Employee was found on the floor of the #2 slurry tank. Employee had significant head injuries and was pronounced dead by the Justice of the Peace. The accident scene is currently under a Section 103(k) order and subject to MSHA investigation.
Truck driver's lid got stuck on safety cage and when he tried to pry it free, it came loose and sprung and hit him. He fell 11 feet. This was told to me by Ralph Rodriguez.
Several utility workers were loading mill rods into the raw mill. As they were unloading a rod from the forklift forks, their finger was caught in between the hand rail and bottom of rod.
EE sustained laceration from metal fragment. Clinic prescribed RX antibiotic as preventative measure.
Production tech was removing a slurry hose for cleaning, EE had shut off all valves as well as open the pressure relief valve to get rid of excess pressure. When removing the camlock there was still residual pressure and caused slurry to spray on EE's face, chest and right wrist.
Electrician was performing normal duties, troubleshooting a blower motor. It was hot in the area they were in and began to feel ill. They radioed that they needed help and when other employees got to them called EMS when they arrived they gave them fluids and determined they were dehydrated.
Laborer was working with the vac truck and needed to cut the old hose off and add a new section. While cutting the bad section, employee was using a utility knife, cutting towards their body when the knife slipped and cut their hand, right above the wrist.
Contractors were in the clinker passenger elevator, the car stopped abruptly halfway down, entrapping them. They contacted their manager at 11:37 to alert them that they were stuck. At 12:07, the Safety Manager called MSHA to report an entrapment. Plant electricians were called to reset the car, they were successful and were able to get them to the ground and extracted safely.
A power outage at the 491 Substation knocked out power to the kiln system, which includes the PHT elevator. At the time of the outage, a production technician was in the elevator and became trapped, at the 30 minute mark MSHA was notified. through the hatch at the top of the elevator car. Employee was able to self-rescue when power was not restored.
Employee was using a sledgehammer to break a mill door free, had to switch tools and when EE reached out to grab the other tool felt a pop in EE's lower back.
Upon exiting the elevator, the micro switch for the inner door failed not reading that the car was on the ground, and started going back up. The elevator went up about 12 feet and stopped, not allowing the employees to exit. There were no injuries sustained.
EE was bitten by a black widow spider on the right calf while picking up and transferring a fire extinguisher.
EE was working with a jack hammer and was removing material with a wheelbarrow (with the assistance of other EEs). EE left the shift fine, woke up the next day with back soreness. Was treated initially and told to tend with self-care. On 3/22 went to see a doctor, doctor diagnosed with a potential bulging disc. Eventually was prescribed, prescription grade medication on 3/27.
An experienced carpenter of 15 years was hired to build forms for concrete. While cutting stakes the IW encountered a knot. Holding the Makita circular saw, in left hand, the saw bound up and kicked back, striking IW's right thumb. IW was taken to the ER, treated and released. On 3/16/24 IW received stitches.
Miner was walking around within the mill room, perform nightly rounds when stepped on a mill ball and twisted their ankle.
At 6:48 pm the console operator dialed 911 in response to a confirmed fire on the clinker belt that feeds our clinker shed. MSHA was notified around 7 pm, and the local Fire Department arrived on-site at 7:04 pm. The Fire Department was able to extinguish and secure the area within 15-20 minutes upon arrival.
No injuries. EE got stuck in clinker elevator due to issues with the door switches and the way they communicate to the system. MSHA was notified due to entrapment EE was removed shortly after 2 hours.
EE reached overhead to disconnect an air line from a quick connect 3/8 air hose. The EE failed to bleed the pressure off the line before disconnecting it. EE lost control of the air hose it bounced off the structure and came back and struck EE on the upper lip requiring three stitches.
EE tripped in the clinker cooler and fell on left knee and sustained a 1.1 cm fracture to the knee cap.
EE trapped in an elevator to clinker silos. Electricians had to be called out to access the emergency switches to raise the elevator. Techs arrived on 8/26 and found a limit switched failed and need to be replaced.
Fire occurred to the High Angle Conveyor (HAC) when power was lost to the plant. A set of triple airlock gates and a knife gate separate the belt from the hot gases. The knife gate failed to shut in fail-safe mode when a valve was inadvertently left off. The HAC belt ignited and burned near a Continuous Emission Monitoring Station. Six compressed gas bottles heated and ruptured.
