EE was stepping away from conveyor support system when a rock rolled off of running conveyor belt and fell, striking EE on left front corner of hard hat. Rock rolled off hard hat & struck face below nose. Injury consists of split upper & lower lips (6 stitches), cracked mandible below nose and 3 cracked teeth. No concussion or loss of consiousness.
Emp was loosening bolts on crusher and while pulling breaker bar, he felt pain in groin area.
EE WAS SHOVELING STONE FROM CONVEYOR BELT, FELT TWINGE IN STOMACH. (HERNIA SURGICALLY REPAIRED ON 2/15/05.)

EMPLOYEE WAS INSTALLING TWO NEW ELECTRIC MOTOR STARTERS IN EXISTING PANEL BOX. TURNED BREAKER OFF TO WHAT HE THOUGHT WAS THE CIRCUIT HE WAS WORKING ON. IT WAS NOT THE RIGHT BREAKER. CAME IN C ONTACT WITH HOT WIRES CAUSING A FLASH.
