Miner was walking down the stairs of a tool trailer carrying tools. The first step of the stairs had been repaired with grating but when the miner stepped on the step the repair came loose causing the miner to lose footing. The miner dropped what EE was carrying and grabbed the railing with right hand. As EE caught self on the railing, EE's weight twisted right wrist.

EE was climbing ladder when his muddy safety boots slipped on ladder. Left side contusion and cracked lower vertebrae - MRI only No Meds. No Treatment by doctor at E.R.
EE tripped on shaker tray and fell onto hand cart. EE's lower back sore and left wrist sore and bruised.
Employee slipped on ice on 01/09/09 and said he was o.k. - Was not reportable - Employee stated on 03/25/09 that it got worse and was seeking medical treatment.
EE was carrying a tail pulley with another EE. They were putting the tail pulley in back of foreman's pickup truck bed. While putting the tail pulley back in the truck bed, EE slipped on another tail pulley twisting his ankle.
Employee was descending a ladder from the conveyer, after greasing and performing minor maintenance. He slipped landing on his heels, falling back onto the ground, jarring and injuring his back.
Walking over to loader #193, he slipped on ice, injuring his low back. Heavy rain with night temperatures below freezing.
EE WAS WORKING OFF THE LADDER 8-10 FT IN THE AIRHE WAS FIXING A CLEANING THE BELT OFF THE SCREENER PLANT, WHERE THE PRIMARY CRUSHING CONVEYOR FEEDS THE SCREENER CONVEYOR. THE LADDER KICKED OUT CAUSING DENNIS TO FALL 8-10 FT.
