Caught in, under or between a moving and a stationary object

miner was attempting to break a stuck boulder on a jaw crusher feed chute using a pry bar. While striking downward EE's RT thumb was crushed between the bar and toe plate of the platform EE was kneeling on.

Employee was removing/unjamming rocks from a cone crusher. While adjusting the rocks one fell on the tip of EE's right ring finger causing a small laceration and swelling.

Employee said right eye was irritated. Employee taken to Davis Duchr eye clinic. Clinic said there was no foreign object in the eye. Eye was flushed and was told to put drops in and it would be fine. Employee does wear contacts which might have scratched the lens.

Employee was lifting crusher screen when he dropped it against left shin causing scrape.

Employee was changing crusher dies when the welded hook fastened to the die failed. Legs of the employee were trapped underneath the crusher die.

EE was lifting blocks when he felt slight pain in stomach went to MD 9/11/2009 diagnosed with hernia. Able to work until surgery can be scheduled.

EE was carrying a tail pulley with another EE. They were putting the tail pulley in back of foreman's pickup truck bed. While putting the tail pulley back in the truck bed, EE slipped on another tail pulley twisting his ankle.

Operating Bobcat, he positioned Bobcat to hook onto trailer. Trailer has a jack w/wrench. He lifted tongue of trailer to hook onto ball of the Bobcat.