EE was using a wire wheel on a grinder. It did have a guard and the trigger lock is not present. EE claims that the wheel caught and torqued toward them. It made contact with their right leg. EE was taken to the hospital and given stitches. No restrictions or light duty.
EE claims an injury on 2/19/19 that EE hopped and turned midair to avoid another EE brushing against EE. None of the crew remembers the incident. EE reported on 2/20/19. Seen by Dr. on 2/22/19 and diagnosed with Scoliosis. Medical evaluation on 3/25 determined pain to be work related.
EE stepped awkwardly off the last step and rolled their ankle. EE took their normal days off and returned on the 2/22/19. EE asked to be seen by a doctor. An x-ray shows that they have a fracture.
Employee was cleaning mud from a tower using a water lance. They stepped backwards and slipped on material they just cleaned from the immediate work area. Employee was seen by a Dr. on 2/4/19 and placed on restricted duty.
Employee was using a pulley and winch snatch block to lower thimbles into a hole. When pulling the pulley/winch snatch block back up, EE grabbed the wire and it caught EE's left index finger and cut EE's finger below the finger nail.
Contractors in the cement silo passenger elevator became trapped when one of the safety switches tripped (opened). It took longer than 15 minutes to reset the elevator and get the men down.
EE was preparing to remove a chunk from a cement silo cone. When the port was opened material flowed out. While trying to close the port dust got into EE's eyes. EE was seen by a Dr. and prescribed antibiotics.
EE was replacing a spout plug in the shipping silos spout. When EE looked up to align the plug with the bolt holes dust fell into EE's eyes. EE was wearing safety glasses and hard hat. EE had lowered the spout and knocked any loose material out before beginning work.
Fire was possibly started by welders working in the area earlier in the day. MSHA was called but the length of the fire did not meet MSHA report. Hot work permit issued and followed. Coal system has been locked out. No coal was in the day tank. Belting was the only loss. Fire was reported at 6:21pm and extinguished at 6:28pm.
At 10:16 an employee reported was stuck in the elevator at the cement silos. The elevator car was about 3 to 4 feet above ground. The employee was able to get out at 11:16. The elevator over speed brake locked and stops the elevator from operating.
EE was on level 3 of the tower. Pallets of material was being lifted up to this level by a crane. EE was using a tag line to help guide the load over a handrail when ee felt a pain in right bicep. EE returned to job but on 7/20/17 during a follow up visit the Dr. Prescribed physical therapy.
Employee was using a come along and choker to pull the HDR. The HDR had become stuck and would not move in either direction. When EE pulled with the come along the choker failed and the hook came back and struck the EE. EE sustained a broken pinky finger on EE's right hand.
A small fire was located in the southwest corner of the coal stockpile. The excavator was located in the quarry and had to be brought up to stockpile to dig the fire out. Fire was extinguished at 9:48 while on the phone with MSHA 1 call. High winds and spontaneous combustion are the likely cause. No worker activity is being done in this area.
Employee was stuck in an elevator for greater than 30 minutes. Door limit switch was not being made. Service technician arrived and got employee out at 5:18PM
A glow on top of the coal stockpile was observed. It was confirmed to be 3 small flames. Because of location it required an excavator to extinguish. The excavator was located on the other side of the plant. Because of pre-shift and travel time it took longer than 30 minutes to extinguish. The fire was extinguished at 9:51 p.m.
While stepping the EE felt a pop in left knee. EE was seen by the doctor on 10/24/2016 and prescribed physical therapy.
Service Technician was diagnosing elevator issues from the 4/6/16 incident. The technician tripped a door safety switch and the elevator stopped. The elevator system would not reset and a second technician had to be called. It took about 30 minutes for that technician to arrive and reset the elevator.
Personnel elevator at the cement silos stopped within 1 feet of the bottom floor. The exterior door would not open and it took a service technician over 30 minutes to arrive and reset the elevator.
Removing cleaning build up from inside walls of dryer crusher using high pressure water. The cleaning was being done from elevated outside cleanout ports. Due to build up on bottom of vessel opening with limited access to this area men began cleaning from below bottom opening. Material buildup on vessel broke loose during operation and struck men causing bodily injury.

Following a temperature and pressure increase in the bag house, five vent doors opened as designed. Investigation is still ongoing regarding the circumstances underlying the temperature and pressure increase.
