The injured employee was attempting to drill out a plugged purge line with a large hand drill. The bit caught in the hole and cause the drill motor to spin around and strike the employee in the right cheek. On Monday 02/19/2018 the employee experienced increased swelling and sought medical attention where they treated employee for infection.

Employee was performing an overhead patch repair on the kiln eyebrow. EE was tamping plastic using a hand held pneumatic pounder. The air exhaust from the tamper blew material behind safety glasses into right eye.

Employee was removing table liners from the coal mill. One of the liners had a strap around it so that it could be lifted with the hoist. The employee was taking up slack from the choker when the liner tilted and fell over, striking the employee's leg.

The employee was attempting to roll a piece of 12"x24" 13 gauge sheet metal in a sheet metal roll bender. The employees hand was positioned too close to the rolls which hand went through. This resulted in thumb being degloved and index finger broken.

Employee was lining up a punch and die set in a Piranha Ironworker. In doing so, the set up was not aligned and the punch came into contact with the inner edge of the die. As the employee bumped the machine to recheck alignment the pressure of the punch and die contact caused a small sliver of metal to break off and strike the employee's left cheek.

EE was removing the rail from the top of a horizontal I-beam using a cutting torch at waist level. When the last weld was cut the rail fell from the beam striking ee in the right leg at the knee then sliding down leg.

Employee was inside an excavator breaking rock up. A rock slammed the arm of machine down and jarred the machine, causing mid to lower back pain. Diagnosis: Trapezius strain.

When chipping refractory wall using chipping gun, a small piece of refractor/slag fell hitting miner on tip of thumb.

Removing and cleaning build up from inside walls of dryer crusher using high pressure water. The cleaning was being done from elevated outside cleanout ports. Due to build up on bottom of vessel opening and with limited access to this area men began cleaning from below bottom opening. Material buildup on vessel broke loose during operation and struck men causing bodily injury.

EE was working on an auto sampler. Air was turned off and the electrical equipment was turned off. Residual air pressure in the lines allowed the unit to cycle when the EE freed the blockage with his finger. The EE lost about 1/4 inch of his left index finger including some bone. Stitches to close the wound

EE was cleaning out a chute when the material broke free and forced the jackhammer into his neck. EE sustained a laceration to right side of neck that required stitches.

The injured employee was tasked to drill holes through the ACL Kiln pelletizer pan so new liners could be installed. Apparently, and unknown at the time, a piece of material got in his left eye. After work, he sought medical attention for eye pain and his eye doctor removed a foreign body and eye drops were prescribed. Incident reported/investigation began 04/21/15.

While machining parts the employee reached with his gloved hand to swipe away metal chips when the cutting head pulled his hand into the mill severing his right middle finger.

The slurry water supply hose appeared to be leaking, it was under ice so we attempted to raise it's pump by cranking the hand crank. I felt a sharp pain, pull or tear in right shoulder.

Employee guiding mixer unit slips down into tankers & agitates the load enabling pump out. Injury to right index, middle, ring & pointer finger. Smash with no broken bones. Faulty equipment design w/ no identified hand placement area & a floating cap on mixer is guided by 3 flat pieces of metal. Cap side loaded & entered into the tanker it jarred falling on employee's rt hand.

EE was grinding pipe when metal shavings blew under face shield and safety glasses into eye. Eye flush was done on site, but later taken to urgent care for removal of shavings. Initially the Dr. note was unclear on First Aid or Medical Treatment. Confirmation was given on 11/04/2014 that the procedure was Medical Treatment.

Employee was attempting to remove the bucket tooth on the 988H Cat loader with an air hammer. The air chisel bit on the air hammer broke off, causing the hammer to slip and pinned his left middle finger between the air hammer and the loader tooth.

Failure to control equ/roller

Affected employee was standing in the middle of an empty barge preparing to tie it up when the barge bumped the dock barge causing the employee to lose his footing and fall onto his right shoulder.

An employee was cutting metal with a small hand grinder with a cutting wheel attached. The grinder "kicked back" causing a three inch laceration to the top part of his middle thigh. The employee required stitches to close the wound.

Employee was rebricking inside the kiln and using a 4" portable grinder to smooth the metal surface on 12/10/2013. Safety glasses were worn at all time while in the kiln and safety glasses and a face shield were worn while grinding. Employee reported on 12/11 his felt material on his eye. Treatment was sought the following day.

While welding, the injured employee was chipping slag from the weld when a small piece of metal from the chipping hammer, about 1/16" long, came off and imbedded about 1 inch below the employee's left eye. The employee was wearing safety glasses. The employee was taken to the hospital to have the piece removed which required two stitches.

Employee was reaming a hole using an electric end grinder. The wind blew a foreign object into his left eye. Employee was wearing safety glasses and a full face shield. After flushing the eye with eyewash, employee stated that his eye was still irritated. Employee was taken to Care Now clinic. No foreign object was discovered in his eye, but the eye was slightly scratched.

Employee was welding and his eye felt irritated at the end of his shift. The irritation didn't go away so several days later he sought medical attention. A foreign body was found in his right eye.

