Employee was working in the CMF on the splitter nearest the office. Was helping the operator move a rock standing on the machine leg. The employee's foot slipped and went into a slit. The employees right foot then got stuck between two pieces of iron when a waste tub was slid into place by the material handler.

EE is a Splitter Operator in our Chilton Manufacturing Facility at our Chilton Quarry. EE was leaving the shift early at 5:00pm as EE was feeling ill. As EE was walking out to the car in the parking lot, EE slipped landing on right side. A salt/sand mixture and salt is used regularly in winter to limit slippery spots in our parking lot and employee walkways.
Employee works as a Material Handler in shipping and receiving. While ee was grabbing a box from a pallet, feet slipped forward on the ice. EE said ee, "jarred neck." Employee did seek medical treatment and the doctor diagnosed it has a neck strain.
Employee had pulled into the quarry and parked semi in yard to hook up flatbed trailer. Fell of top step of truck & landed on gravel lot. Broke left wrist, right thumb & needed stitches in forehead.

HE TWISTED HIS KNEE TRYING TO GET OUT OF THE WAY OF THE STONE FALLING OFF OF THE BELT. RESULTED IN HIM STRAINING HIS KNEE AND BEING ON CRUTCHES.
EE SLIPPED ON OIL WHILE CLEANING HIS SAW, HE FELL AND HURT HIS LEFT KNEE. HE WAS CARRYING ACID AT THE SAME TIME OF THE FALL RESULTING IN SPILLING ACID ON HIMSELF. HE SHOWERED IMMEDIATELY.
THIS INJURY OCCURRED ON 8-23-00. DIDN'T GOT TO SEE A DOCTOR UNTIL 8-29-00. HE HURT HIS BACK LIFTING A LARGE ROCK, AND TRIPPED ON A SMALL STONE, CAUSING HIM TO TWIST BACK WHEN HE FELL.
