The employee was using a vacuum truck performing normal cleanup duties. The employee felt EE had strained EE's back which EE said has occurred several times before. A third party injury care administrator prescribed self care of ice, heat, and ibuprofen. The employee was doing better but on 9/19/2019 the employee elected to go to a chiropractor making it recordable.
Injured employee (IE) was backing a forklift down a concrete forklift ramp. When the forklift got near the bottom a piece of the concrete broke loose and caused a hole approximate 12 inches in diameter. The forklift tire went into the hole and the front end of the forklift quickly dropped approximately four inches. The sudden drop and stop jarred the IE and injured IE's back.
While backing a 775 CAT haul truck to dump material the rear wheels slide sideways into a slight depression and slowly tipped onto its right side. The driver was wearing seatbelt, was assisted out of the cab by a coworker, but bruised inner left thigh during the incident and was place on light duty.
The injured employee was operating a forklift on a gravel drive / parking area to empty a material tube. When returning to EE's area the forklift was struck by a semi-truck traveling at approximately 5 mph. The impact jostled the lift and operator resulting in a cervical and lumbar strain and restricted duty.
Employee was operating a 980 front end loader on a Barge to cleanup the deck of residual limestone. The loader bucket caught the edge of the barge and jarring the loader operator.
Employee was taking a return dust sample from the return dust screw area. He took the sample from the dust relief port, not the sampling port. His 2nd and 3rd fingers on his right hand were amputated.
Employee backed off stock pile.
Victim & co-worker were cleaning a belt line w/ a jackhammer & vacuum truck. Oversized material plugged the 6" vacuum hose, victim grabbed and removed the oversize material with his left hand. Breaking the material free from the hose end, his left arm was sucked into the hose up to his shoulder. Co-worker immediately activated a valve to stop the vacuum freeing the victim.
EE was driving a track mobile when he hit a bump and began to feel discomfort in his back. EE went to chiropractor had his back adjusted and was released with no restrictions and no lost days.
EE was in a man basket when the mast stuck momentarily then released causing a jarring to the employee. EE did not receive medical treatment until 5/10/11 when he was referred to a chiropractor.
As EE was entering loader, EE closed door on his right thumb.
The employee was outside of a Terex Haul Truck when he reached back in the cab and activated the Engine Compartment Suppresion system for the truck. The employee then inhaled the dry chemical extinguishing agent that was released. Employee complained of shortness of breath and was taken for medical attention. Employee was released to return to work on 10/29/10.
Employee was riding in a pickup with a supervisor when they hit a bump in the road, jarring the employees lower back. Employee had continued pain in back and was seen by a doctor on 5/5/2010 and placed on work restrictions.
EE was working on a vac truck when the hose blew around and struck EE on the left arm. This caused swelling and bruising.
EE involved in minor vehicle accident on 9/19/09. Reviewed at time, had no complaint of injury. EE described back spasms on 9/26/09. EE attended by doctor on 9/29/09 (days off) event became reportable.
The employee was attempting to empty out a dust bin by having an electrician reverse the screw direction. The employee was standing 10' from the screw when the screw became compacted with dust causing some to spray in the direction of the employee striking him in his eyes. The dust spray knocked the employee's safety glasses off. Employee was given a prescription.
EE was using the Vac truck to vacuum out the clinker drag pit. His right arm felt itchy. His arm was partially covered in clinker dust. Upon inspection, he noticed that the upper bicep & lower tricep were red & tender. He washed the arm - 1st Aid. DR determined that he had a 2nd degree burn from an interaction of high alkaline dust with his perspiration.
Employee observing a concrete pour. Pump truck belonging to company handling sanitation of work sites portable toilets began backing up. The employee was behind the truck. Employee was bumped in lower back by equipment on back of pump truck and knocked forward; the employee caught himself with his left arm. Incident occurred on 10/2, the employee did not see a dr. until 10/6.
After the driver had closed the hatch on his truck, he pulled the carbis platform up. The platform did not latch and began to go back down. As it did so the driver tried to catch it and reached across and grabbed a stair. As he pulled it up, his palm was pinched between the stair and the handle, causing the driver to need ten stitches.
Employee was welding in a spider (basket) when slack in cable stopped and dropped spider 2 feet abruptly, jerking employee against the hand rail striking left lower rib.
The EE was operating a manlift to access through the window to weld new roofing panels in place. The manlift was positioned under the roof & used to elevate the worker's head & shoulders between the open roof support beams. The employee's right hand rested upon the safety cage bar over the controls as he ascended and the middle finger pinched between the cage bar & a support beam.
While flying a piece of grating into place with a 2250 crane, the grating hit a piece of existing steel. This contact caused a toe board which was welded onto the grating to be knocked off. The toe board fell approximately 20' to the next level below and struck the employee above his right eye.
Employee was using a plant made "air lance" to clean debris off a guarded tail pulley. He had finished and was disconnecting the air lance when his head hit a low overhead beam and these two motions combined pushed the air lance into the tail pulley which then jerked it from his hands.
The employee involved in the incident was attempting to make a left turn using a walk behind loader when the controls became stuck in the left turn position which caused the machine to make a full circle and strike the employee on the left knee. Employee taken to seek medical attention and was returned to work on restricted duty after being diagnosed with a left knee bruise.
EE was backing a CAT 777 haul truck and hit the 992 loader boom with the left top corner of the bed. This caused the haul truck to stop suddenly, causing the EE discomfort in his neck. The EE did not receive medical attention until 5/25/07.