EE was cutting a 2x4 and saw got in a bind and jumped back cutting his left thumb.

Employee was welding with improperly fitting welding hood. Employee noticed eye pain consistent with "flash burn". He was treated with antibiotics to prevent infection on 7/11/13

The operator & 2 helpers were attaching the jib to the boom of a 90 ton hydraulic crane. The operator removed the pin on the side of the boom before he installed the 2 pins that hold the jib in place. This caused the entire jib to drop to the ground. One side of the jib struck the operator's hard hat as it fell to the concrete paving.

Cleaning mixer, glove of right hand got caught on paddle which pulled hand in. RIGHT HAND laceration, required 13 stitches. No broken bones, no severed ligaments or tendons.

EE was removing a 3/4" bolt using a Tone SR71E Simple Torque gun. As the bolt became loose, he released the trigger and reached in to remove the bolt while the gun was still spinning. His left thumb became pinched between the torque reactor bar and the steel beam. His basic reaction was to pull his thumb free. As he did, the pinched condition caused his thumb to be injured.

Cutting plate with torch, block under plate kicked out, letting plate strike his right middle finger and took off the nail.

Employee was standing on the bricking platform in the kiln when the pneumatic brick ring was charged with air. The air blew a small amount of dust into the employee's right eye. Employee was unable to dislodge the object with flushing of the eye.

While grinding during the course of the day, it is believed that a piece of metal landed on employee's eyebrow or eyelash and worked its way into the eye after leaving plant.

Working with an impact socket, the "knuckle" came loose and struck his left pinky finger several times at the joint just below the fingernail.

While removing brick/ refractory from cooler tube in preparation of rebuilding the cooler tube, EE sustained an injury to his right middle finger. He was using a pneumatic chisel to break brick. A lifter with base was above the brick to be removed. As EE was chiseling said brick the lifter/base came loose and injured the finger.

The "headache" ball of a 5180 Grove crane went past the two block switch and over the top pulley and came down 290 feet into the cab of the crane. This resulted into a compound fracture of the left tibia/fibula of the individual.

I was cutting on the bottom of a washer on the Dredge, when the cutting torch popped and sparks flew from the metal & rust that I was cutting. At the time I felt something stinging in my eye. Tried to wash it out with no success, then reported it to my Supervisor. I was wearing safety cutting glasses at the time.

While running Gap Vac to clean up material around feed belt, the hose was not sucking, so employee went to clean the hose out, uncoupled hose and got his right arm caught in 6 inch hose and bruised his arm.

Employee was removing a nut from a liner bolt using a ridged threader. He positioned a wrench on the nut and was holding the wrench over the frame of the machine. He inadvertently stepped on the guarded foot activator activating the machine and pinching the finger between the wrench handle a machine frame.

Employee was using a cutting torch on a fuel tank when something flew into his left eye. The foreign object was under his left upper eyelid, and after several attempts to wash the object out to no avail, the employee sought medical help. The employee was wearing safety glasses with side shields, but did not put goggles on.

The handle of an air lance pinched the webbing in between the employees middle and index finger on his right hand. The employee received 4 stitches in this area and was returned to work at full duty.

EE states that while he was vacuuming in the grate cooler his elbow started hurting. There was no bump or fall just pain. EE did not report medical treatment until returning from layoff on 3/2/2011.

Grinding on a flange when a piece of steel got into the employee's right eye. He was wearing his shield and glasses. Steel was not embedded; used antibiotic eye drops as a percaution.

Employee was grinding a clinker sample in the impactor. When the employee bent down to retrieve the sample bucket, a piece of material fell in his right eye. The employee was wearing safety glasses. Employee received medical treatment and given prescription eye drops. He returned to work the same shift without restrictions.

Employee was cutting wizzer blades out of the separator using a welding torch when he reached out to remove a piece he passed his left arm through the flame of the torch exposing the skin between the flame retardant jacket and his glove. Injury became infected on 5/29/2010 and he received medical treatment.

EE was cutting with metabo. The cutting disk wedged and kicked back. The power tool fell and then cut his left leg below the knee. Wound was cleaned and he received six stitches to close the wound. EE was returned to work with no restrictions.

The employee was cutting small pieces of steel in the metal shear. He was using a piece of metal to push the pieces into place. Rams, which hold the metal flat just before the cut, came down forcing the metal he was holding to crush the tip of his finger between the metal and the table surface of the shear. The tip of his left middle finger was lost due to tissue damage.

EE was pushing rock screens with a 375 Komatsu dozer when he began to experience problems with the dozer. Mobile mech.came to diagnose the problem. EE raised the blade put the dozer in drive,the dozer moved forward 5' and then jerked hard twice.The EE continued to push screens up the pile when his neck stiffened up. EE was seen by the doctor and given RX meds.and work restrictions.

Using an airlance (45') to clean motor and gearboy on the column, dust got into eyes. Employee was wearing safety glasses with side shields.

The employee was welding an ibeam the steel i beam was rolled over to access the other side, the beam came loose from a chain and fell, about 2 feet onto his left foot. His left big toe was fractured behind the steel toe in his boot. This resulted in a lost time injury
