Showing all 12During Removal of X-bracing, bolts were removed and a weld failed that was holding a 10' piece of 6x6 angle. Angle came down and fractured employee's arm.
EE was observing the maintenance crew work, EE was standing 15' away with all P.P.E. on when a piece of metal the crew was chipping flew at EE striking EE in the tooth.

2'6"x29' Steel Concrete form clamp system failed, allowing the form to seperate into 2 sections (2'6"x9' and 2'6"x21') as it was being lowered to the ground. 9' section broke away from main piece and dropped to the ground. 21' section remaining suspended by crane dropped to the ground impacting injured.
THE EE WAS HELPING THE WELDER AND EVEN THOUGH HE CLAIMED HE HAD A WELDING HELMET ON HE STILL RECEIVED FLASH BURNS TO HIS EYES.
