EE was removing a broken coil spring from a jaw crusher. EE thought had released all tension on the spring, but bottom broken section was still under tension. As EE was removing spring, bottom section released smashing LT thumb.
Employee was working on tail pulley when it slipped and partially fell on left thumb. Left thumb badly swollen - clinic said ice only - no stitches - no broken bones - was given narcotic for pain.
Employee leaned on hot muffler with palm of left hand. Employee went to E.R. and was treated. Employee said he felt fine and reported back to work on 9/13/13 with limited duty.
EE was climbing ladder when his muddy safety boots slipped on ladder. Left side contusion and cracked lower vertebrae - MRI only No Meds. No Treatment by doctor at E.R.
EE was assisting in setting up the crusher and said he strained his back. Said he was ok, this claimed turned to reportable because ee is seeing a chiropractor.
EE was lifting conveyor and re-adjusting lift chain/chain slid on link severing left thumb tip.
Employee was descending a ladder from the conveyer, after greasing and performing minor maintenance. He slipped landing on his heels, falling back onto the ground, jarring and injuring his back.
SHOVELING CONVEYOR SPILLAGE, EE STRAINED LOWER BACK.
EMPLOYEE CALLED & STATED HE HURT HIS BACK AWHILE AGO. HE SAID HE NORMALLY GOES TO THE CHIROPRACTOR 2 OR 3 TIMES & IT HELPS THE PAIN BUT IT DID NOT HAPPEN THIS TIME. HE IS IN SEVERE PAIN & WALK ING AWKWARDLY.
RESETTING CONVEYOR WITH LOADER AND LIFTING CHAIN. CHAIN WAS UNHOOKED AND SWINGING WHILE LOADER OPERATOR WAS BACKING UP. CHIN LATCHED ON THE CONVEYOR WHILE LOADER WAS STILL BACK UP, SWINGING TH E CONVEYOR INTO EE AND LANDING ON HIS RIGHT FOOT.
REPAIR WORK BEING PERFORMED, USING A HAMMER. THE HAMMER WAS SWUNG, HITTING INJURED EMPLOYEE'S LEFT 2ND FINGER. (BROKEN)
