Dozer hit a boulder & tipped over.
While said operator was breaking granite into smaller pieces with a hydraulic breaker, a shard of granite went through the excavator cab safety glass and struck operator in the forehead. He remained conscious and alert. Was brought to local clinic where they sutured the contusion. Doctor said he could return to work next day
EE was welding, slag got on EE's boot, resulting in a burn to his top left foot. Went to the doctor and medication was prescribed.
Backed off an embankment resulting in jolt to neck. Employee has no lost/restricted time but went to the chiropractor for adjustments.
Hit a big rock with dozer. Neck went out of place Did not seek medical treatment until 6/19/09.

EMPLOYEE WALKED INTO AREA WHERE MECHANICS WERE RUNNING A GRINGER, HE TURNED AROUND TO GET SAFETY GLASSES AND GOT A PIECE OF SLAG IN HIS RIGHT EYE.
Employees were lifting electrical box on crusher. Lift chain unhooked over balancing box, fell and struck employee's right lower leg, fracturing it.
EE WAS GRINDING WITHOUT FACE SHIELD. ONLY SAFETY GLASSES WERE WORN. EE HAD PARTICLE OF METAL IN HIS EYE. LEFT EYE.
