MONTANA CITY PLANT Metal/Non-Metal

Employee slipped and fell on a patch of ice exiting a doorway on the silo tops near belt conveyor 12. Employee reported incident 1/5/26, sought medical 1/28/26 and was placed on restrictions on the follow up appointment on 2/3/26.

Employee was opening the manual slide gate on the clinker silo with a ratcheting wrench. They applied pressure to the wrench and the ratcheting mechanism wasn't fully engaged, allowing the wrench to slip and smash the employees ring finger between the wrench and the feeder structure. Employee was wearing leather gloves- 4 sutures in fingertip

Employee was retrieving garbage bags from a dumpster that had mistakenly been thrown in the wrong roll off. EE was using a grabber tool to retrieve the bags, as EE was pulling a bag towards the front of the dumpster, EE felt a pop and discomfort in EE's neck. Taken to the clinic on 7/7/25 and diagnosed with a strained muscle in EE's neck and placed on light duty.

While swinging a sledgehammer to remove the roll pins on the blow bars of the secondary crusher the employee felt a sharp pain in the lower right back. Was seen by the doctor on 2/26/25 and placed on restrictions for a muscle strain in the lower back.

Employee was unhooking a semi from a side dump trailer. As EE pulled the release pin EE felt a pop in EE's left arm. Employee had tenderness in bicep area, continued working and sought medical attention on 10/16. MRI on 10/22 showed a partial tear in left bicep.

Employee was removing the #2 door on the raw mill. The door was tight requiring employees to strike the door to get it to open. On the second swing EE felt a pinch in EE's midback, straining EE's back.

Employee was getting onto the loader and slipped on the step. Employee caught themselves with their arms but pulled right shoulder. Employee reported injury to supervisor 2/10/2024, was seen by doctor and no findings in an Xray on 2/14/2024, MRI completed on 2/21/2024 and prescribed physical therapy for a small tear in the right shoulder.

Employee was installing conduit. As EE was looking up and walking, EE tripped and fell against a barrel of grinding balls striking their right hip/ thigh. Bruising and swelling of the impacted area.

An electrician was striping an electrical cable lead with a striping knife when EE's left hand slipped. EE rolled left palm over the tip of the striping knife lacerating EE's left hand. Employee received 4 sutures to close the wound. Employee was wearing gloves at the time of the accident.

Employee was walking from the quarry garage to the truck shop when employee slipped on a small patch of snow cover ice. Employee slipped and fell, landing on back and hitting head. Area had been sanded.

Employee has been exposed to occupational and recreational noise for 30 years, resulting in hearing loss. Employee is provided with, and utilizes hearing protection. Employee is enrolled in a hearing conservation program and is trained annually.

Employee reported injury on 3/19/22, EE felt an initial twinge in groin area on 3/12 and again on 3/19 while conducting clean up work. Went to physician on 3/19, returned to work performing regular work activities as a Control Room Operator. Referred to a specialist and an operation was conducted on 4/15 for a hernia. Has returned to EE's next scheduled shift 4/18/2022 as CRO.

Employee was checking for wear/slop on the carrier rollers using a prybar. Bar slipped from the roller and pinched employee's right ring finger between the bar and the frame of the conveyor.

Employee was removing a bucket in the raw elevator, when the bucket was knocked loose the bucket fell as the employee went to catch the bucket it caused right forearm and hand to twist outward feeling pain in right bicep. The employee went to the doctor on 1-28-22 where it was determined that employee had a torn bicep tendon.

Employee went to reposition themselves by reaching overhead, while doing so they felt some pain in right shoulder. The employee went to the doctor on 3-25-21 and was diagnosed with a shoulder impingement.

Employee slipped while walking behind the shop and did the splits, employee had pain in right hip after a medical review it was determined the employee aggravated an underlaying medical condition and needs the hip replaced.

On 5-19-20 employee noted some stiffness in EE's lower back from the repositioning of 20' section of pipe being fabricated for rail offload. Employee went to the doctor on 6-8-20 and a steroid was issued for a pinched nerve in the lower back. No work restrictions were issued for the injury

Employee was adjusting tire chains for reinstall on a 988 loader, when employee pulled on the chain employee felt a pull in the groin area resulting in a hernia, the employee was placed on restricted duty on 5-21-2020.

