Managment took him back to the doctor three times over the next six weeks. A speclist prescribed an MRI. On 03/07/2007, diagnosed the injury as a fracture and prescribed a leg brace. At this time it became a reportable injury.
Contractor suffered a broken left leg and right heel when he inadvertently stepped back off mill pier while installation of raw mill bearing housing was underway. Employee fell approximately 15'5". Employee had removed his fall protection to gather parts on the ground below and had failed to put it back on.

As the employee was getting ready to step on the crusher he was working on, he stepped on a piece of angle iron that was tack welded on the liner. The angle liner broke off causing the employee to fall about three feet backwards onto the head of the crusher.

A customer truck driver pulled his truck onto the weighout scale. He was getting out of the truck to get his scale ticket when he slipped and fell to the ground. He was later diagnosed with fractured ribs. The injured customer driver was employed by J&S, Inc.

GOT OUT OF CAB OF WATER TRUCK TO MAKE SURE IT WAS LINED UP. STEPPED ON LEFT SIDE OF CAB AND LOST HIS FOOTING, FELL APPROXIMATELY 72" TO THE GROUND.

EMPLOYEE FELL BACKWARDS INJURING RIGHT ARM, WRIST, NECK AND BACK. HE DID NOT REPORT TO SUPERVISOR UNTIL 11-8-05. INITIALLY TREATED AS 1ST AID. ON 2ND VISIT TO DOCTORS 11-9-05 CHANGED TO REPORTABLE DUE TO MEDICATION PRESCRIBED.
The employee was repairing a hole in Chute C-9 while using an extendable ladder. The legs on the ladder he was working on slipped out causing the employee to fall. He struck his leg on the tailguard.
