Fell backward onto stairs; bumping back & dropping item being carried on knee

Doing routine maintenance, wrench slipped & caused laceration to top of hand.

Running to shut off spokane and fell injuring chest area. Cracked a rib which resulted in one lost day from work.

Backed off an embankment resulting in jolt to neck. Employee has no lost/restricted time but went to the chiropractor for adjustments.

Lifting roller into conveyor, pulled muscle in lower back.

It was cold and frosty on the equipment and ladders. EE was coming down from doing task and slipped on ladder and landed on a step, groin first.