EE was removing a liner bolt with an impact drill, when the drill slipped EE grabbed the drill by the socket, the split pin holding the head in place on the socket backed out 1/32" catching EE's glove & twisting EE's hand, breaking the sesamoid bone in the left thumb. EE went to the doctor on 12-30-19 where it was determined EE had a broken bone.

EE was walking over to turn on the cutting torch tanks when EE's foot got stuck in mud as EE was rounding the corner, the employee felt a pop in EE's knee and has been placed on restricted duty until an MRI can diagnosis the injury fully

EE was transferring cement from silo 2 to silo 5 when EE opened a valve and cement dust got into EE's right eye.

EE was removing a piece of angle iron from an elevated position when some dust dropped from above getting into right eye, the employee flushed the eye and believed the object was removed. Upon waking the next morning the employee had irritation in eye, ee was taken to urgent care where a piece of dirt was removed and an antibiotic prescribed.

EE came into report that EE has been experiencing bi-lateral wrist pain with numbness and tingling in EE's hands. EE stated the condition developed over time and believes that the condition is a result of the repetitive motion of loading and unloading cement trucks for multiple years. EE reported to an occupation illness doctor, EE was placed on modified duty on 11-27-18.

EE was looking at a seal as the crusher door was being closed, EE rested EE's hand on the housing and pinched EE's left thumb between the door and the housing. X-rays were negative and no stitches were required the employee had to have the thumbnail removed.

Employee was attempting to move the lid covering the feed throat of the primary crusher when EE felt a pop in EE's lower left bicep, an MRI on 9-12-18 determine a partial tear of the bicep muscle occurred.

EE was lifting a 5-gallon bucket of oily waste to empty into the oil dumpster when EE felt a pop in left shoulder.

EE felt a pop in right shoulder while installing buckets on a bucket elevator, EE was placed on light duty 4-26-18 with a possible rotator cuff tear

The employee was emptying a trash barrel into the dumpster when the barrel slipped further into the dumpster pinching EE's right ring finger between the barrel and the edge of the dumpster. The ring finger required three stitches to close the laceration.

The employee was removing a bucket tooth and retainer pin on a Cat 988 loader, employee missed the driver tool used to remove pins striking right thumb with a 3lb hammer. The thumb broke in two places near the mid-knuckle.

EE reported pain radiating down from neck into right arm that they believe occurred from years of shifting haul truck. The employee received a shot to relieve pressure around the nerve in the cervical plexus.

Employee was replacing cutting edge on a Cat 988 loader, when removing a bolt employee grabbed the head of the bolt and cut left ring finger requiring four stitches to close.

EE was lifting the secondary crusher suspension bar into place when felt a strain in lower back. The incident occurred on Monday, July 3rd, 2017 and EE reported the injury on July 7, 2017. EE received treatment from a chiropractor and is working modified duty.

Employee was walking over to lock up the gas pumps for the day and slipped and fell landing on their left shoulder.

Employee was using a hand held air hammer to free up bridged material. As EE went to sweep the material with hand off the small ledge of the port, the material sloughed pulling right arm in the bin. The employee sustained a small burn under the right triceps where a doctor perform debridement of the scab on 9-2-16 and issued a prescription.

Employee was attempting to remove the keeper pin on a D-8 Cat blade with a 2 pound Sledge hammer and a punch. As EE swung the hammer, it glanced off of the punch, hitting EE in the left hand between the thumb and pointer finger. EE sought medical help on 4/13/16, but there were no fractures. EE returned to the doctor on 4/18/16 and was placed on light duty.

Employee was on the silo top sweeping material away from a conveyor belt. EE was backing up while sweeping when EE tripped on a platform, landing on EE's right elbow. EE broke the radius bone near the elbow joint on EE's right arm.

EE has been experiencing bilateral hand and wrist pain for some time. Tingling and shock like episodes are occurring bilaterally at times. EE has been icing the affected area and taking ibuprofen, which helps, but does not alleviate the symptoms. He sought medical help on 11/9/2015, and will be starting physical therapy. He has no job restrictions.

