A LARGE SECTION OF ROOF (SLIP HORSEBACK) FELL JUST AFTER LOADING THE FIRST SHUTTLE CAR OF THE PILLAR PUSHOUT IN NO. 4 ENTRY AT SURVEY STATION NO. 1288. THE ROOF FALL WAS ONE LARGE SECTION AND WAS APPROX. 18' WIDE, 0-8' THICK AND AT LEAST 50' LONG. THE ROOF WAS A SLICKENED SIDED SLIP (HORSEBACK) THAT WAS UNDETECTABLE AND FELL WITHOUT WARNING. INTERVEIWS WERE HELD WITH PERSONS WHO FOUND AND RECOVERED THE VICTIM, BUT IT COULD NOT BE DETERMINED THE REASON FOR TRAVELING BEYOND THE LAST ROW OF BOLTS.
FATALLY INJURED3EMPLOYEE WAS EITHER IN A ROTARY BREAKER AND THE BREAKER STARTED UP THROUGH A SHORT IN THE POWER OR HEWAS CLEARING THE ENTRANCE CHUTE TO THE ROTARY BREAKER FROM THE FEED BELTS A ND FELL INTO TH9 BREAKER WHILE IT WAS IN MOTION.
INJURED WAS POS3TIONED BETWEEN TWO CARS (RAILROAD) WHILE COUPLING WAS BEING MADE BY LOCOMOTIVE. EITHER COUPLINGS MISSED OR RAIL BROKE CAUSING COUPLINGS TO MISS AND VICTIM WAS CAUGHT BETWEEN TH E TWO CARS. CRU8HING CHEST INJURIES.
POSSIBLE FATAL INJURY OCCURRED WHILE THE DECEASED WAS EVIDENTLY MAKING A SPLICE IN A CABLE.
