employee slipped and fell on their right side of their body

A trespasser not employed by the mine operator entered the property when the mine was not in operation and died as a result of an apparent suicide. *The cause of death was multiple blunt force injuries as a result of jumping off a cliff into the quarry.

Miner was climbing a six foot step ladder. Upon reaching the third rung of the ladder his foot slipped off the rung and he fell to the ground. The ladder was on a concrete pad at a wash plant and soles of boots were wet.

The EE slipped on step of dozer, caught himself with the grabrail and strained his left elbow.

After talking with a individual about an emotional subject, the employee became sick and passed out causing the employee to fall. He struck his head on a rock and lacerated his head. This laceration required (5) stitches.

EE was climbing down machine, when he caught his foot on the lower step and fell about 2 1/2 to 3 feet on his backside, bruised back.

Employee was working on screen deck when he misplaced his footing resulting in a laceration to his left shin which required stitches.

Employee left his flashlight in his truck. When walking to his equipment he tripped and fell. He was laid off, due to workforce reduction. After layoff the clinic put him on restricted duty on 6-22-07. This was after three prior visits stating regular work.

Operator was dismounting piece of equipment. Operator slipped and fell to the ground injuring her wrist.

Employee was walking down steps from lunch room trailer when he rolled his left ankle.

Employee was walking out of the scalehouse and fell on the threshold causing her to twist her ankle.

Employee reported injury and requested to see dr on 9/8/06 stating he had pulled a muscle in groin area due to slipping in mud. Placed on modified duty.

EE was cleaning concrete floor around clarifier. EE slipped and hit right forearm on metal I beam. Resulting in a muscle bruise.

Walking around the screen catwalk and misstepped on slopped catwalk.

While walking down a bank of pit material ee slipped and struck his hand right on a sharp rock causing a small laceration to his little finger.

Employee stepped down to a platform from the screen area and twisted his left ankle.

EE stepped on a muddy gravel area as he was walking over to pick up a hose that he had been usig to wash clean out under the el jay screen. as he did, his right knee went sideways and popped.

Employee slipped off ladder and fell 2 feet onto a platform.

EE CLAIMS HE FELL ON OPERATING LEVERS OF 631 SCRAPER GRANITE INFORMED OF INJURY WHEN WE REC'D BILLING FROM HOSPITAL 1/13/05

Employee tripped while walking up stairs (5) to office and strained his right knee, he has a history of knee problems and we have scheduled an MRI for a possible cartilage tear.

EMPLOYEE WAS ATTEMPTING TO START A SKIP-LOADER TRACTOR. THE STARTER BECAME ENGAGED AND WOULD NOT RELEASE. THE STARTER BEGAN TO SMOKE SO THE EMPLOYEE HURRIED OFF THE TRACTOR THINKING HE NEEDED TO USE THE FIRE EXTINGUISHER. IN HIS HURRIEDNESS HE TWISTED HIS ANKLE.

EE WAS MEASURING A STEEL BAR THAT WAS LAYING ON A RAILROAD TIE. WHEN HE STEPPED BACKWARDS WITHOUT LOOKING, HE TRIPPED AND FELL. HE EXTENDED HIS LEFT HAND AND BROKE HIS WRIST.

EE WAS EXITING DOZER, SLIPPED ON TRACKS AND STRUCK THE DOZER TRACK WITH HIS UPPER TORSO AND BACK.

EE WAS CLIMBING INTO WATER TRUCK, FOOT SLIPPED AND HE FELL TO THE GROUND, LANDING ON HIS RIGHT ELBOW.

PIT SCREEN HANDRAIL BROKE CAUSING A FALL OF APPROX. 5 FEET.

EE HAD BEEN OPENING UPS FREIGHT BOXES AND SHE WENT IN TO THE NEXT ROOM TO GET SOME WATER, WHENSHE RETURNED SHE ATTEMPTED TO STEP OVER A BOX & FELL.

WATER TRUCK DRIVER PULLED HIS WATER TRUCK UNDER WATER LOAD SPOUT TO FILL 3000 GALLON TANK. HE FELL 8 FEET TO THE GROUND.

EMPLOYEE WAS WORKING ON SCREEN. STEPPED BACK & FELL ABOUT 2 FEET ONTO BELT, HIT HIS HIP & TWISTED BACK A LITTLE.

EMPLOYEE WAS WALKING IN SAND FRON THE EXCAVATOR TO THE FEEDER WHEN HE STEPPED ON A ROCK AND TWISTED HIS ANKLE.

EE WAS ENTERING CHUTE BELOW #1 SCREEN OF PLANT WHEN HE LOST HIS BALANCE. HIS LEFT FOREARM STRUCK AGAINST THE ACCESS FRAME WHICH HAD BEEN SHARPENED BY MATERIAL.

EMPLOYEE STEPPED FROM ONE ELEVATION TO ANOTHER, CAUSING A JARRING TO HIS LOWER BACK. EMPLOYEE HAD A PRE-EXISTING BACK/VERTEBRATE DEGENERATION.

WHILE EXITING ROCK TRUCK EMPLOYEE SLIPPED & FELLTO THE GROUND STRIKING HIS HEAD ON THE GROUND. EMPLOYEE WAS WEARING A HARD HAT.