Employee was removing a 5th wheel plate assembly from a 1994 Freightliner truck. His back started getting sore, and progressively got worse over the next 2 days to the point where walking was very difficult. He sought medical care on 9/2/2015.

Employee was opening the bottom guard under the head pulley covering a return roller to clean out material that was building up. When the guard opened, a large hardened piece of material fell out and dropped on top of his left foot.

Employee was coming down from the slurry tanks on an icy metal stairway when he slipped. His right hand was on the handrail, but when his feet slipped, his right elbow went over the rail. He felt something pop in his right shoulder, which gave him shooting pain in the right shoulder and neck. The fingers on his right hand started tingling.

Employee was removing his sweatshirt over his head when he felt something in his left eye. He tried flushing the eye with saline solution and water, but was not successful.

Employee was placing a shim on a pneumatic railcar door to prevent it from opening. Another employee fired up the air compressor, putting air to the car believing that the actuator was closed when it was not. The door shot open pinning employee's left ring, middle and index fingers between the door and the shim, causing severe crushing injuries.

Employee was attempting to remove the bucket tooth on the 988H Cat loader with an air hammer. The air chisel bit on the air hammer broke off, causing the hammer to slip and pinned his left middle finger between the air hammer and the loader tooth.

Employee was standing on the Marland C-Conn gearbox. He was handing another employee a hammer when he lost his footing and fell about 49 inches, landing on the heel of his left foot. He sustained a fracture to his left heel bone.

The employee was lowering a ramp from the hood of the kiln to the front of the kiln. When he got the ramp near the floor, he let the door drop the last few inches. It caused a puff of dust which caused something to get into his right eye. He flushed the eye multiple times to no avail.

Employee was installing a seat in one of the haul trucks when he experienced sharp pain in his right shoulder while setting the seat in place. Employee did not seek medical help until March 31, 2014.

Employees were attempting to clean the crusher. The door had been removed and was leaning against the wall. When one of the employees inserted the air lance in the crusher, the hose snagged the door causing it to fall on the injured employees right foot above the steel toe.

Employee was removing the belly pan off of the D-8 Cat. He had to remove the floor pan and seat. In the middle of working, he felt his right arm getting sore. The next morning he woke up and had a sharp pain in his right shoulder. He did not seek medical attention until 8/21/2013, and was placed on modified duty as of that date.

Employee was lifting up on a pipe wrench that was approximately 2 feet below waist level when he felt a pop in his right shoulder, and experienced immediate pain that radiated down the back of his arm, past the triceps to the elbow. Employee did not seek medical attention until 8/20/2013 and was put on modified duty as of that date.

The employee was lowering the hood of his semi-truck with his right arm when he felt a pull/tear in his right shoulder as the hood weight pulled over the top. Employee sustained a slap tear to the right shoulder, but returned to work the next day on full duty. He had surgery to repair the tear on 11/05/2013.

Employee was in the process of re-bricking the kiln. He had his hand on a pallet of brick that was being used for the overhead rings. A brick slipped between an existing ring of brick and the pneumatic ring, striking his left thumb. This resulted in a fracture to the upper bone in his left thumb.

While loading a rail car out of silo 12 the pulley system that raises and lowers the spout, came off of the sheave. The employee reached out between the top and mid rail to pull the cable back into the sheave, in doing so the tension was released dropping the spout. He suffered abrasions, lacerations, and swelling to his right hand

Employee was attempting to string a 3/4" safety cable across the Raw Mill. He was lifting the cable over his head to get the shackle hooked. The shackle didn't hook and it came back down, pulling his left arm behind him, causing left shoulder pain.

Employee was standing on the bricking platform in the kiln when the pneumatic brick ring was charged with air. The air blew a small amount of dust into the employee's right eye. Employee was unable to dislodge the object with flushing of the eye.

Employee was putting ceramic in the pulverized fuel pipe when he felt something go into his right eye. He tried to flush it out with eye wash, but was unsuccessful. After seeing the optometrist, it was found that he had a piece of metal that was embedded in the eye.

