Employee was lifting rocks out of a crusher and felt a pain in back. Employee was taken for a medical evaluation and x-rays and was diagnosed to have muscle strain and returned to work. On 02/07/2020 the employee reported continued pain was sent back for medical evaluation and 02/10/2020 was given an epidural and instructed to be off till 02/14/2020.

Employee was clearing a blockage in the oversize return chute from the inspection door when a large rock unexpectedly shifted smashing the right index finger between the rock and the inspection door frame .
Employee was removing old plate metal from a JCI screen chute for repairs using a cutting torch. Employee was wearing safety glasses and a face shield. While cutting the employee hit a pocket of dirt cause molten metal to splatter and blow out. A piece of splatter went under the employees face shield and safety glasses causing a burn to the employees right eye lid and cornea.
EE was removing the screw cover for the bag house screw. After removing the bolts the EE discovered there was a tack tac weld. The EE took a 6"" angle grinder to remove the weld as the wheel contacted the weld it kicked back striking the EE in the Right thigh causing a laceration.

During normal duties the miner was taking out the trash. He picked up a container of cigarette butts and waste material, when he felt a strain in his lower back.
Miner was exiting haul truck, Terex TA400, when she let go before she got to the bottom of the ladder.
Truck driver hit by drunk driver. Trucker swerved to avoid fatal accident, bruised right thumb/palm on shift tower. Accident happened on Highway 50 East in Silver Springs, NV.
Miner was changing out the wear plate on an impact curtain. When lifting the new wear plate into place he felt a pain in his left groin. The miner reported the incident to his foreman, it didn't get reported further. On 9/8/15 the miner went to the ER for his injury, and reported his injury to Human Resources. Incident reported to safety on 12/4/15.
Mechanic was using a 75 ton hydraulic press to press a bearing off of a drive shaft. The bearing was stuck, the miner turned around to get a can of P.B. Blast. When he heard a ""pop"". He turned back around and was struck in the face by a piece of the bearing race.

EE was performing serving of equipment. While servicing the 235 excavator the EE attempted to close the oil check door, it kicked back open and jammed the EE's left thumb. Injury was not serious, EE chose to wait for medical treatment. On 8/31/11 EE chose to accept medical treatment which then showed a tear in the tendon.
Employee was utilizing a 6' ladder leaned against the drivers side frame to access 2 battery solenoid boxes. When dismounting the ladder he stepped off the first rung in a backward pivoting motion. His right foot was at an angle & rolled upward as he stepped to the ground. The accident resulted in an unspecified ankle sprain.

Employee was removing melted snow and ice off of the walkway next to the truck scale. He lost his footing and fell impacting the ground on the right side of his body, resulting in a small closed fracture in the pelvic region. (Dr. said not serious.) Employee is moving around on crutches. (Employee was instructed not to shovel due to his age.)
EE was working putting in a differential, after lifting the differential and lining it up, the nut wielded to the differential to stabilize the clevis assembly broke free causing the differential to fall hitting the corner of the pallet below it in return causing it to roll striking ee above the Rt knee. X-rays show no broken bones or limbs. Release the next day.
Lock out tag out procedure was not followed. Miner got his right hand caught in between conveyor belt and slide rollers.

Jaw crusher operator was attempting to dislodge a stone wedged at the crusher jaw throat. While attempting to manually move the stone, another stone slid down the attached grizzly, crushing the employees right hand between the two stones. Employee sustained injury to three fingers on rt hand, with amputation necessary at hospital on first two joints of rt ring finger.

WHILE INSTALLING A HYDRAULIC CYLINDER ON A TRUCK/TRAILER. THE CYLINDER SLIPPED CATCHING THE 3RD FINGER ON LEFT HAND BETWEEN THE TWO METAL SURFACES.