EE was working on an auto sampler. Air was turned off and the electrical equipment was turned off. Residual air pressure in the lines allowed the unit to cycle when the EE freed the blockage with his finger. The EE lost about 1/4 inch of his left index finger including some bone. Stitches to close the wound
EE was cleaning out a chute when the material broke free and forced the jackhammer into his neck. EE sustained a laceration to right side of neck that required stitches.

Employee was working around cement kiln dust when some got in his boot. He didn't try to clean and neutralize the area and therefore received an alkali burn.
During a coal mill system start up, a fire occurred in the coal mill system. The fire forced open the pressure relief doors on the lower level of the bag house. The CO2 suppression system automatically initiated, and the fire was extinguished in approximately 10 minutes.
Employee was inspecting equipment (Iron Ore Slide Gate). After inspection, the slide gate was removed and in hand and brought towards his body to secure. Employee slightly turned his left side and the left side of his nose made contact with conduit support. Nose began to bleed.
A computer glitch during new kiln commissioning caused a kiln shutdown. Approx. 1200 degree F. air was forced into a rubber duct due to a drop in positive air pressure. The hot air eventually caused the duct to catch fire from the inside out.
Employee was using an adjustable wrench to tighten a turnbuckle. The wrench slipped off and struck the left side of his nose.
Clinker exited kiln, fans were not shut off, and dust exited the dust collector @ clinker silo #22
EE (5 Point contractor) left a cresent wrench in the housing that he went to grab and when he placed his and on the angle iron frame to reach into fan housing the set screw that is a 7/16"" bolt, which has been known to fail in previous encounter, sheared and caused the fan and housing to fall from set position to the bottom side of frame.
On 04/02/2014 the EE was shoveling sand in sand tunnel & felt a sharp pain in his back/buttocks area. He reported the incident on 04/03/14 to his Supervisor & was taken for MD assessment. EE was not assigned any restrictions, and returned to full duty that day. On 04/29/2014 EE asked reported pain, taken to MD & was given a steroid shot. No restrictions, & returned to full duty.
Employee had neck tilted back looking at light that was not working on elevation above and wind blew trash containing metal shavings into his eye. Employee was taken to minor emergency clinic (Care Now)and was referred to an opthalmologist for removal of the metal. This was done and he was returned to work.
Employee was hurrying from one job to another when he struck his right leg against a tensioner bolt for a conveyor tail pulley causing severe bruising.
While inspecting welds of inlet plenum section of Main Bag house, employee stepped in hole for inlet duct. Employee fell approximately three (3) feet onto inlet vane.
Employee was rigging a hydraulic cylinder using a chain fall. When the load became suspended, it shifted to one side trapping the 4th finger on his right hand between the load and the chain.
Cleaning service person used office elevator to access another floor (2 story bldg.) when the elevator got stuck. Total time of entrapment was 38 minutes.
Illness - chronic knee condition. Employee felt soreness in knee by the end of the shift. Knee was hurting for several days.
A contractor who was installing piping on the top of the cement silos became trapped when the elevator became stuck for appox. 2.5 hours. An elevator service mechanic was called out to the plant to get him free.
While loosening bolts to remove a section of ""air slide,"" the air slide came loose suddenly and fell onto employee's arm
As a supervisor was making plant rounds, he noticed a smell of smoldering / slow-burning coal in an unused coal hopper. No damage occurred, but hopper was hot and coal was in danger of igniting.
Employee was reaming a hole using an electric end grinder. The wind blew a foreign object into his left eye. Employee was wearing safety glasses and a full face shield. After flushing the eye with eyewash, employee stated that his eye was still irritated. Employee was taken to Care Now clinic. No foreign object was discovered in his eye, but the eye was slightly scratched.
Employee fell from third rung of 12 foot ladder in an attempt to avoid dust falling into his eyes.
Worker was walking around a piece of duct when he stepped into a floor opening adjacent to a fixed ladder. His right knee and left elbow struck the edge of the floor grating as he fell.
EE was building scaffold and passing material overhead when he felt something drop into his right eye.
Employee was traversing from one scaffold platform to another when he miss-stepped. His foot was caught between platforms when he twisted his body and injured his foot.
Employee was welding and his eye felt irritated at the end of his shift. The irritation didn't go away so several days later he sought medical attention. A foreign body was found in his right eye.
EE was cutting a 2x4 and saw got in a bind and jumped back cutting his left thumb.
A port was opened to unclog a blockage at cement silo. Material rushed out of the port and got in employee's eyes.