Employee had spent over 7 hours in the raw mill working in a bent over position on discharge grates. While tightening bolts, he broke an Allen socket and was trying to remove the socket when he felt a pop in his lower back and pain associated with it.

Employee was using a cutting torch on a fuel tank when something flew into his left eye. The foreign object was under his left upper eyelid, and after several attempts to wash the object out to no avail, the employee sought medical help. The employee was wearing safety glasses with side shields, but did not put goggles on.

The employee was walking down an inclined road from the Primary Crusher when his right foot slipped on some ice. His left knee hyper extended underneath his full body weight. He sustained a torn quad tendon in his left knee.

Employee was pulling a cord out of a locker on the precipitators. He thought it was an extension cord, but it had a solenoid on the end that came down and hit him in the upper lip. Employee sustained a 1/3 inch laceration to the upper lip.

Employee was hauling yard clean-up with a truck. A 2 foot by 2 foot rock had rolled down a hill and was in the roadway. He exited his vehicle to roll the rock to the side of the road by himself. During the course of rolling the rock, he felt a pop in his lower back, causing pain.

The employee had been hooked in the ribs on 8/1/2010 by a steer. He was starting to heal by the time of the accident on 9/1/10, but had not seen a doctor for the steer incident. At work on 9/1/10, he was pulling the lid on the turbulator and felt pain in his right rib and muscle on the right side of his back and chest.

Employee was carrying a 40 pound breaker bar. One end slipped out of his right hand, smashing his left ring finger tip between the breaker bar and the basket he was setting it into.

On 3/5/10, employee had carpel tunnel surgery on bilateral hands, due to a cummulative numbing of the hands and arms. Employee alleges this was attributed to 30 years of using hand tools while working as the plant mobile mechanic.

Employee was attempting to step into the welding truck. His right foot was on the ground and the left foot was on the step when he slipped on the ice, twisting his back. He didn't think much of it at the time, but on 02/18/10 his lower back was extremely sore and he had a hard time bending over to put his boots on.

Employee was moving a wheelbarrel loaded with material from underneath a belt on the raw belt floor when he felt a pain in his lower back, followed with a tingling in his feet.

Employee was climbing off of the 988B Cat Loader when his boot slipped off the rung. He fell backwards onto the ground, landing on his back heels, then onto his buttocks. Employee sustained a lumbar disk injury.

Employee was welding in the top of the #1 precipitator. Flux on the welding rod came off and the rod burned up to and through his cloth glove, causing a 2nd degree burn on the left index finger. On May 23rd, the finger became red and swollen. The employee sought medical attention at that time and was diagnosed with an infection, which required antibiotics.

EE fell off of his roof in 12/04.He sustained a right ankle fracture requiring a steel plate.on 5/8/08 the EE had swelling and pain to the right ankle that he alleges is the result of carrying 40pd buckets on a concrete surface during a 3 day period at the plant location.He saw his Ortho Dr on 5/16/08,who determined it was work related.

Employee was manually rotating the turbulator pins on the turbulator, when his left ring finger got pinched between the pin and the turbulator casing. The pinch peeled the skin back on his finger, which required 9 stitches. Employee had properly locked equipment out and was wearing gloves.

Employee has had skin rashes at intermittant places on both arms and legs for approximately 3 years. While seeing his physician for an unrelated matter on 3/3/08, he consulted him about the rashes. The physcian thought it was related to the cement dust at the plant.

Employee was removing a plate on a bearing when the plate fell. It smashed his left ring finger tip between the plate and the base of the bearing.

Employee was using a jackhammer bit to break up a cement chunk that was stuck in cement silo 4. He missed the cement chunk and hit his middle finger and ring finger on the right hand. The right middle finger sustained a contusion and right ring finger sustained a deep laceration, which required 6 stiches to close.

EE was entering the cab of the water truck after filling the truck with water for a dust supression tour for roads and quarry area. While attempting to enter the truck, EE's right foot slipped on the wet truck step. EE lost his grip on grab bar and was suspended momentarily with his shoulder in a hyper-extended position, holding the steering wheel w/left hand.

