Employee was working in the CMF on the splitter nearest the office. Was helping the operator move a rock standing on the machine leg. The employee's foot slipped and went into a slit. The employees right foot then got stuck between two pieces of iron when a waste tub was slid into place by the material handler.

Employee was getting out of the cab of the JCB and missed the step. EE didn't grab the hand rail first and then fell backwards landing on EE's lower back and butt.

Employee was stacking rock near a drain, employee tripped on slope and twisted foot.

EE is a Splitter Operator in our Chilton Manufacturing Facility at our Chilton Quarry. EE was leaving the shift early at 5:00pm as EE was feeling ill. As EE was walking out to the car in the parking lot, EE slipped landing on right side. A salt/sand mixture and salt is used regularly in winter to limit slippery spots in our parking lot and employee walkways.

Employee works as a Material Handler in shipping and receiving. While ee was grabbing a box from a pallet, feet slipped forward on the ice. EE said ee, "jarred neck." Employee did seek medical treatment and the doctor diagnosed it has a neck strain.

Employee had pulled into the quarry and parked semi in yard to hook up flatbed trailer. Fell of top step of truck & landed on gravel lot. Broke left wrist, right thumb & needed stitches in forehead.

Employee was working indoors on a splitter. While splitting a stone, the employee went to brace his foot on the machine and his foot slipped resulting in him falling over onto his back. The employee went and got medical treatment and the final diagnosis was a lumbar strain/contusion.

An employee who is a material handler was working indoors and noticed a co-worker struggling with a large stone. The stone broke in half and the employee tried pulling part of the stone out with a pry bar. While trying to pull out the piece of stone, the employee lost balance, tripped over a scrap tub and hit his head on the splitter machine.

The employee was walking down stairs and got her heel stuck in her pant leg causing her to fall down four steps.

Employee was climbing on a rock and slipped and fell on some loose rock.

Employee was getting out of skid steer. His right foot got caught on the forks causing him to fall; spraining his left ankle/foot. Employee was given a prescription.

Employee was walking along berm on way to lunch break. A stone on the berm slipped, causing the employee to fall. Employee sprained his right ankle. Restricted duty assigned.

Employee was walking across driveway to the break trailer when he slipped on ice and fell landing on his right shoulder. Restricted duty assigned. **Employee did not report the injury until 1 2/15/2009**

Employee tripped over a box of shims; he fell and landed on his right elbow. Restricted duty assigned.

Employee was walking from the break room to his car; he slipped on ice and twisted his lower back. **Employee did not report the injury and seek medical attention until 3/21/08. Employee was released to full duty the next working day; the return to work slip did not indicate employee was given a prescription.

Employee was walking in the quarry and slipped on the ice and fell on his left side. Restricted duty assigned.

Employee was walking to break room to clock in when he slipped on the ice. He fell on his right knee which he recently had surgery on. Restricted duty assigned. Employee was suppose to report to the Chilton Quarry; he instead reported to the Sabel Quarry.

Employee climbed onto conveyor to remove a stone jamming the belt. When he was coming down off the conveyor belt, he slipped and landed on his left knee.Diagnosis:Knee contusion; restricted duty assigned

Employee was cleaning up area. He slipped on the ice while picking up a stone; the stone landed on him. Diagnosis: Neck strain & occipital head injury; Restricted duty assigned

Employee slipped on the ice and hit his right elbow on the stairs of a Loader. Diagnosis: Elbow Contusion. Restricted duty assigned. **Employee did not seek medical treatment until 3/4/08**

Employee slipped on the steps to the platform of the splitter and twisted his ankle. *Right ankle sprain *Off Work

Employee was walking down the hill by the splitter shop, slipped on a patch of ice and fell on his backside. *Did not seek medical treatment until 3-6-07. Contusion of muscles - low back. Restricted duty.

Another EE pushed this EE. Injured EE fell and hit a fire extinguisher causing a laceration needing stitches.

Employee was hurrying around truck. He slipped on a patch of ice. He grabbed the mirror bracket, his wedding ring got caught, causing laceration and possible ligament or tendon damage.

Employee was walking from one building to another and slipped on the snow and landed on his buttocks. *Was not determined to be work related (per Doctor) until 12-29-06. Lumbar disc displacement* *off work*

Employee tripped over a cord as he was walking back to his work station. He landed ont he floor and hurt his shoulder. Left shoulder injury-restricted duty.