Employee was welding with improperly fitting welding hood. Employee noticed eye pain consistent with ""flash burn"". He was treated with antibiotics to prevent infection on 7/11/13
Employee was on a ladder using a welder on a metal frame. The frame suddenly broke loose and then struck the ladder causing the employee to fall approx. 4 feet.
Employee lowering a caisson cage into a drilled pier opening. A wood beam used in stabilizing load was dropped into the hole causing the end of the beam to strike employee in the chin causing a laceration.
The operator & 2 helpers were attaching the jib to the boom of a 90 ton hydraulic crane. The operator removed the pin on the side of the boom before he installed the 2 pins that hold the jib in place. This caused the entire jib to drop to the ground. One side of the jib struck the operator's hard hat as it fell to the concrete paving.
The employee was working with other maintenance employees to align a section of conveyor system. As he attempted to position a piece of the conveyor system, he felt a ""pop"" in his back and reported his injury.
Finger got caught between leverage bar and railing when EE was changing out a valve gate on a silo. Employee was injured on 3/11/13, but sought medical care on 3/25/13.
EE was removing a 3/4"" bolt using a Tone SR71E Simple Torque gun. As the bolt became loose, he released the trigger and reached in to remove the bolt while the gun was still spinning. His left thumb became pinched between the torque reactor bar and the steel beam. His basic reaction was to pull his thumb free. As he did, the pinched condition caused his thumb to be injured.
Employee had been working on a platform beneath a silo cone. As he descended the fixed ladder from the platform, he stepped down onto the floor and ""rolled"" his ankle causing a strain / sprain.
The injured employee was using an impact wrench to remove a nut from a piece of equipment. He lost control of the wrench and it fell onto his ungloved thumb causing the injury.

Employee was pulling material out from under a coke rail car using a come-a-long (pull rake). He was pulling a large chunk of coke from under one of the rail car doors when his left foot slipped on coke beads causing a strain to his left knee.
Employee w/ previous back issues bent down to inspect equipment. The bending action caused pain in his back.
Employee was using a plant made ""air lance"" to clean debris off a guarded tail pulley. He had finished and was disconnecting the air lance when his head hit a low overhead beam and these two motions combined pushed the air lance into the tail pulley which then jerked it from his hands.
Employee was loading empty compressed gas bottles into the back of a pickup. As he was doing so, his left thumb was caught between a bottle that he had previously loaded and the one he was currently loading causing a pinch point causing a fracture.
Employee was repositioning iron bars (mill rods) to be removed & sorted. The weight shifted and his gloved hand was caught between two bars. The quick shift action along with the weight of the bars caused a severe pinch on his left middle finger.

Employee was polishing a 5/8"" steel shaft w/ emery cloth and his gloved hand was pulled into the spinning shaft causing a cut and finger dislocation to his left index finger.
Employee was standing on a grating cleaning a hopper below using an ""air lance"". As he moved to reposition the lance, he stumbled backward, fell down, and landed on his right shoulder.

Employee was removing material jammed along a conveyor. He didn't lock out the conveyor and his hand slipped into a nip point between a bent roller and the machine frame causing the loss of his left thumbnail along with a fracture in this thumb tip.
Employee tripped over a piece of metal bracing on a lifting device and hit his head and hands as he fell.
Roofing employee moved a piece of plywood that was covering a hole in roof. As he moved the plywood, he fell through the hole that the plywood was covering. He was stopped by his retractable life line attached to his body harness. As he fell, his left knee struck the guard rail on a catwalk below.

Employee picked up a fiberglass handle from a shovel that was broken and had splinters. One of the splinters entered into his left index finger.
Employee was using a come-a-long hoist to lift/place a metal ramp. The come-a-long failed to securely engage and the handle spun around and hit the employee in the hand (severe bruise to right top of hand).
Finger was caught in a pinch point between a pry bar and a railing.
Heavy metal door on a clean-out shaft ""popped"" open from pressure of material behind the door. This sudden action pinched the injured employee's right ""pinky"" finger between the door and the wing nut he was removing.
Employee fell off a 3-foot step ladder while scraping build-up off a piece of machinery. Employee was standing on top step of ladder and not using any fall protection / restraint.
Employee was shoveling sand into a wheelbarrow and repeated twisting motion of the wrist caused tendonitis and sore muscles
A line containing motor oil ruptured and injured employee. The high pressured oil cut into employee's clothes, and bruised and abraided his chest and finger. An incorrect pump was used, and identified as a grease pump after the safety incident. The incorrect pump over-pressurized the line causing the it to break at the weakest point in the system.
