Emp was working with two other emp installing erosion control matting and grass seed on a dirt slope. While placing rock and staples for holding the matting the emp right foot slid into a washout causing them to fall. Their foot was wedged in the washout and remained stationary as they fell resulting in twisting to their ankle.

Miner was working on a pipe fitting laying on the ground. Miner attempted to step over the fitting and tripped and fell on EE's arm.
Some ice had built up due to weather on the apron of a shop bay. Employee was attempting to move a truck off the apron so the ice could be cleaned off when employee slipped and fell, injuring left wrist.
While walking up a small pile at the blend 1 conveyor to change wipes, employee stepped on the loose material and left knee gave out.

After the shift employee had exited the Scale House and fell down stairs while walking to EE's car
Injured worker was bent over walking under a belt, stepped on uneven ground, rolled ankle, and almost fell. While trying to catch self from falling the worker strained lower back. The worker did not actually fall. The worker notified supervisor 2 days later, went to the ER where worker was given a prescription for a muscle relaxer and taken out of work for 5 days.
Employee tripped while exiting skid loader and caught thumb in door handle at the same time. Sprained thumb.
Employee stated that EE had decided to check the wedges on the side of the Jaw Crusher to make sure they were tight. EE did not get a lanyard to tie off, or a step ladder. EE said EE worked EE's way out on the side of the jaw, standing on the bottom web of the structure I-beam. EE slipped and fell four feet, primarily on ankle.
Miner was climbing the 4th step of haul truck with cooler over left shoulder and lost balance when cooler slid down shoulder. Miner fell, hitting left heel and left side on ground. Initial injury was a bruised foot/heel. Miner continued to feel pain in left side months after fall and returned to doc. Miner had surgery on 2/10/21 to repair a broken hip.
Employee was dismounting from the top of a horizontal tank, to a ladder to climb down. The ladder's feet slipped, resulting in the employee falling to the ground. Employee was not knocked unconscious and otherwise refused medical treatment. After recurring headaches, medical treatment was sought 6 days after the incident occurred.
Employee descended steps to primary hopper and after turning around lost footing and hyperextended left leg.

The Sales Manager slipped while walking over a small mound of looses rocks and dirt and fell to the ground resulting in an unspecified left shoulder injury possible rotator cuff tear.
EE dismounted front end loader. As EE started to step away from machine EE caught toe on a rock & felt a pop in right knee. EE continued working & didn't request to see a doctor until 04/18/18. Diagnosed with a sprained knee & allowed to continue to work regular job. Report submitted within 10 working days initial doctor visit. *EE began losing time on 6/26/18 for surgery.
Employee was dismounting the 980 front end loader at the end of the shift when ee missed the last step while climbing down. As ee stepped to the ground ee twisted and jarred body resulting in a back sprain. EE sought medical attention later in the day.
While installing a classifier on the wet site of plant, employee was welding on a new sand chute. At break time, the employee was descending the chute and foot slipped causing EE to fall on back. That is when the dirt from the other truck come loose slide down and struck him in the back of his legs ,half way to his calfs. This put him on his knees. While climbing down some steps and upon stepping onto a smooth steel platform, he slipped on some limestone dust and fell on his left side, breaking his left elbow. Employee also mess up his left shoulder, may have injured his lower back.

The operator of a CAT 990 front end loader heard a noise so he opened the inspection door to the engine. After finding nothing he proceeded down the steps to dismount at the rear end of the loader. When stepping down the ladder he missed the first step causing him to fall on his left side.
