While operating a dozer the employee felt a sharp pain in his right shoulder and shoulder blade. Initially this was first aid only. Employee terminated 11/06/07 due to work force reduction prior to being released to return to work full duty (right shoulder surgery performed 12/13/07).

Employee was walking from the bathroom to the lunchroom when he dropped a battery. As he went to retrieve the battery, which had rolled under a bin, a raccoon reached out and bit the employee.

The employee was tasked with performing the monthly fire extinguisher inspection. The employee was in a cab of a front end loader and dropped the extinguisher on the cab floor. As he retrieved the extinguisher he activated it and the dry chemical hit the cab floor and sprayed him in the face.

Employee was assisting a mechanic to manually turn the drive on a Telsmith 68S crusher to prime the oil system when he accidently pinched his right little finger and ring finger in the v-belt against the sheave. The injury resulted in the amputation of the tip of his little finger and reconstructive surgery to the tip of his ring finger.

Employee utilizing Oxy Thermal cutting torch to remove wear parts from impact crusher; while cutting, a piece of slag landed on the employee's left knee and burned through his pants & subsequently burned the outside of left knee. Slag traveled down pant leg and burned top of left foot.

The employee was assisting another employee burning, when the oxygen line developed a leak, causing a flash and burnt the employees right thumb. Water-jel was applied immediately and the employee was treated at the local clinic.

EE was assisting the foreman sort railroad iron with a backhoe. The foreman was pulling a piece of iron with the bucket of the backhoe when it became stuck & then suddenly came loose & flew striking the EE about 8" below the left knee; this resulted in a broken leg.
Tech was reinstalling rim base componets. He had his left hand on componets adjusting to position properly..side ring slipped from tire positioning hand and caught finger tips of fingers on left hand between bead seat band and side flange. Amputated pinky finger at 1st knuckle, laceration index finger, loss of finger tips to ring and middle finger.

On conveyor #2 unplugging head box, using a 5' pry bar to wedge between rocks. Pry bar popped out of rocks & smashed little finger between pry bar & steel plate skirt board. Wound required sutures to close, reason for reporting.

Miner was changing a belt on a crusher, that was locked and tagged. As he was changing the belt, the belt slipped and caught his finger between the belt and pulley. He had to have 4 stitches and broke the bone on his left pointer finger.