Employee struck foot against a piece of machinery; clutter & lack of attention to surroundings was a contributing factor; he was given light duty, medication, and crutches by the doctor
Employee slipped footing while exiting trailer mule. He hurt his back in the process.
Hernia & surgery 2004. Dr. release in February 2005. 4-12-05 his stomach hurt in that naval area & the lower right. He did not want to go to our Dr. at the time & he was already scheduled to see a Work Comp Designated Dr. on April 20th for last year's injury. At that appt., the Dr. said he had an inguinal hernia in the lower right groin area-this was a new injury.
Employee stepped on rake and got hit on face.
His hand was caught between pneumatic shim driver and shim. The head on the driver pinched him.
The welds on an anchor for a hand hoist failed under tension and metal anchor and buckle snapped back and hit his elbow. X-ray does not show fractures. MRI does not show fractures. MRI shows contusion, partial tear of tendons and ligaments in the elbow area. No lost time. Prescribed pain medication.
Employee was pulling kiln shell together using an air torque wrench. Another employee was helping and pulled the trigger of the torque wrench before the injured employee was ready. Employee has an open fracture of distal left fifth digit with soft tissue injury.
Employee's thumb was pinched between edge of metal barrel and metal plate. Wrong hand placement while positioning drum.
An employee was using a screw driver to apply force on two metal surfaces to seperate them. The screw driver slipped and under the force of her own weight, the employee fell forward and her face hit a part of the hopper below her face. The injury resulted in a cut and soft tissue damage.Stitches where applied by emergency physician.Pain and antibiotics where prescribed.
Employee was unplugging pipe with a long bar and he felt a sharp pain on lower back.
Employee was shoveling sand onto a hopper. He shoveled a pile that was close to a conduit when his shovel unexpectedly hit the conduit. He felt a sharp pain on his left forearm and he went to the first aid station.
Employee was carrying two 10 lbs boxes at waist level and stepped into the burner pipe which was having cast able anchors welded on. The employee was looking at the floor to step over the welding leads and ran into an anchor which is very sharp due to being sheared at manufacture.
Employee was working on a platform with a square 32 inch hole cut out on the floor. Employee was not wearing fall protection and walked too close to it and fell 27 feet.
Standing on tailgate of a pickup helping to unload 6"" di. hose. The hose was stuck and he pulled on it when the hose became unstuck, he fell. He tried to step back to catch himself but a chain that holds the tailgate caught his leg and tripped him. He tried with his right hand to catch himself but it cut his hand and he still fell. He landed on his shoulder and hit his head.
Employee was picking up a 5 gallon bucket of a mixed refractory product (approx. 35 lbs.) He was bent over sideways at the waist to lift the bucket and felt a sharp twinge on his left side lower back. He continued to work till later in the afternoon when the pain got worse. He went through the weekend and Monday till Tuesday when he asked to see a doctor, it was still hurting.
Employee was operating a chain whip to clean out a gypsum silo. He was laying on his stomach turning a crank above his head. This body position and body movement strained his back muscles. He should have asked for help or turned the crank in a sitting position.
EE was pulling a concrete come-a-long to remove clinker spillage. He was on both knees as close to his work as he could get. He had been working in this location for approx one hour when he felt a pain in his abdomen. He reported it to his supervisor and to first aid station.
EE WAS JACKHAMMERING AND SHOVELING HARD BUILD UP INSIDE THE UNLOADING PIT WITH OTHER EMPLOYEES. CONDITIONS WERE HOT AND DIRTY. HE HAD HIS SHIRT SLEEVES ROLLED UP WHILE PERFORMING TASK. AFTER AWHILE, EMPLOYEE ROLLED SLEEVES DOWN AND ABOUT TWO HOURS LATER HIS INSIDE OF BOTH ARMS STARTED BURNING ON PRE-EXISTING SUNBURN.
EE WAS REPAIRING A CEMENT PUMP. THEY NEEDED TO DISASSEMBLE THE PUMP AND REMOVE 8 INCH SCREW SHAFT. THEY ATTACHED CHAIN IN A CHOKING MANNER ON THE SCREW. HE THEN PROCEEDED TO PULL SCREW OUT WITH THE HOIST. CHAIN CAME LOOSE AND LASHED TOWARDS HIM, HITTING HIM IN THE MOUTH AND PUNCTURING THE SKIN ON HIS UPPER LIP. RECEIVED 7STITCHES ON HIS TOP LIP.
