Operator was entering an excavator when EE's foot slipped from the step and caused EE to fall backward. EE attempted to ease EE's fall by landed on EE's left arm which caused a fracture.
Miner was climbing onto an excavator and slipped and fell to the ground injuring right hip and lower back. The ground and track of the excavator was wet from an overnight rainfall.
The haul truck was stuck on the dump and the operator exited the cab and was climbing down the ladder and twisted an ankle when they reached the ground breaking the left ankle
A rail car Loader Operator twisted an ankle when the Operator slipped and fell on top of the rail car the Operator was walking on that was wet causing a hairline fracture of the right heel bone.
Employee walked out of building after light rain and foot slipped on concrete rampway exit causing a turn to grab right side handrail preventing fall. The right knee and right shoulder were stiff and sore the next day. Employee provided first aid and diagnostic X-ray/MRI negative on knee but provided cortisone shot on 12/19/19 follow-up.
Electrician rolled right ankle on uneven ground in grassy area while performing routine ground checks on electrical substation. No job restrictions nor days away from work lost to date; however, follow up for pain identified bone lesion and determined as work related with medical treatment recommended.
Employee was exiting dozer, feet slipped out from under employee and fell on tracks. Employee went to ER complaining of back pain, x-rays are negative but employee is now scheduled for an MRI and currently waiting for MRI.
Employee tripped and fell on same level. Reported bruising to right rib cage area on back. Received first aid treatment. Several days later seen by physician. X-rays of area were negative. Returned to normal duty that day. No lost work days, No restricted work days. Prescription muscle relaxer was prescribed to assist with pain.
Employee was on site at BASF Mid Ga facility when EE seemed to pass out and hit the ground. EE was not performing any task at the time. EE received stitches at the hospital. EE was never in a life threating issue.
The Front End Loader Operator on the night shift was leaving the Clay Shed work area to clock out at 7 AM. The Operator left the Clay Shed walking down the inclined driveway past the west side of Fluid Bed. The Operator fell to the ground injuring the left ankle after stepping on a patch of frozen clay.
Employee was walking on flat, dry, clean surface at bagging operation when they tripped over their own feet and fell on same level. Operator extended both arms while falling in an attempt to stop fall. The right hand struck a metal frame causing a laceration that required 7 stitches. Operator states they were wearing gloves at the time of the fall. Restricted duty, no lost days.
The employee was working on the catwalk on top of a silo. He states that he caught his foot causing him to fall forward and hit his knee on the mid-rail of the catwalk. No one on the crew witnessed or heard anything.
The employee was preparing to move a loaded bulk rail car to the track scale and slipped causing his knee to bend under himself as he fell. The employee first reported this injury on 10/1/2015 and was treated with first aid measures but sent for a MRI when soreness continued. MRI diagnosed on 10/15/2015 with a complex right knee meniscal tear that will require surgery to repair.
Employee was on a callout working with contractors to repair process water line feeding the mill. Employee dismounted a front end loader and felt his knee pop when he stepped down.
Employee was walking to his vehicle in the parking lot around 7:00 PM when he stumbled and fell. The employee landed face down on the asphalt parking lot hurting his knee in the process. He was placed on restrictive duty as per doctor's orders.
Mill process operator received a laceration to his right lower leg when one of the concrete support corners under a U-drain cover collapsed. The operator's leg came in contact with the U-drain wall causing the laceration that required several sutures to close.
He was setting up a welder getting ready to work on a bag house. He was working on laying out welding leads getting them up to the bag house from the welder. He went to walk down a small decline near the work area and slipped, fell on left side landing on elbow on concrete. The area was wet and covered in mud and product. It had been raining the night before and that morning.
Mechanic was mounting a Cat 349 Excavator to repair a hydraulic leak behind the boom. Mechanic's right foot slipped and caused him to twist his left leg. Mechanic experience pain in his left knee which result has resulted as a sprain knee.
While walking up an embankment, employee slipped and fell backwards, hitting himself in the face with surveying equipment - causing 8 stitches to repair a 3 cm gash in lower lip/chin.
On 5/7/12, at 11:55 PM., packaging employee was walking to his work station after arriving for his shift. The employee was carrying a backpack with his personal belongings. As the employee started to exit a building, he slipped and in the process caught the backpack on the exterior of the building and the backpack pulled the employee backwards injuring his right shoulder.
On 4/18/12 at 10:30 A.M. a Toddville packaging employee was injured from a slip/fall as he exited the packaging building. As he descended the stairs, the employee slipped and fell twisting his right knee landing on his right shoulder. The employee was sent to a physician where he was diagnosed with right shoulder and left knee sprains. The employee was put on work restrictions.
A Mechanic suffered a sprained right knee after slipping and falling. While performing an equipment inspection, the mechanic stepped over a 12" high concrete retaining wall. The area behind the retaining wall was very slippery due to heavy rain combined with kaolin dust. As the mechanic stepped over the retaining wall, he lost his footing and slipped resulting in a fall.
Employee slipped and fell hitting is hip, shoulder and head on concrete floor. The employee was seen by doctor the next day and recieved first aid care. The employee continued to do full job duties but had gradual worsening of shoulder pain. An MRI was conducted and revealed a small rotator cuff tear on 9/9/30 and the employee was givne prescription medication.
