EE overfilled bucket of bobcat with rock from under crusher hopper, EE was backing up. When positioning bobcat to dump material, bobcat tipped backward into uneven ground causing rocks to fall from back of bucket into cab on miner's leg. Rock lodged into bucket pedal control causing more rocks to fall into cab and strike miner on right leg, causing contusions and abrasions.

Showing all 27Miner was looking for A/C leak when EE's foot slipped from the door way to the track approximately 2 feet. The miner refused medical treatment at the time of injury stating EE's ankle was just sore. The miner stated EE's ankle had not improved on 11/21/2018 and requested treatment, the MRI indicated EE's left Achilles tendon was torn.
miner was operating a crusher when at 11:00 the crusher plugged the miner and ground crew LOTO crusher and began removing material from the crusher. While removing rocks through the access door a rock 20"x15" rock slipped from EE's grip smashing EE's left index finger against the shaft bearing housing.
Employee was carbon arcing inside the materials dryer. A piece of rolled off the welding jacket to EE's groin area causing burns to EE's genitals.
Employee was climbing out of an engine compartment when slipped striking lower right chest area on the frame of the screener
EE was doing an elevator chain and bucket inspection after the elevator had been repaired. EE was sitting on a bucket outside of the elevator, controlling the chain with an inverter and looking up when a 1in -rock became dislodged from the chain/bucket above and struck in the eye. EE was not wearing any eye protection.
While attempting to remove to remove a shiv the pry bar slipped smashing the the tip of the employees right ring finger between the bar and a tension shiv.
After digging out a plugged tail pulley the employee went to start the conveyor belt when a rock 4"X6" fell from the belt striking employee's left hand between the thumb and index finger.
The miner was removing material from an impact crusher that had become plugged up. While standing outside the crushing chamber a rock from the feeder deck became dislodged bounced off a blow bar and struck the miners left knee.
While installing drive belts on a John Deere Inertia crusher the miner felt discomfort in abdomen and lower back. The miner was seen by a doctor at 09:00 12/15/2016. The medical professional diagnosed the problem as a muscle strain and prescribed muscle relaxer, steroids, and restricted duty for 10 days.
Employee attempted to remove rocks from a jammed conveyor belt, ee states the rocks fell onto finger causing the crush injury with chip fracture.
Miner was feeding a portable screen from an 8' bench when EE under cut the bench resulting in the excavator rolling off the bench. The miner was ask multiple times if was injured/needed med attention the miner stated was not injured/declined med attention multiple times. The miner completed shift and performed EE's normal duties til 9/6/16, never reported injury/med attention.
EE was moving piece of metal when it slipped and made contact with EE's mouth, requiring a couple of stitches.
During normal duties the miner was taking out the trash. He picked up a container of cigarette butts and waste material, when he felt a strain in his lower back.
Employee Was clearing topsoil with a dozer. A branch was entangled behind the blade. When the employee tried to remove the branch, it slipped, pinning the employee's right ring finger between the branch and the equipment. When employee removed his glove, the tip of his finger had been removed. Employee kept fingertip and glove with him and was immediately driven to the hospital.
Employee was in the shop area storing bucket pins on a storage rack. A pin fell over pinning his Right index finger between the pin and shelve causing the laceration of the finger.
The miner was using an acetylene torch to remove an unsupported section of an iron platform. The miner chose to make the final cut from below and to the side instead of from above the section. The section fell at an angle striking the miners right foot breaking the cuneiform bone.
Miner was exiting haul truck, Terex TA400, when she let go before she got to the bottom of the ladder.
Truck driver hit by drunk driver. Trucker swerved to avoid fatal accident, bruised right thumb/palm on shift tower. Accident happened on Highway 50 East in Silver Springs, NV.

The miner was using a hand grinder to remove a burr from a piece of metal when a piece of debris bypassed his safety glasses striking him in his left eye.
EE was stepping off ladder onto uneven surface, and felt a pain in foot (i.e. Pulled muscle or sprain).
EE was cleaning mud off of belt when tool caught on belt and injured finger, requiring stitches.
Employee was climbing down ladder on 773D haul truck stepped on a 41/2" rock and twisted his right knee
While using a pry bar trying to remove a piece of steel from between two other pieces of steel. The pry bar slipped and the steel piece fell on thumbs.
While changing teeth on a mill drum the miner could not access 1 tooth holder. so he tried to roll the mill drum forward by manually.
