Employee was on a stepladder when the ladder shifted causing the employee to strain his left shoulder. On 7/9/15 he was diagnosed with a left shoulder rotator cuff tear.
Third-party truck driver alleges he strained his lower back after he dismounted from the cab of his truck. He alleges his left foot stepped into a hole on the ground and he twisted his left ankle resulting in him almost falling. The treating physician issued work restrictions and pain medication. The truck driver was unable to return to work and lost four scheduled work days.
While employee was walking on a product barge, he lost his footing and fell to a maintenance barge below resulting in a fractured right clavicle.
While exiting the load the employee stepped in the mud and his foot got stuck. The employee strained his left knee trying to get his foot out of the mud.
Contract cleaning employee climbing stairs while carrying items in both hands lost balance and fell; however, caught herself with left hand. Injured contract employee reported injury to mine operator on June 5.
Slipped on a piece of metal and fell off the edge of it. When he fell he hit his leg on a piece of metal and bruised his leg.
Individual was climbing the ladder on a 785 haul truck missed the top grab bar and fell to the ground.

EE was in the process of descending the ladder of an off-road haul truck during pretrip inspection and slipped on step (fall to below). EE sustained fracture to left leg (unspecified). EE stated buildup of mud on his boots caused him to lose traction with step surface.
Employee was coming down off a ladder when he stepped back tripping over an air hose he did not see behind him. He put his hand out to break the fall fracturing the 3rd finger of his right hand.
Employee slipped on ice while working in the plant and injured his shoulder.

Injured employee's medical treatment includes a splint and restricted work (no lost time).
EE was switching a flop gate, the wrench he was using slipped off. EE fell and caught himself with his left hand resulting in a injured shoulder.

Contractor from Westate was exiting out of the passenger side of the service truck. Contractor came out facing forward and slipped on the top step and fell to the ground. Contractor then fell and struck his head on the left upper side on an unknown object. Contractor was escorted to Belgrade Urgent care by his supervisor. Contractor received 4 staples to his wound on his head
Three employees were lifting a 100 lb crated bearing. The injured employee tripped on a pallet and fell to the ground. The bearing fell out of the box and broke the left index finger and middle finger. The employee received 15 stitches and was released back to work to full duty.
Employee was climbing adjustable step ladder. After getting to the 3rd rung the adjustable portion of the ladder failed causing the employee to fall off the ladder fracturing his left wrist.
Individual was traveling to get a clinker sample stepping over a chain tripped on the chain and fell striking his shoulder on the ground

While helping guide a loader moving a 14 inch poly pipe the chain slipped over the loader bucket causing the pipe to fall and swept the EE feet and legs out from under him fracturing his left leg
Employee was working on tracks. He did not realize that he was so close to the open part of the truck bed and when he turned to step away he fell into the open section of the trailer landing hurting his left thigh and right shoulder.
EE mounting 988G when step broke at weld. EE swung out and held onto handrail with left hand and all body weight. EE strained shoulder. EE given modified duty on 5/1/12.
The Red River had been flooded for 2 weeks leaving mud along bank edge EE stepped of loader and sunk knee deep in the mud injured left knee twisting to get unstuck.
EE was removing hood of S Elevator (FM2) with overhead hoist. Dust blew off hood as it was positioned in front of fan exhaust. EE attempted to move out of way and tripped over conduit in-between elevator housings. EE landed on shoulder. EE sought treatment on 4/4. Given pain medication and restricted duty for 4 days by clinic.
Walking through chain hanger section of kiln, got foot hung up between hangers and fell striking head on a hanger.
EE arrived at the work site prior to the start of his shift. EE proceeded to head towards the mechanic shop EE parked his car in a location that had been plowed last week it was not the designated parking area. The EE got out of his car and proceeded to walk over an area of accumulated snow and ice. While doing so lost his footing and fell on his side.
Employee was torquing head liner bolts on the raw mill. As employee was torquing the ratchet come loose causing employee to loose his balance. As employee step back to catch his balance he rolled his ankle and his lower leg absorbed all the weight, fracturing his lower leg.
EE was on the crane by the spool and the cable broke causing him to lose his balance so he jumped to the ground instead of falling. He fractured his right heel
Employee was walking down stairs when slipped and fell resulting in a fractured right ankle and torn meniscus in the right knee.
Employee was measuring Bulk Silos, stepped over the transfer line and came down on his left foot turning his ankle causing a fracture to the fibula.

Jack hammering castable at 295 foot section of cement kiln. A piece of coating fell from above & struck left thigh causing him to fall.
The security officer left the guard shack to get a breath of fresh air. Within a few feet of the guard shack, the officer slipped and fell on a piece of ice. When the officer's shift ended, he was treated by medical professionals, found to have a fractured wrist but was released to go back to work. Therefore, no work hours were lost due to the injury.
The miner was walking down a set of stairs in the clinker pit. When he stepped off the bottom step his foot hit an unlevel place on the floor and he rolled (sprained) his left ankle and fell on the floor. He was taken to the ER room.
Shutting door on the loader while getting out. Lost balance and fell to the ground.

EE exiting personal vehicle and slipped in parking lot. EE thought he slipped on round object but could not find anything.
Slip and fall same level, jammed right shoulder, continued working. Surgery on 2/11/2011 on right shoulder will restrict duties.
EE stated that he stepped of haul truck ladder at end of shift and aggravated a pre-existing back injury. EE stated he twisted back, possibly from a loose stone or small hole. EE did not feel any pain at that time, but woke the next day with back pain. EE's attending Dr. gave him 2 days off work, and returned to work with no restrictions after his scheduled week of vacation.
While climbing up a ladder to fuel machine, employees hand slippped off the rail causing him to fall to the ground. Employee suffered bruises to back and leg and is on restricted duty.
While getting out of the car for work EE slipped and fell on ice in the gravel parking lot. Due to freezing rain conditions the parking lot was slick. The parking lot had sanding stations and had been sanded.
EE was on a ladder repairing florescent light fixture in 9' ceiling in the plant conf. room. EE broke his right arm when he caught himself after missing last step of the ladder as he was climbing down.
Employee was exiting cab of locomotive and while stepping out on the two steps that stepped down to the cat walk he did not get solid contact with the 2nd step and twisted his leg. He grabbed the rail and stopped his fall but his leg hit the catwalk with alot of force...breaking the bone below the knee in 2 places.
EE fell while supervising coal spill clean-up at tripper belt in the material storage shed.
EE was walking from Clinker Cooler when EE stepped off 3" platform which he did not see, landing off balance on his R foot. When EE tried to recover with L foot, EE slipped in slurry falling to his R knee. EE flet pain in R knee and inside of R groin area.
