EE working in cab D11R. EE exited cab to dismount machine to ground level get tool from truck. maintained 3 points contact while dismounting. track was unhooked, laying on roller frame in almost flat state. weather conditions, rain and mud, EE foot slipped when EE stepped on push arm. foot slipped, knee hit push arm bracket where the tilt cylinder pins up cause knee laceration

Employee was climbing down from hauler and caught his foot between the last step and bumper, then fell, twisting his ankle and knee.
Employee was climbing down from a hauler. When he stepped onto the ground his foot landed on a can of oil, causing him to twist his knee. Did not seek medical until 10-21-08. Continued to work with no missed time until 4-9-09. He had surgery on his right knee.

EE WAS CHECKING OIL LEVELS ON A 994 CAT LOADER. HE WAS COMING BACK DOWN THE STEPS, HE SLIPPED, TWIST6ING HIS LEFT KNE (LEG).

EMPLOYEE WAS TEARING DOWN THE AUGER FOR MOVE. HE STEPPED DOWN ABOUT 12-14", SLIPPED AND TURNED HIS LEFT ANKLE.
CLIMBED UP ON #1 DRYER TO TURN 1" WATER VALUE ON. WHEN COMING DOWN STOPPED ON WATER HOSE ON FLOOR CAUSING FOOT CREEL.

EMPLOYEE WAS DISMOUNTING HIS DRILL, WHEN HIS FOOT SLIPPED & HE HIT HIS RIGHT KNEE ON THE STEPOF THE LADDER; HE SAID HE WAS OK; LATER HIS KNEEBEGAN TO SWELL, HE WENT TO DR. THE NEXT DAY; 1ST LOST TIME FOR THIS ACCIDENT WAS ON 6/6/2000.