WHILE WORKING WITH THE PLANT MAINTENANCE DEPT DURING ANNUAL SHUT DOWN. HE GOT HIS LEFT INDEX FINGER CAUGHT BETWEEN A POPPIT VALVE DISC SEAL HE WAS INSIDE THE CLEAN AIR DUCT WITH A WRENCH ATTEM PTING TO LOOSEN THE JAM NUT ON THE POPPIT VALVE EE REPAIR WITH 8 YRS EXPERIENCE WAS TIGHTENING TO ADJ. NUT ON THE OPPOSITE SIDE OF THE DISC. WORKER WAS ALSO WORKING WITH MAINTENANCE WAS HOLDI
EE WAS PRESSING A BEARING OFF A FAN SHAFT. HE HAD PRESSED THE SHAFT OUT OF THE BEARING AS FAR AS THE PRESS WOULD ALLOW BEFORE COMING INTO CONTACT WITH A CROSS BAR AT THE BOTTOM. THE BEARING WA S VERY CLOSE TO BEING OFF THE SHAFT. HE THEN REACHED UNDER THE PRESS TO REPOSITION THE SHAFT OFF THE CROSS BAR, WHEN IT CAME OFF THE CROSS BAR. IT CAME OUT OF THE BEARING AND FELL DOWNWARD, M
A SHEAR PIN NEEDED TO BE REPLACED IN THE GRATE DRIVE AND EE WAS USING A 2 POUND HAMMER AND PUNCH TO DO THIS. THE HAMMER WAS A COMMUNITY TOOL USED BY EACH OF THE PRODUCTION CREWS IN THEIR JOBS. EE IS FILLING IN ON THE SHIFT FROM THE UTILITY CREW AND DOESN'T HAVE HIS OWN TOOLS. THE HEAD ON THE HAMMER WAS NOT SECURED AND WHEN HE BROUGHT IT BACK THE HEAD CAME OFF STRIKING HIM IN MOUTH
EE WAS UNPLUGGING #1 RAW MILL THROAT. HE HAD A BAR STUCK DOWN IN THE MATERIAL, & WAS HITTING THE BAR WITH A SLEDGE HAMMER WHEN HIS HAND SLIPPED UP THE BAR & WAS HIT BY THE SLEDGE HAMMER. HE WAS WEARING GLOVES WHEN THE ACCIDENT HAPPENED.
EE AND TWO COWORKERS WERE WORKING TAKING LINERS OUT OF THE FINISH MILL WHEN THEY KNOCKED A LINER OUT. IT FELL AND HIT THE LINER ON THE BOTTOM. A PIECE CHIPPED OFF AND HIT EE IN THE RIGHT LEG B ELOW THE KNEE AND STUCK IN HIS LEG.
EE WAS REPLACING A HOSE ON THE SLURRY ANALYZER AND WASN'T SURE OF THE SIZE OF THE HOSE. WHILE CUTTING A PIECE OF HOSE OFF HE CUT HIS RIGHT INDEX FINGER.
TRUCK DRIVER FOR A TRUCKING COMPANY PARKED HIS CEMENT TANKER TOO FAR FROM ACCESS PLATFORM AND FELL WHILE TRYING TO ACCESS TOPOF CEMENT TANKER CAUSING FRACTURES IN LEG, HIPS AND SKULL.
WHILE EE WAS INSTALLING CONDUIT ON BELTS 3 TO BELT 2 TRANSITION TOWER- HE PICKED UP A STEP LADDER TO REPOSITION IT. AT THIS TIME HE FELT DISCOMFORT IN HIS LOWER BACK.
EE WAS CLEANING THE DUST CHAMBER THAT HAS TO BE CLEANED EVERY SHIFT. HE WORKS WITH AN AIR LANCE TO REMOVE DUST THAT HAS BUILT UP ON THE SIDE WALLS. EE WAS REMOVING THE BUILD UP FROM THE SIDE W HEN A LARGE SLAB OF DUST CAME DOWN HITTING THE AIR LINE AND HIT THE EE IN THE JAW. HE WAS KNOCKED UNCONSCIOUS. IT IS NOT KNOW HOW LONG HE WAS OUT.
EE WAS TRIMMING HEDGES AROUND THE FRONT OFFICE BUILDING, AND GOT INTO SOME POISON IVY.
