Employee was placing filter fabric over drain, climbing layers of rock. While climbing, employee rested hand on rock and it moved. Employee fell with rock striking right lower leg and foot. ER exam showed fractured fibula 3in. above ankle.

Showing 50 of 79This former employee was terminated at 4:20 PM on this date. As they were leaving they asked another employee who was unaware of the termination for a radio. At approx 4:40PM they used the radio requesting an ambulance, alleging that they had fallen down the stairs, hurting their back, leg, arm & ankle. Upon examination by a doctor, no injury was found about their body.
Operator dismounting a motor grader, missed the step & tried to catch self with left hand & ended up twisting shoulder & heard a popping noise. Employee was diagnosed with a ruptured bicep tendon & rotator cuff tear on 10/05/18. Employee has not missed any scheduled shifts & has scheduled surgery for January 2019. EE had surgery 1/21/19 & will miss upcoming scheduled shifts.
Employee was welding on truck box 12.516. While employee was repositioning self to continue welding, employee slipped and fell. Employee put right hand down to break fall, landing on hand causing small finger to dislocate. Employee was wearing proper hand protection. Employee went to ER and had finger relocated.
EE tripped on loose rock on Mine Floor & cut upper left leg. EE received stitches.

Employee slipped on the ice walking to a crew van causing pain in EE's right knee. The employee had a non work related surgery on this knee in Nov 2017, an MRI was performed on 3/6/18 which resulted in knee surgery on 3/20/18 with a diagnosis of a recurrent symptomatic medial meniscus tear. The first day the employee missed work was on 3/19/18
Warehouse employee was boarding a fork lift, employee's shoes had some mud on the bottom, employee slipped and bumped right knee. Initially it appeared the knee was just bruised. On 2/27 the doctored order an MRI because knee continued to be sore, the MRI determined a torn meniscus, the doctor ordered physical therapy. The first physical therapy visit was on 3/13/18.
Exiting rock truck, caught back of boot on 2nd step, lost balance & fell forward hitting head on fire ext. bracket.
EE was helping take ATRS pads off on 1 side the pad on other side turned over & hit self in face & fell against rib. EE had broken teeth & palate.

EE followed up with doctor a week later & was taken off work on 08/08/2017 until evaluated by specialist. EE was diagnosed with a ligament tear in left wrist.
EE squatted down when raised up & turned felt pain in lower back, went on to section started hanging curtain. EE said pain got worse, went to local ER. EE stated the dr. said it was muscle spasm. EE went to family dr. next day, EE is off work do not have a return to work date.
Employee was on EE's grease truck, pumping grease in barrels. EE had 3 point contact but lost EE's grip with right hand, causing EE to fall from 4th step, landing on frozen ground. EE's right shoulder was injured, with bruising & swelling. Doctor has EE off work.
Got out of shot loader to check the road condition, slipped & fell on ice, hurt left arm
EE was tightening wheel lugs on shuttle car, bar slipped putting EE's weight on right ankle, EE went down in pain.
Employee was pulling cable into shuttle car, cable was tied off with tie wire. Tie wire broke, employee fell backwards catching self with left arm, causing pain in left shoulder. Employee did not starting missing work until 08/23/2016.

Employee was out on the boom of the loader cleaning the windshield. He fell when he was crossing back over the handrail onto the catwalk. He fell from the catwalk to the ground.
Employee slipped and fell while on main frame of HWM causing him to hit his face on the vertical swing jack. Result of injury was a nasal fracture and employee returned to work on 05/28/15 to full duty with no further follow-up appointments necessary.

While stepping down from the dozer, ee stepped in a hole twisting his ankle.
The EE stated that he was stepping across a 4" pipe on the floor. He stepped on a mat used for a jog station. The mat had mud on the surface, whick caused the EE's feet to move out from under him, resulting in a fall.
