.

Employee was holding a rope to align a new conveyor belt. EE wrapped the rope around EE's left hand and as the belt was being positioned EE received a rope cut that needed at least 10 stitches.

Contractor EE was repairing a ripper tooth from a D10 Dozer. EE hit the ripper tooth with a hammer a couple of times to get it in place for the retaining pin. A piece of metal sheared off of the ripper and embedded into the EE's left arm. EE eventually went to the ER for treatment. It was removed and EE was prescribed antibiotics.

EE was swinging a sledge hammer, missed the bolt smashing EE's right pinky finger between (C) channel and wood end handle. Causing EE to receive stitches.

While performing maintenance attempting to cut a piece of rubber with utility knife, knife slipped causing cut to LEFT pinky finger. Employee was taken to local Urgent Care for observation and diagnosed with laceration to LEFT pinky.

Employee was using a dead blow hammer to clear salt on the the 3rd screen bank and hit EE's left pinky finger.

While performing maintenance on the secondary mill the miner was using the hammer and during a swing felt a pinch and/or sting in EE's left bicep. The miner suffered a tear in EE's left bicep.

While working at the Juniper Pit in Fernley, Nevada on drill D-48, mechanic sustained a dental injury when a wrench slipped and struck EE's tooth during a repair task.

Miner was cutting a piece of belting off of a roll when the knife slipped and the Miner cut EE's left hand in two places. The laceration resulted in two sutures.

4/28-First Aid EE was tightening the bolts on the 3" inch flange pump when heard a pop sounded followed by a very strong & painful cramping in the right bicep. Followed by throbbing pain and reddish bruising forming on the right bicep. 5/5 -Went recordable due to PT

EE was cutting a belt and the knife slipped cutting EE's knee

Employee was chipping tiles out of a TEMA in the prep plant. Employee was wearing safety glasses and a full-face shield. Employee felt something in the right eye and went a mirror to find a foreign debris in the right eye. Employee asked for assistance from another employee to flush eye. The employee was taken to a local optometrist and prescribed antibiotics

Employee was attempting to dislodge a stone in a jaw crusher with a long steel rod. As EE used downward pressure to strike the stone, EE struck EE's right hand on the edge of a steel plate that surrounded the crusher.

Employee was removing spray bar clamps from an old wash screen. After removing the hardware, the U-bolt was stuck in position around the bar, Employee used a 10" pry bar to free the clamp. While pulling downward the pry bar slipped and pinched the palm side tip of EE's left ring finger, causing a laceration requiring two stitches.

Employee was using a pry bar to adjust hoist cylinder, when the bar slipped and struck EE's mouth and broke a tooth.

Employee was using a bar to dig out rock that was stuck in a cone crusher. When the rock broke, the bar hit the mantle, pushing the bar back and turning it in the employee's hand which cut employees fingers.

Cutting 4 inch water hose with multi tool, tool slipped and cut hand through glove. Leaving roughly a 1 inch cut.

Wrench struck safety glasses, employee suffered laceration to face that required stitches.

Shop employee was attempting to remove a trailer tire from the front axel of a tanker truck with a sledge hammer. The sledge hammer bounced back and pinched the employees hand against the fender of the truck causing a 1/2" laceration that required 2 stiches.

The injured was trying to insert/align a shear pin in the relay bar on shield #2. While trying to align the pin, a come-along that was hooked from the header to the relay bar became unhooked, striking the employee on the top of the right hand causing a laceration which required sutures to close.

While removing a bearing, employee struck self in face with hammer

The utility person was cutting the seal out of a 1-quart bottle of engine oil with a belt knife when they accidentally cut the end of their left thumb. 3 Stitches were required no work was missed.

IE was changing a roller on the slope belt. The chain hoist supporting the roller slipped and IE's finger was caught between the roller and the frame. Injury was not the result of a training or compliance issue.

On 4/17/26 an employee was breaking up core samples with a hammer when EE felt discomfort in EE's right shoulder and hand. Employee sought additional treatment and was placed on restricted duty beginning 4/27/26.

Miner was cutting a piece off of a conveyor belt using a utility knife. Miner was cutting towards their body , knife slipped and made contact with right forearm causing a laceration. Laceration was approx. 1" long and several mm deep and required medical attention.
