EE was coming down from the loader when EE thought EE was on the last step of the ladder and fell on EE's right side injuring EE's right wrist. Weather conditions were very windy that day.

The IE was walking down a flight of steps. On the last one they stepped awkwardly causing them to roll ankle. The X-ray came back negative and the employee was diagnosed with a sprained ankle. A follow up appointment on 2/13 resulted in restrictions.

EE was walking down the stairs from the K3/K4 burner floor towards the K4 coal mill while carrying samples. EE stepped down from the last step in the stairs, rolled EE's ankle and fell to the ground.

Employee was walking along the combing on a barge that was brought in and rolled EE's ankle. At the time there was just some swelling and no further treatment. Employee stated in March EE was still feeling pain periodically. An MRI was ordered and referral to an Orthopedic. The Orthopedic surgeon provided a prescription for Physical Therapy on 4/6/2026 to strengthen ankle.

EE WAS TRANFERRING FROM THE GANGWAY TO TRAILER WHEN LEFT HEEL OF BOOT CAUGHT ON THE DOME LID CAUSING THE EE TO STUMBLE. THIS RESULTED IN A STRAIN TO THE LEFT KNEE WHILE REGAINING THEIR BALANCE.

Employee was connecting a hose to the top of a pneumatic railcar prior to loading. While moving the vent hose employee lost balance and fell forward. Employee reached out to catch themselves with a hand and felt a pain in shoulder. Pain did not subside over time and was diagnosed via MRI that was confirmed by an orthopedic doctor and was given a cortisone injection on 3/16/23.

Quarry Haul Truck driver missed last step and fell to ground. Right leg was caught in last step on the ladder and hurt right ankle. EE was taken to the doctor and they were diagnose with a strain of right Achilles tendon and Contusion of right foot. EE was given an immobilizing device.

Employee was coming down the stairs (jumped) at the dryer platform and when EE reached ground level, EE's ankle rolled and EE fell to the ground, injuring EE's ankle and EE's elbow. Employee was terminated for cause on 07/22/2022 (unrelated to the injury).

An operator was exiting the railcar mover via the fixed ladder. When stepping from the bottom step to the ground roughly 12 inches below, the employee felt a pop in knee. Ground conditions were favorable with no mud or loose material.

Employee was removing the guard on a conveyor belt while standing on a ladder. Fell off of ladder about 4 feet and landed on right side. MRI review on 12/2 revealed torn rotator cuff and employee was placed on restricted duty.

EE had walked just outside the hydrate building North of blend loadout when EE collapsed falling to the ground. Medical opinion is that this occurred due to EE becoming overheated. However there is nothing to confirm the diagnosis. EE had a laceration requiring 5 sutures to close just over the left temple.

Operator began to maneuver drill from the low-boy. The spotter signaled operator to stop, operator engaged the parking brake, unbuckled seat belt, stood up to access the situation. At this point the cribbing rolled, shifting the front tire off the trailer. At this same time the operator exited the cab and sustained an injury to heel.

The employee started work at 7:00 PM on May 31 and was walking back to change house after the shift ended at 7:00 AM on Jun 1. EE stepped on a RR track, then tripped. EE's foot caught on the track and EE's hand broke the fall. EE's finger caught on a bent spike. EE was wearing all of the PPE except gloves. EE fractured right ring finger and lost a fingernail.

Employee was coming down the stairs of DCL#5 at the 75 Loadout. As EE stepped down on to the asphalt off the stairs EE's left foot gave out and rolled as EE planted it. The ankle swelled up immediately. The employee sought medical attention the following day and received restrictions to not be on feet for more than 4 hours at a time.

Roller Mill Operator twisted right ankle while operating the bagging station after noticing a torn bag that was traveling up the bag flattener conveyor belt. The Operator exited the bagging station access platform at an angle and when stepping down, missed the second step and twisted right ankle when contacting the floor.

The employee was climbing a ladder. Right foot slipped on the fifth rung of the ladder. Employee did have three points of contact and was not holding anything while climbing. Employee fell and sprained right knee.

Miner was descending from a ladder, lost footing and used right arm to hold the ladder to prevent a fall. Miner sustained a sprain of the right shoulder rotator cuff. Physician put the miner on restricted work activities, and was released back to work immediately.

Employee (EE) was walking past an open pallet of empty bulk bags when EE tripped on a length of cordage that was attached to the pallet at one end and loose at the other. The EE had one foot placed on the loose end and the other caught under the cordage causing EE to trip. When EE fell EE impacted the ground with EE's right shoulder.

Employee was walking up the walkway adjacent to the coal belt, tripped on something and fell onto the walkway. Stated they did not know what they tripped on, as there were no hazards present and the walkway was clear. EE braced the fall with right hand, but left hand contacted the grating, causing swelling to left hand and a fractured left pinky finger.

EE had to clear the chute at the head section of BC 463 it was clogged with wet coal and coke. After clearing the chute, EE was coming down the ladder and stepped off the last rung onto the small concrete. On EE's next step EE missed the concrete landing and fell 13" to the concrete level below and hit left shoulder. It was raining very hard at the time of the accident.

Employee connected the hook of the SRL to a positioning D-Ring on EE's harness the hook was not properly seated on the D-Ring, but rather the D-Ring was pinched between the tip of the hook and the hook gate (not seated in the hook throat).

Employee fell from a vertical ladder at the #2 coal mill on the Cooler floor, and struck head, causing a concussion. Employee does not remember what happened and there were no witnesses.