Forklift operator backed into pedestrain pinning him between the forklift and a bench. Forklift operator did not look before backing and backed up at a high rate of speed. No equipment was damaged.
The injured employee was attempting to free a stationary roller that a belt conveyor moved tires onto. As the employee was moving the roller the belt came on and caught his hand between the head pulley and the roller. He received bruising on his hand and was taken to Little River Memorial Hospital where he was treated and released back to restricted duty.
Employee was removing material jammed along a conveyor. He didn't lock out the conveyor and his hand slipped into a nip point between a bent roller and the machine frame causing the loss of his left thumbnail along with a fracture in this thumb tip.
Employee was struck by a bar during efforts to free a piece of ramp section from the forks of a lift truck. The bar was used to anchor the section in place while the forklift backed up. The bar was pulled toward the employee during this process striking him.
EMPLOYEE WAS OPERATING THE 988 CAT LOADER. HE WAS FILLING THE BUCKET WITH LIMESTONE FROM THE QUARRY AND WHILE DIGGING INTO THE FACE OF THE WALL, CONTACTED SOLID ROCK. THIS JARRED HIS BACK, CAUSING DISCOMFORT BETWEEN HIS SHOULDER BLADES. HE WENT TO SEE THE CHIROPRACTOR ON 7/20/05.
EMPLOYEE OPERATING FORKLIFT OVER WET ROADWAY. EE DROVE THROUGH STANDING WATER & LEFT FRONT TIRE FELL INTO AN OPEN MANHOLE. UNKNOWN AS TO WHAT HAPPENED TO COVER. BELIEVED TO HAVE BEEN PUSHED UP BY WATER & REMOVED UNKNOWINGLY BY A LOADER DURING CLEAN-UP.
Employee was operating a Bobcat Loader, drove over bump in road, ribs came in contact with safety bar causing bruising to ribs. Reason for reporting: Restricted duty. Employee reported the injury on 1/3/2005.
RR car with defective brakes, broke from standing position, employee jumped on car to set brakes when locomotive connected, straining right shoulder.

Employee speeding in golf cart, could not stop, avoided oil storage rack and ran into building. Caught foot between building and golf cart, small bone fracture.
EE WAS DRIVING THE FRONT-END LOADER DOWN FROM THE QUARRY AND HIT/DROVE THRU A DIP IN THE ROAD. THE AIR SUSPENSION SEAT OF THE LOADER WAS NOT OPERATIONAL.
EE WAS MOVING RAILCAR OPENER TO OPEN THE DOOR AND STRAINED RIGHT SHOULDER WHILE PUTTING THE OPERNER ON THE DOOR. REASON FOR REPORTING: RESTRICTED DUTY.
EMPLOYEE WAS USING AN AIRLANCE TO REMOVE BUILDUP FROM A CONVEYOR TAILPULLEY. HE INSERTED THE AIR LANCE THROUGH THE PULLEY GUARD. IT WAS CAUGHT IN PULLEY PULLING HAND AGAINST THE GUARD.
EE FAILED TO PROPERLY SECURE THE RAMP TO THE TRAILER. A PARTIAL SEPARATION FROM THE TRAILER AND RAMP OCCURED. FORKLIFT "RODE" RAMP DOWN APPROX. 4' TO THE GROUND. FORKLIFT LANDED ON TIRES. EE WAS WEARING SEATBELT. EE WAS BRUISED, BUT AFTER EXAM. AT LOCAL CLINIC, WAS GIVEN ONE DAY OF RESTRICTED DUTY. HE RETURNED TO FULL DUTY ON HISNEXT SCHEDULED SHIFT.
EMPLOYEE WAS SITTING IN HIS TRUCK WHILE BEING LOADED. THE LOADER OPERATOR DROPPED A LARGE ROCK IN THE TRUCK WHICH JARRED THE DRIVER.
EMPLOYEE WAS RELATCHING A DUMPSTER AND SMAHED HIS FINGER BETWEEN THE DUMPSTER AND THE MAST OF A FORKLIFT.
THE EMPLOYEE WAS CLEANING OUT A CHUTE TO RAW STORAGE BUILDING. THIS CHUTE WAS PLUGGED AT THE TRANSFER TOWER THAT DIVERTS LIMESTONE FROM THE BUILDING TO THE LIMESTONE TANK. THE FLOP GATE FLOPPE D HITTING THE RIVOT BUSTER AND PINNED HIS HAND TO THE WALL OF THE CHUTE BREAKING THE BONE IN HIS LEFT HAND FIFTH FINGER.
EMPLOYEE WAS HAULING BRICK ON FORKLIFT FROM BRICK WAREHOUSE TO KILN COOLER WHEN HE FELT BURNING IN HIS NECK.
EMPLOYEE WAS OPERATING WATER TANK TRUCK; SHE BACKED IT UP NEXT TO A SHALLOW EXCAVATION WITH THE REAR RIGHT WHEEL VERY CLOSE TO THE EDGE OF THE EXCAVATION. WHEN SHE PUSHED IN CLUTCH TO PUT IN GEAR TO PULL TRUCK FORWARD, THE TRUCK ROLLED FURTHER BACKWARDS INTO EXCAVATION. THIS CAUSED THE TRUCK TO ROLL OVER ONTO THE PASSENGER SIDE OF THE TRUCK.
