Employee was on the trailer, getting ready to set the first load. He stepped back and his foot went in a hole in the trailer. He lost his balance and fell backwards. His body was hanging off the side of the trailer and his foot was still caught in the hole. He has a severely sprained right ankle.

Employee was putting a pipeline back together when the pipe smashed his left knee against the hydraulic ram on the work barge.

Employee was operating overburden belt fed by the backhoe. A large chunk of overburden fell off the top of the belt hitting the employee on the lower left side of the back.

EMPLOYEE WAS CUTTING A 1"" RUBBER HOSE WITH A RAZOR KNIFE. THE KNIFE SLIPPED OFF THE HOSE AND CUT HIS LOWER RIGHT WRIST REQUIRING FOUR STITCHES.

Over-exertion in lifting objects

EMPLOYEE WAS USING A 12 LB SLEDGE HAMMER TO DRIVE OUT A TRACK PIN IN THE MASTER LINKS ON THE TRACKS OF A KOMATSU PC HYDRAULIC EXCAVATOR. WHILE SWINGING THE HAMMER, HE FELT EXCRUCIATING PAIN IN HIS RIGHT STOMACH.

Contact with heat

EE WAS REPAIRING A MUDBELT & BECAME OVERHEATED. HE RESTED, THEN WORKED THE REST OF THE DAY AT A LESS STRENUOUS JOB. HE WORKED THE FOLLOWING WEEK IN A/C EQUIPMENT BUT CONTINUED TO SUFFER HEADAC HE & FATIGUE. HE SAW DOCTOR MONDAY, 7/22 & WENT IN FOR TESTS 7/23. AFTERNOON OF 7/23 HE QUIT FOR ANOTHER JOB.

Absorption of radiations, caustics, toxic and noxious substances

EE WAS CLEANING MUD BELT WHEN HE WAS STUNG BY A WASP ON THE LEFT FOREARM. HE REPORTS THAT HE WOKE UP ON 5-11-02 WITH A KNOT AND PAIN AT THE SIGHT. ON 5-14-02 HE WENT TO THE DOCTOR.

EE WAS WELDING ON GRAVEL PLANT. HE RAISED HIS SHIELD TO LOOK AT THE WELD AND A PIECE OF SLAG POPPED OFF AND INTO HIS RIGHT EYE.