EE was grinding on the chain of the bucket elevator, wearing safety glasses & a face shield, when a piece of metal imbedded into his eye. EE did not find out until 6/6/07 the fragment was imbedded.

Employee was re-installing the drill bit on the Ingersoll/Rand drill. He had lifted the bit collar up to thread the bit into the hammer. The bit seemed to be in a bind, so he bumped the hammer with his shoulder to straighten it out. The hydraulic pressure had bled down, binding the steel. His left index & middle fingers were caught between the bit & the bit collar.

Employee was swinging an inspection bar to secure the bottom door lock on a railcar and injured his lower back.

Employee was running one inch impact wrench with a socket that has the roll pin and keeper. The rubber keeper broke and the roll pin came out, causing a puncture laceration on the meaty part of his left palm.

Employee & co-worker were removing bolts from the secondary crusher liners. EE grabbed ahold of the bolt head at the same time the co-worker used an air ratchet on the nut. The air ratchet spun the bolt, lacerating the EE's right thumb & index finger. The EE received 9 stitches at the clinic.

Employee was washing his haul truck. He stepped off of the back of the truck with the water hose and twisted his back. Employee didn't seek medical attention until 7-31-06.

EMPLOYEE WAS SWINGING A SLEDGE HAMMER TO KNOCK OPEN THE DOOR LOCK ON A RAILCAR (FIFTH ONE THIS MORNING) WHEN HIS BACK SPASMED, SENDING HIM TO HIS HANDS & KNEES.

Employee was in the process of removing a steel tube on the IR 185 Compressor oil cooler line, when he cut his left middle finger on the tube.

Employee was checking the flow on Silo A when he opened the cap on the clean-out port after having put air in the system. Once the cap was off, the air blew Synthetic Gypsum onto him. Employee sustained first degree chemical burns to his armpits, back, legs, stomach, & buttocks.

EMPLOYEE WAS OPERATING THE 988 CAT LOADER. HE WAS FILLING THE BUCKET WITH LIMESTONE FROM THE QUARRY AND WHILE DIGGING INTO THE FACE OF THE WALL, CONTACTED SOLID ROCK. THIS JARRED HIS BACK, CAUSING DISCOMFORT BETWEEN HIS SHOULDER BLADES. HE WENT TO SEE THE CHIROPRACTOR ON 7/20/05.

EE was working on the west "man-door" entrance to the coal mill building. He had the door propped open with a shovel. when he stood up, he knocked the shovel, the wind caught the door and slammed it against the left little finger, partially amputating it above the 3rd joint nearest the fingernail.

EMPLOYEE SLIPPED ON TOP OF BC-2 & WRENCHED HIS RT. ELBOW TRYING TO CATCH HIMSELF. HE SOUGHT MEDICAL ATTN. ON 4/22/05.

EE WAS WORKING ON BE-3 TRACTION. EE HIT THE BEARING WITH A HAMMER AND A SMALL PIECE OF METAL HIT EE IN THE END OF THE NOSE. EE NOTICED A FEW DAYS LATER THAT A METAL SLIVER HAD BEEN EMBEDDED IN THE TIP OF NOSE

EE WAS INSTALLING SHAFT SLEEVES ON THE SECONDARY CRUSHER ROTOR. THE SLEEVE HUNG UP ON THE SHAFT SO EE CONTINUED BEATING ON THE SLEEVE WITH A RUBBER MALLET. EE FELT RIGHT WRIST POP AND OVER TIME IT WASN'T GETTING BETTER. EE SOUGHT MEDICAL ATTENTION ON 12/15/04.

EMPLOYEE WAS CROUCHING DOWN TO PICK UP SOME WOOD RAILS. HE FELT HIS LOWER BACK "POP" DURING THE ACT OF CROUCHING DOWN.

EE WAS INSTALLING NEW GRATES IN THE EAST SIDE OF THE SECONDARY CRUSHER WHEN HE HURT HIS BACK BY WRESTLING WITH THE GRATES AND HANDING THEM THROUGH THE INSPECTION DOOR. HE FURTHER AGGRAVATED TH E CONDITION WHEN LIFTING A CHAIN FALL JIG ON 1/20/04.