EMPLOYEE WAS WORKING AT #1 FINISH MILL. SHE WAS HELPING REMOVE GRINDING BALLS FROM THE MILL. THEY WERE DUMPING BALLS VIA A SPECIALLY MADE DISCHARGING DOOR WHEN ONE OF THE BALLS CAME OUT AT AN ODD ANGLE, STRIKING HER ON HER LEFT CHEEK THEN GOING DOWN HER SHIRT BURNING HER ON HER BACK AND WAIST.
EE WAS MOWING THE GRASS AND BRUSHED AGAINST THE PEAR TREE BEHIND THE BREAKROOM. THIS DISTURBED THE YELLOW JACKET THAT WAS IN THE TREE AND CUASED IT TO ATTACK THE EE.
EE WAS HELPING JACKHAMMERING CHUTE OUT WHEN THE JACKHAMMER BIT GOT HUNG. HE WAS TRYING TO PULL THE BIT OUT AND FELT BURNING IN HIS GROIN ON THE LEFT SIDE.
THE EMPLOYEE WAS CLEANING OUT A CHUTE TO RAW STORAGE BUILDING. THIS CHUTE WAS PLUGGED AT THE TRANSFER TOWER THAT DIVERTS LIMESTONE FROM THE BUILDING TO THE LIMESTONE TANK. THE FLOP GATE FLOPPE D HITTING THE RIVOT BUSTER AND PINNED HIS HAND TO THE WALL OF THE CHUTE BREAKING THE BONE IN HIS LEFT HAND FIFTH FINGER.
EMPLOYEE WAS A MEMBER OF A 4 PERSON CREW ASSIGNED TO CLEAN-UP THE COOLER ROOM. EMPLOYEE WAS ON THE NORTH SIDE OF #1 CLINKR COOLER ABOUT #3 FORCED AIR FAN, SHOVELING PILES OF CLINKER DUST INTO A WHEEL BARROW WHEN HE SHOVELED INTO A PILE AND HIS SHOVEL CAME IN CONTACT WITH A CONCRETE PIER HOLDING UP THE COOLER CAUSING HIS RIGHT WRIST TO BE SPRAINED.
EE WAS IN THE COAL UNLOADING AREA. EE WAS USING A SLEDGE HAMMER TO OPEN DOOR LATCH ON COAL CAR. WHEN HE SWUNG THE SLEDGE HAMMER TO HIT THE LATCH, HE MISSED THE LATCH AND THE WEIGHT OF THE HAMM ER TWISTED HIS UPPER BODY.
EMPLOYEE WAS IN QUARRY WORKING ON 1C CONVEYER. HE REMOVED THE MOTOR COUPLING GUARD AND TIED A ROPE AROUND IT AND SAT IT ON TOP OF THE HAND RAIL.WHEN HE WAS RELEASING HIS HAND FROM THE GUARD TO GRAB THE ROPE, THE GUARD SLIPPED AND FELL CATCHING HIS RIGHT HAND BETWEEN THE GUARD AND HAND RAIL. MIDDLE, RING, AND LITTLE FINGER WERE CUT REQUIRING 11 STITCHES, REQUIRING STITCHES.
WHILE WELDING, EE GOT SOMETHING IN HIS EYE. THE EYE WAS FLUSHED AND NOTHING WAS FOUND AND HE RETURNED TO WORK. THE NEXT NIGHT SAYING HE WAS FINE. LATER ON THAT NIGHT HE STARTED EXPERENCING PAI N IN RIGHT EYE AND WAS TAKEN TO ER. EE WAS WEARING FACE SHIELD AND WELDING HOOD AT TIME OF INJURY, BUT THERE WERE HIGH WINDS.
CONTRACTOR EE WAS GRINDING WHEN SOMETHING FLEW INTO HIS EYE AND SCRATCHED HIS RIGHT EYE CORNEA. CONTRACTOR EE WAS WEARING SAFETY GLASSES AND GRINDING SHIELD, BUT WIND WAS BLOWING STRONG.
EE WAS WORKING ON #1 ELECTROSTATIC PRECIPITATOR USING A CUTTING TORCH. EE HAD MOMENTARILY REMOVED THE GLOVE ON HIS LEFT HAND. HE ACCIDENTLY SLIPPED AND THE TORCH FLASHED OVER HIS FINGERS BURNI NG THEM.
