A miner was using a stand-on skid steer to perform housekeeping around a transition point of two outdoor conveyors when they struck their hard hat on the end of a pipe. The miner was taken to the emergency room where they were diagnosed with a head and cervical spine injury and prescribed lost time.

On 3/21/24 a maintenance employee reported to the supervisor they hurt their knee on 3/16/24. Injured employee was working to free a stuck forklift. The injured employee was operating a Kubota to free the forklift. The team used a strap to try and pull the forklift free, once the slack was out of the strap the Kubota came to a stop, the employee's left knee contacted the dashboard.

A contractor truck driver was leaving the site after picking up a load when they reached down to pick up a water bottle and veered off of the main road out of the plant down a road bank into a rock berm and overturned the trailer causing a neck strain.

While unchoking a feed chute to #4 conveyor, an employee opened an inspection door/hatch above the conv. before closing the manual feed slide. This allowed the heated rock to flow from the opened hatch on to the floor. below. The heated rock contacted the employees' leg and caused a burn to the skin between the shin and knee.

Employee was hauling waste on the 966 loader with a full bucket and with the bucket up to the stock pile. The right side of the bucket clipped and bent the I beam on the headlap scale. Employee stated: was driving that way instead of around to get more loads out before employee went home. Employee subsequently also complained of neck and back pain on 3/16/2021.

After the employee moved the tripper on 20 conveyor from tank A11 to tank A2, the tripper started rolling back to the east because the #20 conv belt hit the handrail and catwalk crosswalk that was still in place west of tank A1. The contact of the conveyor with the crosswalk forced the tripper to roll back to the east and hit the employee that was on the A row side of the tripper

As the loader operator was backing out of muck pile ee began to articulate the loader to square up with it. In this process the loader bucket came in contact with the back corner (driver side) of box. No damage occurred employee felt pain in neck and back

EE experienced a sudden stop while scooping roofing granules with a skid steer from the rail loading area. The sudden stop caused EE to be restrained by the lap bar. This sudden restraint eventually caused soreness in the rib area.

While driving 813 Haul truck employee experienced low back pain and numbness in legs during normal job duties. Operating conditions normal as any other day. Employee noticed that the seat was at its end of life and hand no more cushion over some parts of the seat. Employee reported injury on 08/15/2013 and went to doctor because pain increased. Employee is now off work.

While retrieving a pallet of chemical, an employee was backing up slowly with a forklift and struck an upright building support that was in his blind spot. The forklift came to an abrupt stop. The employee reports a burning pain across his lower back.

Employee punched out from work. While driving home he left parking lot and was driving on paved quarry road. Almost hit deer and crashed bike. Employee suffered scrapes bruises to head face and arms and was brought to ER by ambulance. He returned to work on July1.

The employee saw her own doctor to investigate a sore shoulder that she thought might be related to sleeping. The doctor suggested that it might be related to work. When the employee heard that information, she remembered driving the secondary waste truck over unpaved roads at Greystone were particularly rough due to frost heaving. Workers comp has said it is work related now.

Miner operating forktruck moving pallet with screen crates. Forks not completely under load and load not tipped back. Pallet fell, causing crate to break. Miner had puncture wound on (L) palm by nail protruding from broken crate. EE was not wearing gloves.

On loaded trip with finish truck, he crossed over train tracks to slow and jarred his back. This happened about 6pm on 6/14/07. He reported it on 6/20/07 when treating it himself didn't work. (ice & aspirin). At the time he didn't think it was anything to worry about, but pain did not go away.

Employee went to the nurse on June 6, 2007 and stated two weeks prior his right knee started to hurt, due to driving the 936 and 988 loaders. He states that the rough drive and having to push hard on the pedal is making his knee hurt. Nurse treated and will have to due follow up.

An employee was operating a Skid Steer. The employee went in to scoop up a pile of fines and ran into something solid. This caused the skid steer to come to a sudden stop. This caused the employee's arms to be driven backwards. This caused a strain on the right shoulder.

Employee was digging into a rock pile with a front end loader, when he tilted the bucket upward, the back end of the loader came off the ground causing the employee to pitch forward. He struck his stomach on the round ball on the steering wheel causing an abdominal contusion.

A Tank Loader was injured when his right arm was pulled into #18 Tripper Conveyor between the head pulley and the cleaning brush. Injuries included accidental avulsion of some of the worker's right forearm muscle tissue. The employee is hospitalized at this time.

EMPLOYEE WAS STANDING IN FRONT OF GYRO PIT, DIRECTING DUMP BERM REMOVAL. TOP LIP HAD BEEN BROKEN FREE AND AS FRONT END LOADER TILTED BUCKET UP TO MOVE IT OUT, IT SLID UP AND CLOCKWISE TOWARD THE EMPLOYEE. HE LEAPED BACK TO AVOID BEING HIT AND DROPPED 13 1/2 FEET DOWN INTO THE CLEARED PIT.

Employee was using a push broom brush (Handle was removed) to remove rock buildup between two troughing idlers on a conveyor. The conveyor was running at the time and the brush was caught between the idler and the conveyor which pulled in the brush and the employees right hand.

EE WAS CLOSING THE DOORS TO THE MAN LIFT ELEVATOR IN C&S AND CAUGHT HIS HAND BETWEEN THE DOORS. HE SUSTAINED A CRUSH TYPE OF INJURY RESULTING IN A SEVERE CONTUSION ONLY. PREVIOUSLY THESE DOO RS HAD BEEN DIFFICULT TO CLOSE. THEY HAD JUST BEEN REPAIRED AND NO LONGER NEEDED THE FORCE THAT WAS USED TO CLOSE THEM.
