Showing all 15Employee was working under wash screens to pull the drain plug out of the concrete screw. The drain plug was frozen and the employee walked back to the maintenance truck to get tools. EE slipped on the ice and struck their head on the ground below.
Employee Statement - Put tubes out. Had one remaining and took it to stemming pile where extra tubes are kept. Walking back to drill, I slipped on some stemming #6 stones that were on top of smooth bedrock. My right foot went up and my left foot ended up underneath me. I felt my left ankle pop.
Ice on ground after snow storm and thaw/freeze. Worker was walking to pick up truck and fell. As EE was falling EE tried to put arm out to catch self which caused a Humerus fracture to right arm.
Stepped out of the skidsteer and twisted ankle.
The contractor driver slipped when tarping the trailer. MSHA was notified at 2:47 p.m.
THE DRIVER WAS ATTEMPTING TO CLOSE THE SLIDING WINDOW ON THE WATER TRUCK FROM THE SEAT & AFTER BEING JAMMED ON THE 1st ATTEMPT, PULLED HARDER ON THE 2nd ATTEMPT & THE WINDOW BECAME FREE & THE DRIVER LOST HIS BALANCE & FELL OUT OF THE CAB ONTO THE GROUND LANDING ON HIS FEET & THEN FELL FORWARD ONTO A PALLET OF SCREEN CLOTH CUTTING HIS LEFT KNEE.
While climbing up into the terex haul truck (45 ton #26.8939) His right foot slipped off the step, causing him to fall He fell about 4 ft. He said he had pain in right foot and right knee. About and hour after accident he went to St. Lukes Hospital. They took x rays and was discharge )sprain right knee and ankle). He had wet shoes when he went to get into the truck.
He slipped on loader step while getting in. Snow on shoes (no fall).
EMPLOYEE WAS UNLOADING SKIDLOADER OFF OF A UTILITY TRAILER. WHEN HE STEPPED OFF TRAILER HE STEPPED ON UNLEVEL GROUND AND TWISTED HIS ANKLE. HE WENT TO THE HOSPITAL FOR X-RAYS. NO BROKEN BONES WERE FOUND. HE RETURNED TO WORK THAT DAY WITH A A SEVERE ANKLE SPRAIN.
WHEN HE WAS GETTING OUT OF MANLIFT, HE STEPPED OUT OF MANLIFT INTO A TRENCH AND TWISTED HIS LEFT ANKLE. HE WENT TO WOOD COUNTY HOSPITAL AND GOT AN X-RAY.
LOADER OPERATOR WAS WALKING TOWARD THE PRIMARY CRUSHER. HE STEPPED ON UNEVEN FROZEN GROUND AND HIS RIGHT ANKLE TWISTED/ROLLED, CAUSING A SPRAIN/FRACTURE TYPE INJURY. EE STATED THAT HE HAS HAD SOME HISTORY OF PROBLEMS WITH THIS ANKLE.
EE WAS STANDING ON A WALKWAY. AS HE TURNED TOWARD HIS WORK HIS LEFT KNEEE TWISTED CAUSING DISCOMFORT (1/30/01). NO MEDICAL TREATMENT UNTIL 2/10 WHEN X-RAY TAKEN. SAW DOCTOR ON 2/17 & HAD FLUID REMOVED & PLACED ON RESTRICTED DUTY.
EE WAS FUELING UP AND WALKING AROUND THE FUEL PUMP WHEN HE SLIPPED ON THE ICE. HE ENDED UP DOING THE SPLITS AND PULLING HIS GROIN.
AT THE END OF HIS SHIFT, WHEN EXITING DOZER, EE STEPPED OFF INTO A HOLE. HE SLIPPED AND FELL ON HIS RIGHT ELBOW, FRACTURING IT.