EE WAS CLIMBING OUT OF THE RT-80 (MOBILE EQUIPMENT) WHEN HIS FOOT SLIPPED OFF OF THE STEP. HE WAS HOLDING THE HAND HOLD WITH HIS RIGHT HAND AND HURT HIS RIGHT SHOULDER WHEN HE TRIED TO KEEP FROM FALLING.

EE WAS WORKING UNDERNEATH A LOADER, WHEN A PIECE OF SILICA FELL INTO HIS RIGHT EYE. THE PIECE OF SILICA BECAME EMBEDDED IN HIS CORNEA.

EE BRUSHED THE TIP OF A TORCH WITH HIS LT. INDEX FINGER NOT KNOWING HE HAD A SMALL HOLE IN THE END OF HIS GLOVE, WHICH ALLOWED THE FLAME TO ENTER THE GLOVE. HE SUSTAINED A 3RD DEGREE BURN TO T HE TIP OF HIS LT. INDEX FINGER.

EMPLOYEE WAS HOLDING A STAKE FOR A CO-WORKER TO HIT WITH A SLEDGEHAMMER. THE INJURED EE PUT HIS WEIGHT ON THE STAKE WITH HIS RIGHT HAND TO STAND UP, WHEN THE CO-WORKER CAME DOWN ON THE STAKE WITH THE SLEDGEHAMMER.

EE WAS OPENING A RAIL CAR DOOR WITH AN INSPECTION BAR WHEN HE PULLED THE VERTABRAE IN HIS UPPER BACK.

EE WAS JACKHAMMERING CONCRETE AROUND BC-10 WHEN A SMALL PIECE OF STEEL BOUNCED OFF THE INSIDE OF HIS SAFETY GLASSES, LODGING INTO THE IRIS OF HIS RIGHT EYE.

EMPLOYEE WAS POSITIONING A 9 FT PIECE OF 2 INCH CONDUIT ON BUCKET ELEVATOR 5 WHEN HE FELT DISCOMFORT IN HIS BACK.

EE WAS USING A PRY BAR TO REMOVE SOME LINER PLATES FROM THE BACK END OF THE KILN WHEN HE EXPERIENCED PAIN IN HIS BACK AND NECK.

EE WAS LIFTING SOME BREAKER ASSEMBLIES FROM AN AWKWARD POSITION WHEN HE FELT PAIN BETWEEN HIS SHOULDER BLADES. INCIDENT WAS NOT REPORTED TO EMPLOYER UNTIL 1-17-01.

EE WAS JACKHAMMERING CASTABLE REFRACTORY IN THE COOLER. THE VIBRATION DISLODGED HOT CLINKER DUSTWHICH FELL FROM AN ELEVATED LEDGE ONTO EMPLOYEE.THE HOT DUST BURNED HIS NECK, EARS, WRISTS, AND LEFT CALF.

EE WAS OPENING THE BOTTOM DOOR OUT OF A RAIL CARWITH A LONG BAR. THE DOOR WAS TIGHT & EE FELT HIS BACK GO OUT OF PLACE

EE WAS WORKING ON DA-21 FAN, STANDING IN AN AWKWARD POSITION WHEN HE BENT OVER TO PICK UP AN IMPACT GUN. HE EXPERIENCED PAIN IN THE LOWER BACK, RESULTING IN LOWER BACK STRAIN.

EMPLOYEE FELT A SMALL TWINGE IN HIS LEFT LOWER BACK WHEN PICKING UP A SMALL BARRELL OF GREASE THAT HAD TIPPED OVER WHILE HE WAS CHANGING A DISPENSER. ABOUT 15 MIN. LATER, HE PICKED UP A 5 GAL LON BUCKET TO EMPTY THE WATER OUT. HE WAS UNABLE TO PICK UP THE BUCKET AND FELT SEVERE PAIN IN HIS LEFT LOWER BACK.

EE BENT OVER TO PUT GAGE ON ACETYLENE BOTTLE AND FELT PAIN IN HIS LOWER BACK.
