A forklift operator made a complete stop on the forklift and then exited the machine. EE walked away from the machine and the forklift began to roll following the EE. EE was at the warehouse racks and the machine hit EE at the rack causing fractures to 5 ribs
Dump truck employee was driving, crossed a berm and rolled over resulting in a fracture to the posterior 12th rib.
A miner was operating a Ditch Witch SK 600 when the equipment suddenly jolted. This action caused the miner's finger to get smashed between a support bracket and the handrest. It resulted in a fingertip avulsion requiring sutures. The miner was released from the hospital the same day and returned to work the next morning.
Employee opened the latch and cover of the rotary vane discharge chute for inspection. While using EE's right hand to clean the chute, EE's hand contacted the vane of the rotary feeder. This resulted in an amputation at the level of the distal interphalangeal joint of the 3rd and 4th digits.
Employee was cleaning up Loadout area with a skid steer. The employee contacted concrete footing with the right edge of the skid steer bucket. The sudden impact propelled the employee forward and the top of their head hit the rearview mirror. Incident resulted in a laceration on their head that required 8 staples to close.
Employee injured right hand and arm after making contact with a moving conveyor return roller.
The IE is a security guard. EE lowered a chain across a roadway, drove forward a few feet with EE's left leg outside the vehicle and the leg jammed into the ground causing EE's left knee to be injured with a break of a bone in the knee.
Employee stated that EE felt pain in EE's neck and back when the loader operator dropped a load of sand into EE's haul truck.
Employee stated they hurt their back when the truck shifted gears. A witness stated the employee was frustrated with having to work and slammed themselves against the back of the truck seat.
the Load out operator had just finished loading a flour truck, moved the loadout spout, tied up the dust collector spout and was on top of the truck closing the back hatch when the truck started to move forward and they lost balance somewhat. The employee regained their balance and was then at the end of their self retracting lanyard and fell to the ground.
Miner injured their right hand and received six sutures to the right thumb when a railcar collided with a platform gangway that was not mechanically secured in place.
EE placed right hand on lid of dust bin as forklift was setting down. EE sustained minor injury. EE was offered medical treatment and declined. On 4/27/19 EE requested medical treatment and was seen by MD. Treating MD could not see issue and released to full duty. On 5/2/19, MD determined diagnosis was incorrect and a fracture was diagnosed.
Employee was in a 126' boom lift attempting to relocate the boom lift unaware the basket was lodged on structure. When basket of boom lift broke free the basket jerked causing employee who was holding on to handrails of basket to smash right hand between basket rails and structure causing employee to smash/pinch right hand and 5th fingertip.
Miner alleges lower back pain from operation of a front end loader. Reported at 9:00 am 11/1/18...no definitive time of a specific incident.
Miner injured upper left arm when operating a haul truck and hitting a bump in the travel way.
Employee was driving an end loader feeding material. Employee backed up loader while grasping the steering wheel with left hand and looking over right shoulder, the loader made contact with a hole in the pad area, jarring the employee in the cab. Employee reported incident 06/14/18. PT prescribed 07/05/18
Employee stated that while operating a loader in the pit, employee backed into a depression and jarred back. Employee requested medical treatment on 12/04/2017 and was given work restrictions.
While using a bobcat to clean sand near a truck scale, an employee injured their rib (closed fracture), when the bobcat struck a protruding concrete footing. The employee was wearing a seatbelt, however employee was forced forward and struck a safety bar in midsection. Medical treatment received 4/18/2016.
Two Dump trucks hauling on dirt road when 1st truck stopped, 2nd truck did not see due to the dust and struck the rear of 1st truck. Driver of 1st truck momentarily lost consciousness resulting in his truck going over small embankment. The injured employee was taken to hospital and received treatment for cuts on head, broken collar bone & possible concussion. Both trucks disabled.
Flange fell off the vac truck while emptying at site.
Haul truck driver was performing normal task when he felt an irritation in his eye. After the use of eye wash station employee stated his eye still felt irritated. Taken to doctor as precautionary measure. Prescription medication prescribed, returned to full duty.
Employee was dumping a portable sand dumpster and his thumb nail on his right hand got mashed between the dumpster and backstop on the forklift.
