Employee was assisting in dumping 10" pieces of plastic pipe. The trackhoe was moving a piece of the pipe when the chain came unhooked and the pipe fell on him hitting him on the left side of the head and on his left shoulder.

EMPLOYEE WAS WORKING ON TRACK ADJUSTER FOR HYDRAULIC EXCAVATOR. HE COULD NOT GET THE TRACKS TO MOVE SO HE USED THE BUCKET ARM OF THE TRACKHOE TO MOVE TRACKS. GREASE SHOT OUT OF THE TRACK ADJUSTER AND HIT HIM IN THE EYES.
