Employee struck right elbow on hand rail while exiting cab of haul truck.

While welding grizzly bars on the dredge the employee lost his footing falling into a sitting position and striking his left elbow on one of the bars.

While trying to pry a screw out of a pump with a 5' bar, the bar slipped off the fulcrum point causing a strain to the right shoulder.

The employee and co-worker were in the process of replacing a grease barrel on the Hitachi shovel. While moving the full barrel with the co-worker, the injured employee felt a sharp pain in the abdominal area.

EMPLOYEE WAS PICKING UP A PIECE OF METAL (12" X 36") TO LAY ON A STACK OF METAL. HE LOST HIS GRIP ON THE PIECE OF METAL AND IT FELL ON HIS FOOT BEHIND THE STEEL TOE AREA OF HIS SAFETY BOOT.

Employee was cutting belting with utility knife, when it slipped, cutting his arm.

While operating a dozer the employee felt a sharp pain in his right shoulder and shoulder blade. Initially this was first aid only. Employee terminated 11/06/07 due to work force reduction prior to being released to return to work full duty (right shoulder surgery performed 12/13/07).

The employee was assigned the task of greasing rollers in Building 7. As the employee was walking he twisted his knee.

Employee was walking from the bathroom to the lunchroom when he dropped a battery. As he went to retrieve the battery, which had rolled under a bin, a raccoon reached out and bit the employee.

The employee was tasked with performing the monthly fire extinguisher inspection. The employee was in a cab of a front end loader and dropped the extinguisher on the cab floor. As he retrieved the extinguisher he activated it and the dry chemical hit the cab floor and sprayed him in the face.

While overhauling Unit 7FKI a pipe slipped and fell on employee.

The injured was decending the Cherry Picker and when he reached ground level he felt a pain in his right ankle.

While walking down the stairs with an armful of obsolete stock the employee slipped and fell down the stairs.

Employee cut his right knee when walking past some equipment with a piece of metal sticking out of it. His knee hit the metal and the laceration required eight stitches. He is unable to squat or kneel.

WAS PULLING HOSE ON FUEL TRUCK, WHEN HE SLAMMED HIS ELBOW INTO THE STEEL TRACK OF THE SHOVEL (HITACHI).

CONTRACT LABOR, NOT AN EMPLOYEE AT APPROXIMATELY 9:30 PM WAS USING A COMEALONG TO PUT A HYDRAULIC MOTOR ON A FEEDER, WHEN THE COMEALONG SLIPPED AND THE HOOK AND HANDLE SMASHED AND CUT HIS RIGHT INDEX FINGER. ALSO SMASHED THE TOP OF HIS RIGHT HAND, CLOSE TO HIS INDEX FINGER.

While working at the Chip Plant, the employee was stung by a bee, on the chin.

Employee cut the 4th finger on an AC fan. Wound required sutures and this is the reason for reporting injury.

While using a torch to cut off the swing die bolts, one of the bolts struck out hitting the employee above the left knee then rolling up the left arm.

Employee was assisting a mechanic to manually turn the drive on a Telsmith 68S crusher to prime the oil system when he accidently pinched his right little finger and ring finger in the v-belt against the sheave. The injury resulted in the amputation of the tip of his little finger and reconstructive surgery to the tip of his ring finger.

Repetitive motion from entering/exiting the loader.

Employee was digging out the primary crusher. A rock rolled down from the feeder and struck the employee in the left lower back.

While climbing off a loader the employee may have experienced a knee injury.

Employee stated that he hit a bump while driving a haul truck and jarred his back.

Employee utilizing Oxy Thermal cutting torch to remove wear parts from impact crusher; while cutting, a piece of slag landed on the employee's left knee and burned through his pants & subsequently burned the outside of left knee. Slag traveled down pant leg and burned top of left foot.

The employee was assisting another employee burning, when the oxygen line developed a leak, causing a flash and burnt the employees right thumb. Water-jel was applied immediately and the employee was treated at the local clinic.

While cleaning up a construction site the employee was lifting a 30 lbs piece of metal when he felt a strain in his low back.

While operating a Cat 773D haul truck, employee bumped his head against the window frame, causing a contusion and small cut to the left side of his head. He suffered from headaches and was placed on restricted duty.

The employee was in the process of using a hammer to hit a wedge when a piece of metal broke off and struck him in the left wrist.

Employee changing ball valve on grease line(pressure side). Employee did not release/purge pressure from system prior to attempting to change ball valve. Employee received a high pressure injection injury(grease).

Getting ready to install hoist cable on dragline, employee stepped down turning his knee on rock.

When the employee dropped into a crawl hole he landed wrong on his left knee.

The employee was operating the locomotive and when he was given the signal to stop he struck his shoulder on the window frame.

Hot slag from welding flew into eye of employee.

While attempting to bend a metal tab on a Clinker Cooler door using a four pound sledge hammer, the hammer hit the edge of the door, deflected and hit the worker's left thumb. The impact resulted in a laceration requiring sutures.

While placing a wrench on a cam nut the wrench slipped and fell to the deck where the employee's hand was located. The wrench struck the employee's right hand.

While carrying a 1 1/8' x 4' x 8' wire screen cloth in the tertiary tower the employee felt a strain to the upper back.

Installing head liners in the ball mill, employee lost footing while hooking liner to the hoist and strained his upper back. Employee was placed on restricted duty for 2 days.

While removing the use liner from the head of an Omni Cone Crusher, the employee was lifting with a crane and hitting the liner with a 3 lb hammer to break it loose. The hammer struck the employee's right middle finger.

Driller pulled truck over to park it and to get out of the truck. When he stepped out of the truck, a rattle snake bit him.

Employee was climbing in to a chute to do repair work when he slipped and fell and injured his wrist and brusied over back

EE was assisting the foreman sort railroad iron with a backhoe. The foreman was pulling a piece of iron with the bucket of the backhoe when it became stuck & then suddenly came loose & flew striking the EE about 8" below the left knee; this resulted in a broken leg.

Returning from dumping a load of rock, he hit a pot hole in the road, which jarred him in the truck & caused him to strain his back.

Carpel Tunnel Syndrome; repetitive motion injury.

The employee was assisting moving tire grates from the truck tire wash area, when he was struck by a chain on his inner knee area.

Tech was reinstalling rim base componets. He had his left hand on componets adjusting to position properly..side ring slipped from tire positioning hand and caught finger tips of fingers on left hand between bead seat band and side flange. Amputated pinky finger at 1st knuckle, laceration index finger, loss of finger tips to ring and middle finger.

On conveyor #2 unplugging head box, using a 5' pry bar to wedge between rocks. Pry bar popped out of rocks & smashed little finger between pry bar & steel plate skirt board. Wound required sutures to close, reason for reporting.

Clearing belts with blower. Put hand on belt. Fan moved catchig finger between pulley and belt.

Miner was changing a belt on a crusher, that was locked and tagged. As he was changing the belt, the belt slipped and caught his finger between the belt and pulley. He had to have 4 stitches and broke the bone on his left pointer finger.
