EE WERE PUTTING A PIPELINE BACK TOGETHER, LIFTING THE PIPE AT AN ANGLE. THE HOIST FOOK THAT WAS HOOKED INTO THE CHAIN WELDED ON THE PIPE CAME LOOSE AND STRUCK HIM IN THE LEFT SIDE OF THE FACE.
EE WAS WELDING ON GRAVEL PLANT. HE RAISED HIS SHIELD TO LOOK AT THE WELD AND A PIECE OF SLAG POPPED OFF AND INTO HIS RIGHT EYE.
