Employee was installing a door at the backend of the kiln. They knelt down to retrieve a tool from a coworker and rolled their right ankle. Resulting in an ankle sprain.

Employee states EE was welding on 3/27/2026 when a spark went under and in welding jacket. Employee states EE brushed it off and went about EE's normal day. EE reported that EE's injury may be infected on 3/30/2026 and was taken to a clinic for evaluation and prescribed antibiotic cream. No work restrictions were given to employee.

Employee slipped and fell on a patch of ice exiting a doorway on the silo tops near belt conveyor 12. Employee reported incident 1/5/26, sought medical 1/28/26 and was placed on restrictions on the follow up appointment on 2/3/26.

Employee was opening the manual slide gate on the clinker silo with a ratcheting wrench. They applied pressure to the wrench and the ratcheting mechanism wasn't fully engaged, allowing the wrench to slip and smash the employees ring finger between the wrench and the feeder structure. Employee was wearing leather gloves- 4 sutures in fingertip

Employee was retrieving garbage bags from a dumpster that had mistakenly been thrown in the wrong roll off. EE was using a grabber tool to retrieve the bags, as EE was pulling a bag towards the front of the dumpster, EE felt a pop and discomfort in EE's neck. Taken to the clinic on 7/7/25 and diagnosed with a strained muscle in EE's neck and placed on light duty.

While swinging a sledgehammer to remove the roll pins on the blow bars of the secondary crusher the employee felt a sharp pain in the lower right back. Was seen by the doctor on 2/26/25 and placed on restrictions for a muscle strain in the lower back.

Employee was unhooking a semi from a side dump trailer. As EE pulled the release pin EE felt a pop in EE's left arm. Employee had tenderness in bicep area, continued working and sought medical attention on 10/16. MRI on 10/22 showed a partial tear in left bicep.

Employee was removing the #2 door on the raw mill. The door was tight requiring employees to strike the door to get it to open. On the second swing EE felt a pinch in EE's midback, straining EE's back.

Employee was getting onto the loader and slipped on the step. Employee caught themselves with their arms but pulled right shoulder. Employee reported injury to supervisor 2/10/2024, was seen by doctor and no findings in an Xray on 2/14/2024, MRI completed on 2/21/2024 and prescribed physical therapy for a small tear in the right shoulder.

Employee was installing conduit. As EE was looking up and walking, EE tripped and fell against a barrel of grinding balls striking their right hip/ thigh. Bruising and swelling of the impacted area.

An electrician was striping an electrical cable lead with a striping knife when EE's left hand slipped. EE rolled left palm over the tip of the striping knife lacerating EE's left hand. Employee received 4 sutures to close the wound. Employee was wearing gloves at the time of the accident.

Employee reported injury on 3/19/22, EE felt an initial twinge in groin area on 3/12 and again on 3/19 while conducting clean up work. Went to physician on 3/19, returned to work performing regular work activities as a Control Room Operator. Referred to a specialist and an operation was conducted on 4/15 for a hernia. Has returned to EE's next scheduled shift 4/18/2022 as CRO.

Employee was checking for wear/slop on the carrier rollers using a prybar. Bar slipped from the roller and pinched employee's right ring finger between the bar and the frame of the conveyor.

Employee was removing a bucket in the raw elevator, when the bucket was knocked loose the bucket fell as the employee went to catch the bucket it caused right forearm and hand to twist outward feeling pain in right bicep. The employee went to the doctor on 1-28-22 where it was determined that employee had a torn bicep tendon.

EE was removing a liner bolt with an impact drill, when the drill slipped EE grabbed the drill by the socket, the split pin holding the head in place on the socket backed out 1/32" catching EE's glove & twisting EE's hand, breaking the sesamoid bone in the left thumb. EE went to the doctor on 12-30-19 where it was determined EE had a broken bone.

EE was removing a piece of angle iron from an elevated position when some dust dropped from above getting into right eye, the employee flushed the eye and believed the object was removed. Upon waking the next morning the employee had irritation in eye, ee was taken to urgent care where a piece of dirt was removed and an antibiotic prescribed.

EE came into report that EE has been experiencing bi-lateral wrist pain with numbness and tingling in EE's hands. EE stated the condition developed over time and believes that the condition is a result of the repetitive motion of loading and unloading cement trucks for multiple years. EE reported to an occupation illness doctor, EE was placed on modified duty on 11-27-18.

