After talking with a individual about an emotional subject, the employee became sick and passed out causing the employee to fall. He struck his head on a rock and lacerated his head. This laceration required (5) stitches.

WHILE LOOSENING NUTS ON JAW CRUSHER WITH WRENCH THE NUT BROKE FREE AND THE WRENCH SMASHED #5 RIGHT FINGER AGAINST CORNER OF JAW CRUSHER FRAME CUTTING IT. INJURY RESULTED IN 6 STITCHES.
