A forklift operator made a complete stop on the forklift and then exited the machine. EE walked away from the machine and the forklift began to roll following the EE. EE was at the warehouse racks and the machine hit EE at the rack causing fractures to 5 ribs

A miner was operating a Ditch Witch SK 600 when the equipment suddenly jolted. This action caused the miner's finger to get smashed between a support bracket and the handrest. It resulted in a fingertip avulsion requiring sutures. The miner was released from the hospital the same day and returned to work the next morning.

Employee opened the latch and cover of the rotary vane discharge chute for inspection. While using EE's right hand to clean the chute, EE's hand contacted the vane of the rotary feeder. This resulted in an amputation at the level of the distal interphalangeal joint of the 3rd and 4th digits.

Employee was cleaning up Loadout area with a skid steer. The employee contacted concrete footing with the right edge of the skid steer bucket. The sudden impact propelled the employee forward and the top of their head hit the rearview mirror. Incident resulted in a laceration on their head that required 8 staples to close.

The IE is a security guard. EE lowered a chain across a roadway, drove forward a few feet with EE's left leg outside the vehicle and the leg jammed into the ground causing EE's left knee to be injured with a break of a bone in the knee.

the Load out operator had just finished loading a flour truck, moved the loadout spout, tied up the dust collector spout and was on top of the truck closing the back hatch when the truck started to move forward and they lost balance somewhat. The employee regained their balance and was then at the end of their self retracting lanyard and fell to the ground.

EE placed right hand on lid of dust bin as forklift was setting down. EE sustained minor injury. EE was offered medical treatment and declined. On 4/27/19 EE requested medical treatment and was seen by MD. Treating MD could not see issue and released to full duty. On 5/2/19, MD determined diagnosis was incorrect and a fracture was diagnosed.

Employee was in a 126' boom lift attempting to relocate the boom lift unaware the basket was lodged on structure. When basket of boom lift broke free the basket jerked causing employee who was holding on to handrails of basket to smash right hand between basket rails and structure causing employee to smash/pinch right hand and 5th fingertip.

While using a bobcat to clean sand near a truck scale, an employee injured their rib (closed fracture), when the bobcat struck a protruding concrete footing. The employee was wearing a seatbelt, however employee was forced forward and struck a safety bar in midsection. Medical treatment received 4/18/2016.

Two Dump trucks hauling on dirt road when 1st truck stopped, 2nd truck did not see due to the dust and struck the rear of 1st truck. Driver of 1st truck momentarily lost consciousness resulting in his truck going over small embankment. The injured employee was taken to hospital and received treatment for cuts on head, broken collar bone & possible concussion. Both trucks disabled.

Employee was driving the company pickup and was crossing a RR track but failed to notice that the sand normally between the rails had been cleaned out leaving a 5" drop. Even though the employee was wearing his seat belt and hard hat he bounced up into the cab roof and injured his neck. Lack of visibility was the main contribution to the incident.

While using a walk behind forklift, the employee was performing a turn when she felt a pain in her shoulder, initially refused medical treatment. Recently her shoulder was bothering her, so she sought medical treatment and was told she needed physical therapy and there is a possibility of surgery at some point.

Employees was holding a strap on a super sack to be moved to another location. Employee told the forklift driver to move ahead into the back right corner of strap. When the end of the forklift tine caught the tip of employees L- thumb, between the forklift tine and the strap, causing a cut.

Employee was attempting to retrieve a tool from the back of a 4x4 utility cart(Pug). As he reached into the bed of the Pug, the operator started to back up, striking the employees right foot. This resulted in a broken leg. The backup alarm was working on the utility cart. The employee stated that a lack of communication and inattention were to blame. Restrictive duty.

Employee was moving pipe with back hoe equipment, carrying pipe between the knuckle and using the bucket for support. Employee's safety glasses became fogged up due to cold weather. Employee stopped to wipe off glasses when the pipe fell into the water/sand area, which splashed up into employee's eye.