EE was looking at a seal as the crusher door was being closed, EE rested EE's hand on the housing and pinched EE's left thumb between the door and the housing. X-rays were negative and no stitches were required the employee had to have the thumbnail removed.

EE was lifting the secondary crusher suspension bar into place when felt a strain in lower back. The incident occurred on Monday, July 3rd, 2017 and EE reported the injury on July 7, 2017. EE received treatment from a chiropractor and is working modified duty.

Employee was using a hand held air hammer to free up bridged material. As EE went to sweep the material with hand off the small ledge of the port, the material sloughed pulling right arm in the bin. The employee sustained a small burn under the right triceps where a doctor perform debridement of the scab on 9-2-16 and issued a prescription.

Employee was attempting to remove the keeper pin on a D-8 Cat blade with a 2 pound Sledge hammer and a punch. As EE swung the hammer, it glanced off of the punch, hitting EE in the left hand between the thumb and pointer finger. EE sought medical help on 4/13/16, but there were no fractures. EE returned to the doctor on 4/18/16 and was placed on light duty.

Employee was on the silo top sweeping material away from a conveyor belt. EE was backing up while sweeping when EE tripped on a platform, landing on EE's right elbow. EE broke the radius bone near the elbow joint on EE's right arm.

EE has been experiencing bilateral hand and wrist pain for some time. Tingling and shock like episodes are occurring bilaterally at times. EE has been icing the affected area and taking ibuprofen, which helps, but does not alleviate the symptoms. He sought medical help on 11/9/2015, and will be starting physical therapy. He has no job restrictions.

Employee was removing a 5th wheel plate assembly from a 1994 Freightliner truck. His back started getting sore, and progressively got worse over the next 2 days to the point where walking was very difficult. He sought medical care on 9/2/2015.

Employee was coming down from the slurry tanks on an icy metal stairway when he slipped. His right hand was on the handrail, but when his feet slipped, his right elbow went over the rail. He felt something pop in his right shoulder, which gave him shooting pain in the right shoulder and neck. The fingers on his right hand started tingling.

Employee was placing a shim on a pneumatic railcar door to prevent it from opening. Another employee fired up the air compressor, putting air to the car believing that the actuator was closed when it was not. The door shot open pinning employee's left ring, middle and index fingers between the door and the shim, causing severe crushing injuries.

Employee was attempting to remove the bucket tooth on the 988H Cat loader with an air hammer. The air chisel bit on the air hammer broke off, causing the hammer to slip and pinned his left middle finger between the air hammer and the loader tooth.

Employee was standing on the Marland C-Conn gearbox. He was handing another employee a hammer when he lost his footing and fell about 49 inches, landing on the heel of his left foot. He sustained a fracture to his left heel bone.

The employee was lowering a ramp from the hood of the kiln to the front of the kiln. When he got the ramp near the floor, he let the door drop the last few inches. It caused a puff of dust which caused something to get into his right eye. He flushed the eye multiple times to no avail.

Employees were attempting to clean the crusher. The door had been removed and was leaning against the wall. When one of the employees inserted the air lance in the crusher, the hose snagged the door causing it to fall on the injured employees right foot above the steel toe.

Employee was removing the belly pan off of the D-8 Cat. He had to remove the floor pan and seat. In the middle of working, he felt his right arm getting sore. The next morning he woke up and had a sharp pain in his right shoulder. He did not seek medical attention until 8/21/2013, and was placed on modified duty as of that date.

Employee was lifting up on a pipe wrench that was approximately 2 feet below waist level when he felt a pop in his right shoulder, and experienced immediate pain that radiated down the back of his arm, past the triceps to the elbow. Employee did not seek medical attention until 8/20/2013 and was put on modified duty as of that date.

The employee was lowering the hood of his semi-truck with his right arm when he felt a pull/tear in his right shoulder as the hood weight pulled over the top. Employee sustained a slap tear to the right shoulder, but returned to work the next day on full duty. He had surgery to repair the tear on 11/05/2013.

On May 23rd, the finger became red and swollen. The employee sought medical attention at that time and was diagnosed with an infection, which required antibiotics.

The bit seemed to be in a bind, so he bumped the hammer with his shoulder to straighten it out. The hydraulic pressure had bled down, binding the steel. His left index & middle fingers were caught between the bit & the bit collar.
