Employee was stepping off the side of company unit # 14-26, employee's foot got caught resulting in employee falling and injuring his right wrist.

Strained lower back pulling a hose and lifting blocks.

While said operator was breaking granite into smaller pieces with a hydraulic breaker, a shard of granite went through the excavator cab safety glass and struck operator in the forehead. He remained conscious and alert. Was brought to local clinic where they sutured the contusion. Doctor said he could return to work next day

Employee was cutting a piece of rubber with a box cutter and cut his left index finger, cut resulted in employee getting stitches.

Laceration to left index finger, EE rec. 5 stitches.

Setting down a piece of iron, left index finger got between iron and rock which resulted in a laceration to left index finger, needing stitches.

EMPLOYEES WERE PUTTING CHUTE BACK ON. CHUTE SLID PINNING EE'S LEFT HAND CAUSING INJURY.

EE WAS USING A CHAIN TO TIE UP A ROCK BY THE JAW. HOOK SLIPPED & EE HIT CHEST AREA INTO CATWALK RAIL, STRAINING MUSCLE.

EE WAS GRINDING WITHOUT FACE SHIELD. ONLY SAFETY GLASSES WERE WORN. EE HAD PARTICLE OF METAL IN HIS EYE. LEFT EYE.

EMPLOYEE WAS ACCESSING THE LADDER TO BOARD THE LOADER AND SLIPPED ON STEP AND INJURED L KNEE.

EMPLOYEE WAS PUSHING ON THE OUT RIGGER OF CRANE WHEN HIS INDEX FINGER GOT CAUGHT BETWEEN OUT RIGGER AND FENDER.
