Employee was getting loaded by shovel 11. Shovel had electrical fault and loaded bucket dropped into truck box striking box. Employee was seen by mine EMT for back muscle pain and has followed up going to coalition to be evaluated by Dr. Doctor has employee going to physical therapist for muscle spasms. EE has been given a full return to work.

Because of way EE was seated, head moved forward & contacted steering wheel which caused bloody nose. EE contacted supv, told supv EE was rocked by load. EE was asked if they wanted to see EMT but denied & finished shift. Employer was notified 5/10/19 EE would need phys therapy & Rx
Employee was bolting top when bolt bent, mashing right hand against mast head and crushing right ring finger from the nail bed up. Employee was taken to the ER where ee required sutures and was referred for further treatment.
Employee was pull starting a Honda trash pump. Compression built up in engine pulled cord out of employee's hand impacting 4th finger causing fracture.
While operating a roof bolter walking behind it a chain broke and a piece of metal struck employee in the right eye.
Employee was adjusting curtain while roof bolter was tramming into entry. The drill hit gob/rock & shifted striking employee in chest with the T Bar. Employee has broken ribs & collapsed lung.
while bolting top, drill steel hung, bumped rotation, steel flew out & struck left cheek. Employee had stitches & returned to work on the next regular scheduled shift.
Reported 1/30/19 - Proceeded to back up the drill, the rear of the drill dipped sharply down, causing the front of the drill to jump into the air, throwing employee into the control console, woke up the next morning with pain in the right knee
walking behind machine & it slewed left & edge of canopy hit EE in the chest. EE's momentum placed EE close to the canopy. EE was briefly pinned against the rib.
Employee was operating D9T #54 doing slope work on lower banner & backed over a rock on offside jarring neck & back.
EE was installing rib bolt when drill steel came out of chuck. EE reached to position steel & hit the rotation lever instead of the feed lever. EE caught a finger between steel and spinning drill head. The skin at tip of finger is torn off. EE had surgery on finger & they removed the bone at tip of finger to repair skin around end of finger. EE is off work & will have lost time.
Installing roof bolt when EE spent the bolt next to the top & small piece of material went into EE's eye, EE had safety glasses on at time of accident. EE went to eye doctor & a piece of metal was removed from eye. There is no lost time.
Employee states that he was going up a hill on a dozer and went over an axel berm and the dozer slammed down and jerked him whipping his neck.
Employee states that he was doing reclamation work and while coming down over a slope he got on a rock and lost control and began sliding. He states that he let the blade down to slow him and it almost stopped the dozer. When the dozer almost stopped it slung him forward.
Employee was cleaning holes in beams using a drill and the drill got caught up and twisted his right middle finger.
Employee smashed fingers with canopy
While pushing a tractor trailer up a hill the grader kicked out of gear. EE rolled backwards down the hill and hit the ditch and hillside.
EE was working on a bulldozer which required him to remove the hood. While lifting the hood off it flipped up on one end and the chain came loose allowing the hood to fall pinning ee hand.
Helping the mechanic install a new cutting edge on a bulldozer, when the mechanics grinder wheel broke off, striking ee knee.
Employee was leaving dozer cab for a break, slip and fell between tracks and frame hurting left leg/side
while employee was welding, welding helmet was changing dark due to night fall, he changed to safety glasses to finish. A piece of hot slag hit glasses, dripped onto right eyelid, burning lid and eye
-became reportable after the 3/12/14 office visit - The drill ran over a rock slamming it to the ground jarring back.
The alleged accident was from a slip on a slick grader step which he stated his left foot became lodged between step and he landed with his back hitting against the grader machine
turned around in the drill to check the holes he drilled & hurt back.
While operating dozer, a rock struck track slinging his body forward and strained his neck and back
