On December 28, 2025, Miner was working on the demolition and removal of an existing dryer. Miner attempted to cut an additional rigging point while load was suspended. Miner accidentally cut the supporting rigging causing the load to shift and strike Miner in the abdomen/chest area resulting in a broken rib and internal bruising.

EE reports cleaning the bottom floor of the Mogensen building when two palm size rocks fell from the Mogensen screener and hit the EE in the upper back and neck area. EE complains of continuous pain, soreness, and injury.

EE was operating an electric power pack to remove the roll spring bolts from the roll crusher. EE was using a bolt and a ram to press the bigger bolt free. Once the EE relieved the pressure off the ram the bolt fell and struck the employee's right hand and pinky finger. The EE was wearing gloves during the incident.

A miner was holding a section of conveyor belt while another miner used an angle grinder with cut-off wheel to cut screws from a splice. The angle grinder kicked and made contact with the miner holding the conveyor belts left forearm resulting in a laceration. The miner received 5 stitches to treat the injury.

EE was using a hoist to lift the copper bags at #2 mixer. The injured EE tried to reposition the chain using the lifting straps when the hoist stop lifting. When the chain cleared the hoist housing and proceeded to lift the straps tightened up on the EE pinky finger.

EE was attempting to unchoke a Rotex. The EE locked and tagged out the unit but did not let the unit come to a complete stop. EE failed to perform the last step (Try Out) before reaching in and attempt to unclog the unit. The momentum of the machine coming to a stop in its normal resting position made contact with the EE's hand.

A miner was cleaning out a chute and auto sampler when the sampler cycled pinning the employees right arm against the sampler and opening of the chute. The employee was taken to the hospital and diagnosed with a compression injury.

Individual was using a 4.5 inch hand held grinder with wire wheel brush to clean mortar off of a hand trowel. Wire wheel brush caught corner of hand trowel and broke a piece off. Broken piece struck individual in back of hand causing laceration to finger.

Employee reported injury while guzzling 2 days after it happened. on 4/8 The Plant Nurse looked at his eye it was red, irritated, and had a visible abrasion to it. She instructed employee to see eye doctor immediately. Employee was given antibiotic drops to take 4x's a day and was instructed stay out of work environment until 4/9/2009.

While drilling a hole in no. 2 oil tank cover with a milwaukee 1/2 inch drill. The bit caught and the drill spun causing strain on left thumb.

Employee performed unsafe act trying to jam socket on drill by pushing down on floor, socket slipped, exposing ring finger which was lacerated in the process. Employee was taken to emergency room to receive 6 stitches & immediately after returned to work regular duty, lead doing paperwork.

Miner was re-welding cracks on the rotex frames and at some point a piece of the metel got in his eye. He was not aware that this happened until he arrived at home and his eye started to bother him. He went to the ER and they took a small piece of metal out of his eye. He was wearing the proper PPE during the welding.

Employee was drilling a plate for a pump stand in the maintenance fab area and some how he got his right hand with a glove on it tangled in the rotating drill bit. Which twisted his hand and gave him a few small cuts and a dislocated ring finger. Took him to the medical center they gave him first aid reset finger.

An employee was entering the automix's mixing pot. As the employee entered through the hatch, the slide gate closed. When the slide gate closed, it caught the employee's foot injuring it.

Employee felt something in eye while grinding. All proper PPE was used. Employee is not sure when dust or debris got in eye but the pain started while he was grinding. Debris in Eye. Employee flushed eye removed debris and was given perscription eye medication. Employee has no follow-ups with doctor.

Employee reported burn to supervisor on 4-11-06:"a hot piece of material went down my right sleeve when welding or burning. I washed burns and did nto pay attention to the burns; now elbow sensitive to touch, looks like infection in skin over elbow.

AN EMPLOYEE WAS CHECKING OUT THE BOX ON THE ROTEX. THE EMPLOYEE WAS LIFTING THE SKIRT OF THE ROTEX TO EXAMINE IT. THE EMPLOYEE DID NOT ALLOW THE ROTEX TO COME TO A COMPLETE STOP. DUE TO THIS ERROR OF JUDGEMENT, THE EMPLOYEE SMASHED HIS HAND BETWEEN THE BOX AND THE HOPPER WHILE LIFTING THE SKIRT.

While tightening chain with OH crane to lift roll steel he got left middle finger caught in between the chain and the hook causing finger to be crushed/lacerated. Employee was taken off work for one day and was put on restrictions.

The employee was positioning a large steel box into place with the help of another employee that was operating a manual overhead hoist. The steel box slipped from the hoist hook and dropped 4-6 inches smashing the employee's left middle finger between the box and the concrete floor.

EMPLOYEE WAS COMBINING POWDERED CHEMICALS WITH SOLUTIONS IN A MIXING POT TO CREATE A SLURRY. THE MIXER POT AGITATOR EXPELLED SOME OF THE SLURRY WHICH ENTERED A SPACE BETWEEN THE EMPLOYEE'S FACE AND SAFETY GLASSES AND CONTACTING THE RIGHT EYE.

AS HE WAS CHIPPING AWAY WITH A PNEUMATIC CHIPPER TO CLEAR BUILDUP INSIDE THE HOPPER, A LARGE PIECE BROKE LOOSE AND PUSHED HIM BACK. IT HIT THE BACK OF HIS HEAD. HE FELL BACKWARDS AND WAS KNOCKED UNCONSCIOUS. REGAINING CONSCIOUSNESS, HE WAS DISORIENTED. X-RAYS NEGATIVE.

WHILE DISASSEMBLING ROLL CRUSHER DUST SHIELDS WITH IMPACT WRENCH THE OPPOSITE END OF THE DUST SHIELDS BROKE FREE CAUSING THE IMPACT WRENCH TO PUSH FORWARD. HIS LEFT 5TH FINGER WAS MISPLACED ON THE WRENCH AND WAS CAUGHT IN BETWEEN THE WRENCH AND THE DUST SHIELD. HE RECEIVED A FRACTURED FINGER AND RECEIVED SEVEN STITCHES. NO RESTRICTIONS.

HE STATED THAT HE DIDN'T KNOW WHEN HE INCURRED THE FLASH. HE WAS GIVEN PRESCRIPTION EYE DROPS AND WAS GIVEN PAIN MEDICATION AND RESTRICTIONS OF NO WELDING FOR TOW DAYS.

EE WAS DEBURRING A WRENCH AT THE TIME OF THE INCIDENT. ALTHOUGH THE WHEEL WAS GUARDED HE BRUSHED HIS LEFT INDEX FINGER AGAINST IT. HE HAD GLOVES ALTHOUGH HE WAS NOT WEARING THEM AT THE TIME. HE SUSTAINED A 2" LACERATION TO THE ANTERIOR ASPECT OF THE DISTAL PORTION OF FINGER.