Employee was leaving for lunch. She tripped on some small stones on the outside staircase. Sprained hand. Prescribed medicine.

EMPLOYEE WAS WALKING IN THE WALKWAY BY THE TRAILER AND SLIPPED ON SOME ICE. HE FELL ONTO HIS BACK AND LANDED ON HIS LUNCHBOX. L. RIB #10 CONTUSION, L. LOW LUMBAR STRAIN/SPRAIN. RESTRICTED DUTY & PRESCRIBED MEDICATION.

EE was walking back to his work area out in the quarry and slipped and fell on a patch of ice. Sacral contusion and restricted duty.

EMPLOYEE WAS WALKING & TWISTED HIS ANKLE, HE SPRAINED HIS ANKLE. EE DID NOT SEE A DR. UNTIL 6/7/05.

Employee slipped and fell on ice in the quarry while performing his regular duties of splitting stones. Diagnosis: broken r foot.

Employee stepped down from a forklift. he stepped on a small stone and twisted his ankle. sprained ankle and foot contusion employee did not seek treatment with 11/10/04

EE WAS WALKING BETWEEN SOME PALLETS WHEN SHE TRIPPED OVER A ROPE PARTIALLY HIDDEN UNDER A PALLET. DIAGNOSIS: FRACTURED RIGHT ANKLE.

AEE WAS PULLING OUT HOSES TO SET UP WATER PUMP HE SLIPPED IN THE MUD AND FELL, LANDING ON HIS RIGHT SIDE RIGHT ELBOW JOINT STRAIN

EE WAS WALKING FROM OFFICE TO HIS TRUCK. STEPPED ON A STONE. CAUSING HIM TO TWIST HIS ANKLE. EE FELL ACUTE LEFT THIGH MUSCLE STRAIN.

EMPLOYEE WAS CHECKING THE WEIGHTS OF PALLETS OF STONE ON A FLATBED TRAILER. A WIND GUST CAUGHT HIM AND MADE HIM LOSE HIS BALANCE. HE FELL OFF THE TRAILER AND FRACTURED HIS LEFT HEAL.

EE WAS CARRYING A 20-30LB. ROCK IN A WET WORK AREA WHEN HE TRIPPED AND FELL BACKWARDS. HE LANDED ON A SPREAD PILE OF ROCKS; SPRAINING HIS LOWER BACK.

EE WAS WALKING TO RECEIVE ANOTHER STONE AND SHE TRIPPED OVER A STONE ON THE GROUND, INJURING HER LEFT WRIST.

WHILE CARRYING A STONE. EE TRIPPED DROPPING THE STONE ON HIS RIGHT THUMB.

EE WAS WALKING ON A FLOOR GRATE. THE GRATE WAS SLIPPERY, CAUSING EE TO FALL. THE FALL RESULTED IN INJURY TO HIS LEFT KNEE LIGAMENTS.

WHILE MOVING A WATER HOSE EE SLIPPED AND LANDED ON HIS BACK. HIS RIBS BEGAN HURTING HIM THE FOLLOWING DAY.

EE WAS CLIMBING OUT OF SEMI TRAILER AND FELL TO GROUND AND TWISTED RIGHT ANKLE. STARTED TO SWELL, DIAGNOSIS: FRACTURE TO RIGHT FOOT.

HE TWISTED HIS KNEE TRYING TO GET OUT OF THE WAY OF THE STONE FALLING OFF OF THE BELT. RESULTED IN HIM STRAINING HIS KNEE AND BEING ON CRUTCHES.

EE SLIPPED ON OIL WHILE CLEANING HIS SAW, HE FELL AND HURT HIS LEFT KNEE. HE WAS CARRYING ACID AT THE SAME TIME OF THE FALL RESULTING IN SPILLING ACID ON HIMSELF. HE SHOWERED IMMEDIATELY.

THIS INJURY OCCURRED ON 8-23-00. DIDN'T GOT TO SEE A DOCTOR UNTIL 8-29-00. HE HURT HIS BACK LIFTING A LARGE ROCK, AND TRIPPED ON A SMALL STONE, CAUSING HIM TO TWIST BACK WHEN HE FELL.
