Showing all 32A trespasser not employed by the mine operator entered the property when the mine was not in operation and died as a result of an apparent suicide. *The cause of death was multiple blunt force injuries as a result of jumping off a cliff into the quarry.
Miner was climbing a six foot step ladder. Upon reaching the third rung of the ladder his foot slipped off the rung and he fell to the ground. The ladder was on a concrete pad at a wash plant and soles of boots were wet.
The EE slipped on step of dozer, caught himself with the grabrail and strained his left elbow.

Employee tripped while walking up stairs (5) to office and strained his right knee, he has a history of knee problems and we have scheduled an MRI for a possible cartilage tear.
EMPLOYEE WAS ATTEMPTING TO START A SKIP-LOADER TRACTOR. THE STARTER BECAME ENGAGED AND WOULD NOT RELEASE. THE STARTER BEGAN TO SMOKE SO THE EMPLOYEE HURRIED OFF THE TRACTOR THINKING HE NEEDED TO USE THE FIRE EXTINGUISHER. IN HIS HURRIEDNESS HE TWISTED HIS ANKLE.
EE WAS MEASURING A STEEL BAR THAT WAS LAYING ON A RAILROAD TIE. WHEN HE STEPPED BACKWARDS WITHOUT LOOKING, HE TRIPPED AND FELL. HE EXTENDED HIS LEFT HAND AND BROKE HIS WRIST.
EE WAS EXITING DOZER, SLIPPED ON TRACKS AND STRUCK THE DOZER TRACK WITH HIS UPPER TORSO AND BACK.

EMPLOYEE STEPPED FROM ONE ELEVATION TO ANOTHER, CAUSING A JARRING TO HIS LOWER BACK. EMPLOYEE HAD A PRE-EXISTING BACK/VERTEBRATE DEGENERATION.
WHILE EXITING ROCK TRUCK EMPLOYEE SLIPPED & FELLTO THE GROUND STRIKING HIS HEAD ON THE GROUND. EMPLOYEE WAS WEARING A HARD HAT.