Employee was driving the company pickup and was crossing a RR track but failed to notice that the sand normally between the rails had been cleaned out leaving a 5" drop. Even though the employee was wearing his seat belt and hard hat he bounced up into the cab roof and injured his neck. Lack of visibility was the main contribution to the incident.
The employee was injured while operating the Volvo A40F Haul truck. He was diagnosed with a shoulder strain. The employee reported the pain was an accumulation over a period of 3 to 4 weeks.
Truck bed on haul truck turned on its side on haul road. Excessive speed. Driver complained of back discomfort and was taken to the medical facility for a check up. Truck was checked out and placed back into service the next day.
Employee was driving haul truck on new uneven haul road. Employee woke up the following morning with back pain.
While using a walk behind forklift, the employee was performing a turn when she felt a pain in her shoulder, initially refused medical treatment. Recently her shoulder was bothering her, so she sought medical treatment and was told she needed physical therapy and there is a possibility of surgery at some point.
Employees was holding a strap on a super sack to be moved to another location. Employee told the forklift driver to move ahead into the back right corner of strap. When the end of the forklift tine caught the tip of employees L- thumb, between the forklift tine and the strap, causing a cut.
Employee was attempting to retrieve a tool from the back of a 4x4 utility cart(Pug). As he reached into the bed of the Pug, the operator started to back up, striking the employees right foot. This resulted in a broken leg. The backup alarm was working on the utility cart. The employee stated that a lack of communication and inattention were to blame. Restrictive duty.
Employee was moving pipe with back hoe equipment, carrying pipe between the knuckle and using the bucket for support. Employee's safety glasses became fogged up due to cold weather. Employee stopped to wipe off glasses when the pipe fell into the water/sand area, which splashed up into employee's eye.
Rail car brakes not working properly.
EMPLOYEE WAS LOADING PIECES OF SCRAP METAL INTO BUCKET OF SKID LOADER WHEN A PIECE OF METAL CAME OVER THE TOP OF THE BUCKET. HE STUCK HIS HAND OUT TO PROTECT HIMSELF.
EMPLOYEE PULLED ON A CHAIN DISLODGING THE PULLEY, CAUSING IT TO SWING THE CHAIN, IT PULLED HIS FINGER BETWEEN THE CHAIN AND CONVEYOR STRUCTURE.
EMPLOYEE WAS OPERATING A 980G LOADER AND THROUGHOUT THE DAY NOTICED A PAIN IN HIS LOWER BACK. MANAGEMENT WAS NOT NOTIFIED UNTIL THE FOLLOWING WEEK.
EE BID QUARRY LARGE LOADER OPERATOR JOB. INTO SECOND WEEK EE TURNED JOB DOWN DUE TO BACK PAIN. THIS JOB IS VERY DIFFICULT BECAUSE OF JOLTING MOTIONS.
ASSISTING LEADMAN IN COUPLING TRAILER TO MAINENENE VAN. AFTER LOWERING HITCH ONTO BALL, EE PUT HIS FINGER BETWEEN BALL & HITCH TO CHECK CONNECTION.LEADMAN SHUT OFF VAN & TOOK HIS FOOT OFF BRA KE. THIS CAUSED A SLIGHT MOVEMENT OF HITCH & BALL. EE'S FINGER WAS PINCHED BETWEEN HITCH & BALL POOR COMMUNICATION & INATTENTION TO DETAIL OFTASK AT HAND CAUSED THIS ACCIDENT
EE SEVERED HIS RIGHT INDEX FINGER AT THE HAND KNUCKLE WHILE ATTEMPTING TO GREASE THE NORTH HEAD PULLY ON THE MAIN ORE BELT. A PIECE OF HEAD PULLEY LAGGING HAD SLID NORTH 3-4" AS ONE OF THE HOL D DOWN CLIPS HAD BROKEN THE END OF THE LAGGING WAS BENT ON 1 CORNER IN A HOOK FASHION AS THE EEATTEMPTED TO INSTALL THE GREASE GUN LINE ON THE BEARING FITTING THE LAGGING SNAGGED HIS COTTON G
EE FOLLOWING NORMAL WORK DUTIES OPERATING EUCLID FELT SHARP PAIN IN HIS SIDE AT END OF WORKDAY. PAIN PERSISTED INTO THE NEXT DAY. FOLLOW UP AT LOCAL EMERGENCY ROOM RESULTED IN DIAGNOSIS OF FRA CTURED RIB.
