ee was moving the OSM drill, but it was frozen in the ice, so they tried barring it over and wrenched their back
EE squished their finger between metal frame a piece of granite.

EE lost balance while splitting a block and fell. EE was wearing working at heights protection, however EE's retractable failed to lock up. Before use inspection was conducted, it worked fine. Retractable has been put out of service.
Employee was helping move power cords and pulled a muscle in left arm.

While operating the crane, employee noticed once of the chains got fetched and decided to move towards it without stopping the crane. The crane broke free and whiplashed back at employee and striking EE in the upper left forehead leaving a laceration. First aid administered; ambulance called.
Miner felt pain in right shoulder when exiting a loader.
Miner was moving (walking) a piece of grout to a grout bin for disposal when the grout slipped out of EE's hands and landed on EE's foot

EE WAS DRIVING A IT28B DOWN THE BACK ROAD TO MOVE EQUIPMENT. THE EE WAS DRIVING DOWN THE HILL, AND THE LOADER WAS GAINING SPEED, GOING TOO FAST TO STOP. THE LOADER CRASHED INTO A BERM BLOCK.

Was moving a block under the saw and pushed the wrong button on the overhead crane and pinned hand between the block and the guard on the crane.
Conducting secondary breaking operations. When EE was pulling the wedges the stone broke and EE fell to a lower level fracturing and dislocating EE's ankle.
EE was working on a conveyor roller and a piece of granite was pushed onto hand.

Miner fell from highwall 14'6"" to the lower level. Miner suffered 5 broken vertebrae and laceration to the back of head. Miner was ambulatory after the fall.
Employee was assisting mechanic on repairs to large quarry wire saw yesterday. While removing large sheet metal guard, due to the weight and position the employee hyper-extended wrist.

While handling the hand hammer drill and walking to EE's work area, EE slipped and fell on the ground. The hammer landed on EE's left hand, index finger.
Employee worked a full day on 2/15/16 with no complications. EE woke up on 2/16/16 with acute pain in EE's back. EE did not report to work that day. EE went to EE's own primary doctor. EE had at least two follow-up appointments with EE's primary doctor. EE reported to work on 2/22/16. EE had severe pain in EE's back and was sent home. EE went to the ER that day.

Employee was removing drill steel (approx. 10 feet long) from a drill machine. He tripped on a rock with the steel in his hand. He tried to protect himself from the fall and upon impact the weight of the steel caused his left wrist to twist backwards.