Employee tripped over a raised concrete area, lost his balance, reached out to break his fall, and fell on his hand fracturing the left index finger. EE also sustained a hematoma to distal end of same index finger.
employee was on a temporary cross tie platform, laid across the tracks that was not secure, to fill a truck. It rolled causing the employee to fall on his back and elbow.
EE was assigned to assist BASF maintenance in rebuilding pumps. The ee was carrying part of the pump housing, lost traction and slipped down into a seated position; the pump housing fell across the ee left leg causing a laceration that requited sutures. EE returned to light duty work the next day.
EE was tightening a bolt from the 2nd step of a ladder. His wrench slipped off bolt causing him to lose his balance and fall. He landed on his right hip and cracked his right pelvis.
Employee was descending stairway carrying 4 empty 5 gal water bottles. He was not using the handrails provided. He missed the last step and fell forward onto both knees. He was eventually diagnosed having a "patellar dislocation quad tendon rupture" requiring surgery. Diagnosis did not occur until July 24, 2009. Surgery scheduled for July 28, 2009.
While performing rounds, an operator went to climb two steps. The operator stepped on the first step and felt his Achilles tendon pop. He was transported to the emergency room, where his was given medication, crutches and the ankle was immobilized. The operator injured this same tendon 3 years ago in a non-work related event.
Employee was walking down the stairs to control panel at belt filter when his foot slipped and he twisted his left knee
Employee was spraying weed killer around the sides of the shower house. The employee stepped from the parking lot to the ground and twisted his left ankle on some uneven concrete. There is a 16-inch drop at the edge on the parking lot to the ground.
The employee suffered a hairline fracture to his left foot while descending a three foot fixed ladder. He was examined by an orthopedic specialist where an x-ray revealed the fracture. The Operator returned to work with a removable cast and is scheduled for a follow-up appointment in three weeks. This incident is currently classified as a Recordable Injury- RDC.
Employee exited a control room to a platform and while descending steps caught the back of his boot heel on the previous step and fell backwards injuring his left knee.
Original incident Knee hurting on 5/08 by truck driver. Second 12/02 complained about a knee hurting. Dr.Visit and examination the Dr. found that the employee has a pulled/strained hamstring and the Dr. referred him to have physical therapy.
I-Tech was helping electricians pull new wire in MCC 1. The rope that was attached to the wire came loose and the I-Tech fell to the floor and felt a sharp pain in his right knee.
While lifting a screen deck into place employee stepped on a bolt, slipped slightly (did not fall)and felt a pain in his lower back area. Employee has a sore muscle in his lower left back.
Employee was walking down a slipped concrete surface to pick up a safe work permit that the wind had blown down. He slipped in some mud on the concrete surface. He turned to catch himself, his hard hat fell off and he fell on the hard hat liner and cut his forehead above his left eye and 1" long. 5 stitches.
A Mechanic was lifting a pipe from the ground floor to intermediate level of Building. As the lift began, he began to move on the ground level to access stairs to the intermediate level. He tripped over a 3" drain pipe running at floor level and fell, striking his right knee. He was sent to an orthopedic specialist who diagnosed a fractured patella.
EE was on a ladder adjusting the water flow to the dry scrubber, this valve is not within arms reach. On the 3rd step, EE fell backwards and her right leg wedged between the thrid and foeth steps causing her to receive a open compound fracture.
Employee was pulling a bottle cart when the wheels on the cart got stuck between two motor bases. As he pulled the cart, the cart came loose causing him to lose his balance and fall backwards. As he fell, he grabbed onto a structure to catch himself. When he did this, he felt a pain in his left bicep.
EE reported to work during his off shift to assist another planner troubleshoot functionality of a bagging machine. He reported to work not feeling well and informed coworker of the same. EE passed out, fell to the floor and struck the back of his head on the concrete flooring and sustained a laceration.
EE sustained laceration to head due to contact with grating as a result of a fall of unknown cause, which was recently deemed by treating MD to be the result of EE having a "TIA."
Employee was stepping down & stepped on a motor base. This caused him to stumble and wrench or strain his lower back. When he mis-stepped, he lost his balance. Felt pain in lower back (left side).
Platform and steps had a thin layer of ice resulting from freeze protection measures associated with area safety showers. He was starting the loading station for super sacks and saw that the s teps were icy but did not take any action on the issue. As he was stepping down his foot slipped off the bottom step. He landed on his back on the floor.
Employee was working in a tank, he turned and began to slip on some sludge in the tank. Employee turned to catch himself but his feet stuck causing him to fall and twist breaking his left leg.
A mechanic was lying down inside the 1st stage blunger tank at 1-C Cowles attempting to remove the bottom blade. After an unsuccessful attempt at removing the blade, the mechanic lost his balance causing his left hand to come intact with the blade. He sustained a laceration across the palm of his left hand.
Employee was picking up tools/materials to relocate to another area when he stepped on a piece of tubing and fell striking his (L) ear on I-beam resulting in a laceration.
The employee was returning to his golf cart from the #4 Bulk Loading area when he slipped and fell. Apparently, in an attempt to brace or recover from the slip, the employee broke his right ankle. The area where the fall occurred is an incline surface. There was wet kaolin in the area where the fall took place.
Job was rained out. Employee was cleaning up site to leave when he stepped on 3/4" conduit and fell. His (R) elbow struck concrete foundation jamming his (R) shoulder. Employee was taken off work by dr. on 8/7/06 and has not returned.
